Effect of Breastfeeding Technique Demonstration on Correct Breastfeeding Positioning among Postpartum Mothers in Nigeria

 

Abstract

Correct breastfeeding positioning is an important component of successful breastfeeding because appropriate positioning and attachment can support effective milk transfer, reduce maternal nipple pain and trauma, and improve breastfeeding comfort and continuation. Postpartum mothers, particularly first-time mothers, may experience difficulties with positioning and attachment because of limited practical knowledge, inadequate breastfeeding support, maternal discomfort, fatigue, misconceptions, or insufficient guidance from healthcare providers. Breastfeeding technique demonstration provides an opportunity for mothers to observe and practice appropriate positioning, attachment, and handling techniques under the guidance of trained healthcare personnel. Against this background, this study investigates the effect of breastfeeding technique demonstration on correct breastfeeding positioning among postpartum mothers in Nigeria. The study will be anchored on Social Cognitive Theory, Self-Efficacy Theory, and the Health Belief Model. Social Cognitive Theory explains how mothers may acquire breastfeeding skills through observation, modelling, guided practice, feedback, and reinforcement. Self-Efficacy Theory emphasizes mothers' confidence in their ability to correctly position and attach their infants and manage breastfeeding challenges. The Health Belief Model explains how mothers' perceptions of the benefits of appropriate breastfeeding techniques, perceived barriers, cues to action, and perceived consequences of incorrect positioning may influence their breastfeeding practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how practical breastfeeding demonstrations may influence correct breastfeeding positioning among postpartum mothers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise postpartum mothers with healthy newborns attending selected hospitals, maternity centres, primary healthcare centres, postnatal clinics, and community maternal and child health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, postnatal clinics, and eligible postpartum mothers. Breastfeeding technique demonstration will be assessed using indicators such as exposure to practical demonstrations, frequency and duration of demonstration sessions, demonstration of maternal positioning, infant positioning, breast support, attachment techniques, hand placement, response to infant feeding cues, supervised practice, corrective feedback, and availability of breastfeeding educational materials. Correct breastfeeding positioning will be assessed using indicators such as appropriate maternal posture, correct infant alignment, infant head and neck positioning, infant body support, infant facing the breast, proper mouth-to-nipple positioning, effective attachment, and observable signs of effective milk transfer. Data will be collected using structured questionnaires, breastfeeding observation checklists, postnatal records, and healthcare-provider documentation. Descriptive statistics will be used to summarize participants' characteristics, exposure to breastfeeding demonstrations, and observed positioning practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of breastfeeding technique demonstration on correct breastfeeding positioning. Where a quasi-experimental design is adopted, breastfeeding positioning scores before and after the demonstration will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that breastfeeding technique demonstration has a significant positive effect on correct breastfeeding positioning among postpartum mothers in Nigeria. Mothers exposed to practical and supervised breastfeeding demonstrations are expected to demonstrate better maternal and infant positioning, improved attachment, and greater confidence in breastfeeding techniques than mothers who receive routine verbal advice alone. Demonstrations may allow mothers to observe correct techniques, practice them under supervision, receive immediate feedback, and correct positioning errors. This practical approach may be particularly beneficial for first-time mothers who have limited previous breastfeeding experience. Improved positioning may also reduce nipple pain and trauma and support more effective breastfeeding. However, maternal fatigue, postoperative discomfort, cultural practices, limited availability of trained lactation personnel, inadequate postnatal support, overcrowded healthcare facilities, and early discharge may limit the effectiveness of breastfeeding demonstrations. The study therefore expects practical, culturally appropriate, individualized, and repeated breastfeeding technique demonstrations to contribute significantly to improved breastfeeding positioning among postpartum mothers in Nigeria. The study is expected to contribute to the literature on breastfeeding technique demonstration, breastfeeding positioning, breastfeeding attachment, postpartum maternal care, lactation support, exclusive breastfeeding, maternal and child health, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, maternity facilities, nurses, midwives, lactation consultants, community health workers, maternal and child health organizations, development partners, and policymakers regarding strategies for strengthening breastfeeding support. The study will also provide evidence-based recommendations for integrating practical breastfeeding demonstrations into antenatal and postnatal services, training healthcare providers in breastfeeding skills, strengthening lactation support, providing breastfeeding demonstration materials, ensuring supervised practice before discharge, and establishing follow-up breastfeeding support for postpartum mothers across Nigeria.
