Effect of Adolescent-Friendly Clinics on Sexual and Reproductive Health Service Uptake among Adolescents in Nigeria

 

Abstract

Adolescents have distinct sexual and reproductive health needs, yet many face barriers to accessing appropriate healthcare services. Limited privacy, fear of judgment, concerns about confidentiality, unfriendly attitudes from healthcare workers, inconvenient service hours, inadequate information, cost of services, and concerns about stigma may discourage adolescents from seeking sexual and reproductive health services. Adolescent-friendly clinics are designed to provide accessible, acceptable, equitable, appropriate, effective, and confidential healthcare services that respond to the specific needs of adolescents. Such clinics may provide sexual and reproductive health education, contraceptive counselling and services, sexually transmitted infection prevention and management, HIV testing and counselling, menstrual health services, pregnancy testing, and referrals where necessary. Sexual and reproductive health service uptake refers to adolescents' utilization of available services within a defined period. In Nigeria, improving access to adolescent-friendly healthcare may increase young people's willingness and ability to seek appropriate sexual and reproductive health services. Against this background, this study investigates the effect of adolescent-friendly clinics on sexual and reproductive health service uptake among adolescents in Nigeria. The study will be anchored on Andersen's Behavioral Model of Health Services Use, the Health Belief Model, and the Health Systems Framework. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how accessible and acceptable healthcare facilities may influence adolescents' use of services. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action. The Health Systems Framework emphasizes service delivery, health workforce, health information, essential medicines, financing, and accessibility as important components of effective healthcare delivery. Collectively, these theoretical perspectives provide a suitable framework for explaining how adolescent-friendly clinics may influence sexual and reproductive health service uptake among adolescents in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from adolescents aged 10–19 years attending selected adolescent-friendly clinics and comparable healthcare facilities across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible adolescent participants. Adolescent-friendly clinic services will be assessed using indicators such as privacy, confidentiality, respectful treatment, convenient operating hours, accessible location, availability of trained healthcare workers, waiting time, affordability, availability of sexual and reproductive health information, contraceptive services, STI services, HIV counselling and testing, menstrual health services, pregnancy-related services, and referral support. Sexual and reproductive health service uptake will be assessed using indicators such as use of contraceptive services, STI testing or treatment, HIV testing and counselling, sexual and reproductive health counselling, menstrual health services, pregnancy testing, and other relevant services accessed within a specified reference period. Structured questionnaires, clinic attendance registers, service utilization records, and facility assessment checklists will be used for data collection where appropriate. Descriptive statistics will be used to summarize participants' characteristics, adolescent-friendly clinic characteristics, and patterns of sexual and reproductive health service uptake. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of adolescent-friendly clinics on service uptake. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that access to adolescent-friendly clinics has a significant positive effect on sexual and reproductive health service uptake among adolescents in Nigeria. Adolescents who have access to clinics that provide confidential, respectful, affordable, and youth-appropriate services are expected to demonstrate greater utilization of sexual and reproductive health services than adolescents who use conventional healthcare facilities without adolescent-friendly characteristics. Privacy and confidentiality may increase adolescents' confidence in seeking sensitive health services, while respectful healthcare-worker attitudes may reduce fear and perceived stigma. Convenient operating hours and accessible locations may further reduce practical barriers to service utilization. Availability of comprehensive sexual and reproductive health information and services may also encourage adolescents to seek preventive and curative care when needed. Conversely, inadequate privacy, unfriendly healthcare-worker attitudes, long waiting times, limited service availability, insufficient confidentiality, and high costs may discourage adolescents from using available services. However, adolescent-friendly clinics alone may not fully determine service uptake because parental or family support, socioeconomic circumstances, cultural beliefs, education, knowledge of available services, peer influence, and perceived need for healthcare may also affect adolescents' healthcare-seeking behaviour. The study is expected to contribute to the literature on adolescent-friendly healthcare, sexual and reproductive health, adolescent health, healthcare utilization, reproductive health services, community health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, adolescent health programme managers, healthcare workers, schools, youth organizations, development partners, and policymakers regarding strategies for improving adolescent access to appropriate health services. The study will also provide evidence-based recommendations for expanding adolescent-friendly clinics, strengthening privacy and confidentiality, training healthcare workers in adolescent-responsive care, improving convenient service hours, ensuring availability of essential reproductive health commodities, reducing financial barriers, strengthening adolescent health education, and integrating comprehensive adolescent sexual and reproductive health services into primary healthcare programmes across Nigeria.
