Effect of Cervical Cancer Prevention Education on Cervical Cancer Screening Uptake among Women in Nigeria

Abstract

Cervical cancer remains an important public health concern among women in Nigeria and is largely preventable through human papillomavirus (HPV) vaccination, appropriate screening, early detection, and timely treatment of precancerous lesions. Despite the availability of cervical cancer prevention and screening services, many women may not utilize recommended screening because of inadequate knowledge, misconceptions about cervical cancer, fear of screening procedures, concerns about cost and discomfort, limited awareness of available services, cultural beliefs, and restricted access to healthcare facilities. Cervical cancer prevention education provides an opportunity to improve women's understanding of cervical cancer risk factors, warning signs, HPV infection, prevention methods, screening recommendations, available screening methods, and the importance of early detection. Improved knowledge may encourage women to seek and utilize appropriate cervical cancer screening services. Against this background, this study investigates the effect of cervical cancer prevention education on cervical cancer screening uptake among women in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Promotion Model. The Health Belief Model explains how women's perceptions of susceptibility to cervical cancer, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence screening behaviour. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, observational learning, social support, and environmental factors in developing preventive health behaviours. The Health Promotion Model emphasizes individual experiences, perceived benefits and barriers, interpersonal influences, and behavioural factors that influence women's participation in health-promoting activities. Collectively, these theoretical perspectives provide a suitable framework for explaining how cervical cancer prevention education may influence cervical cancer screening uptake among women in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise women aged 18 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible women. Cervical cancer prevention education will be assessed using indicators such as exposure to cervical cancer education, frequency and duration of educational sessions, knowledge of cervical cancer risk factors, HPV information, awareness of preventive measures, knowledge of screening methods, information about screening eligibility and recommended intervals, education on the importance of early detection, counselling, referral information, and availability of educational materials. Cervical cancer screening uptake will be assessed using indicators such as awareness and utilization of cervical cancer screening services, previous screening history, current screening participation, completion of recommended screening, type of screening received, access to screening facilities, intention to undergo screening, and documented screening attendance where records are available. Data will be collected using structured questionnaires, validated cervical cancer knowledge and screening assessment tools where appropriate, healthcare facility screening registers, screening records, health education records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to prevention education, cervical cancer knowledge, and screening uptake. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of cervical cancer prevention education on screening uptake. Where a quasi-experimental design is adopted, screening knowledge and uptake before and after structured cervical cancer prevention education may be compared, while relevant demographic, socioeconomic, educational, and healthcare-access factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that cervical cancer prevention education has a significant positive effect on cervical cancer screening uptake among women in Nigeria. Women exposed to structured and evidence-based cervical cancer prevention education are expected to demonstrate greater knowledge of cervical cancer and higher utilization of appropriate screening services than women with limited or no exposure to such education. Education may improve understanding of HPV infection and cervical cancer risk, reduce misconceptions and fear surrounding screening, increase awareness of available screening services, and encourage women to seek preventive healthcare. However, financial barriers, distance to screening facilities, cultural beliefs, fear of positive results, concerns about privacy and discomfort, limited availability of screening services, and inadequate referral and follow-up systems may reduce the translation of knowledge into screening uptake. The study therefore expects accessible, culturally appropriate, sustained, and service-linked cervical cancer prevention education to contribute significantly to improved cervical cancer screening uptake among women in Nigeria. The study is expected to contribute to the literature on cervical cancer prevention education, cervical cancer screening, screening uptake, HPV prevention, women's health, cancer prevention, early detection, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, teaching hospitals, primary healthcare centres, cancer control programmes, healthcare professionals, women's health organizations, development partners, and policymakers regarding strategies for improving cervical cancer prevention and early detection. The study will also provide evidence-based recommendations for expanding community-based cervical cancer education, strengthening screening awareness, reducing informational and cultural barriers, improving access to affordable screening services, strengthening referral and follow-up systems, integrating cervical cancer education into primary healthcare and women's health programmes, and linking health education activities directly with accessible cervical cancer screening services across Nigeria.
