Effect of Breast Health Education on Clinical Breast Examination Practices among Women of Reproductive Age in Nigeria

Abstract

Breast health is an important component of women's health, particularly among women of reproductive age who may experience various breast-related conditions. Breast problems may include benign breast conditions, infections, hormonal changes, and breast cancer. Early recognition and appropriate clinical evaluation of breast abnormalities can facilitate timely diagnosis and management. However, women in Nigeria may have limited knowledge about breast health, breast abnormalities, appropriate healthcare-seeking, and the role of clinical breast examination (CBE). Factors such as fear of cancer, misconceptions, cultural beliefs, limited access to healthcare services, financial constraints, and inadequate health information may also discourage women from seeking appropriate breast health assessment. Breast health education provides an opportunity to improve women's knowledge and encourage appropriate clinical breast examination practices. Against this background, this study investigates the effect of breast health education on clinical breast examination practices among women of reproductive age in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Self-Efficacy Theory. The Health Belief Model explains how women's perceptions of their susceptibility to breast disease, perceived severity, perceived benefits of clinical breast examination, perceived barriers, and cues to action may influence their breast health practices. Social Cognitive Theory emphasizes observational learning, social support, behavioural reinforcement, environmental influences, and self-regulation in the adoption of appropriate breast health behaviours. Self-Efficacy Theory emphasizes women's confidence in their ability to recognize breast abnormalities, seek professional assessment, communicate breast health concerns, and participate in recommended clinical breast examinations. Collectively, these theoretical perspectives provide a suitable framework for explaining how breast health education may influence clinical breast examination practices among women of reproductive age in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise women of reproductive age attending selected primary healthcare centres, hospitals, women's health clinics, community health programmes, workplaces, and other relevant settings across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible women. Breast health education will be assessed using indicators such as exposure to structured educational sessions, frequency and duration of education, information on breast health, breast abnormalities, breast cancer risk factors, warning signs, appropriate healthcare-seeking, benefits and limitations of clinical breast examination, recommended professional breast assessment, referral services, and available breast health resources. Clinical breast examination practices will be assessed using indicators such as awareness of CBE, history of undergoing CBE, frequency of professional breast examination, willingness to undergo CBE, attendance at healthcare facilities for breast assessment, compliance with professional recommendations, and follow-up after abnormal findings. Where appropriate, clinical records and healthcare-provider documentation will be used to verify reported CBE utilization. Data will be collected using structured questionnaires, validated breast health knowledge and practice assessment instruments, healthcare records, screening registers, and referral records. Descriptive statistics will be used to summarize participants' characteristics, exposure to breast health education, and patterns of CBE practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of breast health education on clinical breast examination practices. Where a quasi-experimental design is adopted, CBE practice scores before and after the educational intervention will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that breast health education has a significant positive effect on clinical breast examination practices among women of reproductive age in Nigeria. Women exposed to structured and evidence-based breast health education are expected to demonstrate greater awareness of breast health, improved recognition of concerning breast changes, increased willingness to seek professional breast assessment, and greater utilization of clinical breast examination services than women with limited exposure to education. Education may correct misconceptions about breast disease, reduce fear and stigma, and encourage women to seek timely professional evaluation when they notice persistent or unusual breast changes. However, limited availability of trained healthcare professionals, financial barriers, geographical inaccessibility, fear of breast cancer, cultural beliefs, concerns about privacy, and limited availability of breast health services may affect the adoption of appropriate CBE practices. The study therefore expects accessible, culturally appropriate, practical, and continuous breast health education to contribute significantly to improved clinical breast examination practices among women of reproductive age in Nigeria. The study is expected to contribute to the literature on breast health education, clinical breast examination, breast health practices, breast cancer awareness, women's reproductive health, preventive healthcare, early detection, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, breast health clinics, nurses, doctors, community health workers, women's health organizations, development partners, and policymakers regarding strategies for improving breast health among women. The study will also provide evidence-based recommendations for strengthening breast health education, integrating breast health promotion into routine women's healthcare, training healthcare workers to provide appropriate breast health counselling and examination, improving access to professional breast assessment services, developing culturally appropriate educational materials, strengthening referral pathways, and promoting timely clinical evaluation of breast abnormalities among women of reproductive age across Nigeria.
