Abstract

High-risk pregnancies are an important maternal health concern because undetected pregnancy complications may increase the risk of maternal and fetal morbidity and mortality. Conditions such as placenta previa, multiple pregnancy, abnormal fetal presentation, fetal growth abnormalities, congenital abnormalities, abnormal placental location, and other pregnancy-related complications may require early identification and appropriate monitoring or referral. Antenatal ultrasound services provide an important diagnostic opportunity for assessing fetal development, placental characteristics, pregnancy location, fetal number, and other features that may help identify pregnancies requiring closer clinical attention. However, access to antenatal ultrasound services in Nigeria may be limited by financial constraints, geographical barriers, inadequate equipment, shortage of trained sonographers and healthcare professionals, inconsistent referral practices, and limited awareness among pregnant women. Against this background, this study investigates the effect of antenatal ultrasound services on the detection of high-risk pregnancies in Nigeria. The study will be anchored on Andersen's Behavioral Model of Health Services Use, the Health Belief Model, and the Social Ecological Model. Andersen's Behavioral Model explains how women's predisposing characteristics, enabling resources, and perceived healthcare needs influence utilization of antenatal ultrasound services. The Health Belief Model explains how pregnant women's perceptions of pregnancy-related risks, perceived benefits of ultrasound, perceived barriers, and cues to action may influence their utilization of ultrasound services. The Social Ecological Model emphasizes the interaction between individual, family, community, healthcare-system, and broader environmental factors in determining access to diagnostic maternal healthcare. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal ultrasound services may influence the detection of high-risk pregnancies in Nigeria. The study will adopt a quantitative quasi-experimental, retrospective cohort, or analytical cross-sectional research design, depending on the availability of ultrasound and antenatal records. The study population will comprise pregnant women attending selected antenatal clinics, primary healthcare centres, hospitals, maternity facilities, and diagnostic centres across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible pregnant women. Antenatal ultrasound services will be assessed using indicators such as availability of ultrasound equipment, accessibility of services, frequency of ultrasound examination, timing of examination, availability of trained sonographers or qualified healthcare personnel, quality of ultrasound procedures, referral arrangements, affordability, waiting time, and availability of follow-up services. Detection of high-risk pregnancies will be assessed using indicators such as identification of multiple pregnancies, abnormal fetal presentation, placental abnormalities, fetal growth abnormalities, suspected congenital abnormalities, abnormal amniotic fluid findings, pregnancy-related structural abnormalities, other ultrasound-detected risk conditions, referral for specialist assessment, and documented high-risk pregnancy classification where appropriate. Data will be collected using structured questionnaires, ultrasound reports, antenatal records, maternal health records, referral registers, and relevant healthcare documents. Descriptive statistics will be used to summarize participants' characteristics, ultrasound utilization, findings, and patterns of high-risk pregnancy detection. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of antenatal ultrasound services on high-risk pregnancy detection. Where a quasi-experimental design is adopted, detection rates before and after implementation or expansion of ultrasound services may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that antenatal ultrasound services have a significant positive effect on the detection of high-risk pregnancies in Nigeria. Pregnant women with access to appropriate antenatal ultrasound services are expected to have a higher likelihood of having previously unrecognized pregnancy-related risk conditions identified than women without access to such services. Early identification of high-risk pregnancies may facilitate closer antenatal monitoring, timely referral to appropriate specialists, preparation for delivery in suitable healthcare facilities, and appropriate management of identified maternal or fetal conditions. Ultrasound services may also improve clinical decision-making by providing healthcare providers with relevant information about fetal number, growth, presentation, placental location, and other pregnancy characteristics. However, limited availability of functional ultrasound equipment, shortage of trained personnel, high service costs, geographical barriers, equipment maintenance problems, and inadequate referral systems may reduce the effectiveness of ultrasound services. The study therefore expects accessible, affordable, quality-assured, and appropriately integrated antenatal ultrasound services to contribute significantly to improved detection and management of high-risk pregnancies in Nigeria. The study is expected to contribute to the literature on antenatal ultrasound services, high-risk pregnancy detection, maternal healthcare, antenatal care, obstetric ultrasound, pregnancy monitoring, maternal and fetal health, diagnostic healthcare services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, diagnostic centres, obstetricians, radiologists, sonographers, midwives, nurses, maternal health organizations, development partners, and policymakers regarding strategies for strengthening antenatal diagnostic services. The study will also provide evidence-based recommendations for improving access to quality antenatal ultrasound services, expanding availability of functional ultrasound equipment, strengthening training and deployment of qualified personnel, improving referral systems, reducing financial and geographical barriers, and integrating appropriate ultrasound services into routine antenatal care across Nigeria.
