Effect of Antenatal Education on Knowledge of Pregnancy Danger Signs among Pregnant Women in Nigeria

Abstract

Knowledge of pregnancy danger signs is an important component of maternal health because early recognition of warning symptoms can encourage pregnant women to seek timely professional care and reduce delays in the management of pregnancy-related complications. Pregnancy danger signs may include severe vaginal bleeding, severe abdominal pain, severe headache, blurred vision, convulsions, difficulty breathing, fever, reduced or absent fetal movement, and other symptoms requiring prompt medical assessment. Despite the importance of recognizing these signs, some pregnant women in Nigeria may have inadequate knowledge of pregnancy danger signs because of limited antenatal health education, low health literacy, cultural beliefs, misinformation, and inadequate communication between healthcare providers and pregnant women. Antenatal education provides an opportunity to give pregnant women accurate and repeated information about pregnancy danger signs, their significance, and the appropriate actions to take when they occur. Against this background, this study investigates the effect of antenatal education on knowledge of pregnancy danger signs among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Adult Learning Theory. The Health Belief Model explains how pregnant women's perceptions of susceptibility to pregnancy complications, perceived severity of danger signs, perceived benefits of early recognition, perceived barriers, and cues to action may influence their knowledge and responses to pregnancy warning symptoms. Social Cognitive Theory emphasizes observational learning, self-efficacy, social interaction, reinforcement, and environmental influences in developing appropriate maternal health knowledge and behaviours. Adult Learning Theory emphasizes relevant, practical, participatory, and experience-based education as important elements in improving learning among adults. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal education may influence knowledge of pregnancy danger signs among pregnant women in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise pregnant women attending selected antenatal clinics in hospitals and primary healthcare centres across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, antenatal clinics, and eligible pregnant women. Antenatal education will be assessed using indicators such as exposure to pregnancy health education, frequency of educational sessions, duration of sessions, content coverage, healthcare-provider counselling, use of visual or printed educational materials, demonstrations, question-and-answer opportunities, individualized counselling, group education, and reinforcement of key maternal health messages. Knowledge of pregnancy danger signs will be assessed using indicators such as recognition of severe vaginal bleeding, severe abdominal pain, severe headache, blurred vision, convulsions, difficulty breathing, fever, reduced or absent fetal movement, and other clinically relevant warning signs. Participants' knowledge may be assessed using a structured knowledge score based on correctly identified danger signs and appropriate responses to those signs. Data will be collected using structured questionnaires, validated maternal health knowledge assessment tools where appropriate, antenatal attendance records, health education records, and relevant maternal health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to antenatal education, and levels of knowledge of pregnancy danger signs. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of antenatal education on knowledge of pregnancy danger signs. Where a quasi-experimental design is adopted, knowledge scores before and after structured antenatal education may be compared, while relevant demographic, socioeconomic, obstetric, educational, and healthcare-access factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that antenatal education has a significant positive effect on knowledge of pregnancy danger signs among pregnant women in Nigeria. Women who receive structured and comprehensive antenatal education are expected to demonstrate higher knowledge scores and better recognition of pregnancy danger signs than women with limited exposure to maternal health education. Repeated education during antenatal visits may improve retention of important information and help women distinguish normal pregnancy changes from symptoms requiring urgent medical attention. Interactive education, practical examples, visual materials, and opportunities to ask questions may further improve understanding and correct misconceptions. However, overcrowded antenatal clinics, limited consultation time, language differences, low literacy levels, inadequate educational materials, shortage of trained health educators, and irregular antenatal attendance may reduce the effectiveness of antenatal education. The study therefore expects accessible, culturally appropriate, participatory, and regularly reinforced antenatal education to contribute significantly to improved knowledge of pregnancy danger signs among pregnant women in Nigeria. The study is expected to contribute to the literature on antenatal education, pregnancy danger-sign knowledge, maternal health literacy, antenatal care, pregnancy health education, obstetric emergency preparedness, maternal healthcare utilization, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, midwives, nurses, obstetricians, maternal health programmes, development partners, and policymakers regarding strategies for strengthening maternal health education. The study will also provide evidence-based recommendations for integrating standardized pregnancy danger-sign education into routine antenatal care, developing simple and culturally appropriate educational materials, training healthcare providers in effective maternal health communication, adapting education to local languages and literacy levels, reinforcing danger-sign messages throughout pregnancy, and strengthening linkages between antenatal education and timely emergency obstetric care across Nigeria.
