Effect of Community Ambulance Services on Emergency Referral Completion in Rural Nigeria

Abstract

Timely access to emergency healthcare is essential for preventing avoidable complications, disability, and death, particularly in rural communities where healthcare facilities may be geographically distant and transportation options are often limited. Emergency referral is an important component of the healthcare system because patients with conditions requiring specialized or higher-level care must be transferred promptly from lower-level facilities or communities to appropriate referral centres. However, emergency referrals in rural Nigeria may be delayed or incomplete because of inadequate transportation, poor road infrastructure, financial constraints, long distances, limited ambulance availability, and weak coordination between referring and receiving facilities. Community ambulance services provide organized emergency transportation for patients requiring urgent referral and may improve the ability of rural communities and primary healthcare facilities to transfer patients safely and promptly. Emergency referral completion refers to the successful transfer of a patient from the referring point to the designated receiving healthcare facility within the required period and according to the referral plan. Against this background, this study investigates the effect of community ambulance services on emergency referral completion in rural Nigeria. The study will be anchored on Andersen's Behavioral Model of Health Services Use, the Health Systems Framework, and Donabedian's Structure-Process-Outcome Model. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how transportation availability may influence access to referral services. The Health Systems Framework emphasizes service delivery, health workforce, health information, transportation and medical technologies, and access to essential healthcare services as important components of an effective health system. Donabedian's Model explains how healthcare structures and processes influence service outcomes and provides a basis for examining how community ambulance infrastructure and referral processes may affect referral completion. Collectively, these theoretical perspectives provide a suitable framework for explaining how community ambulance services may influence emergency referral completion in rural Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from selected rural primary healthcare facilities, community ambulance service providers, and patients or caregivers who have utilized emergency referral services across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, rural communities, primary healthcare facilities, and eligible participants. Community ambulance services will be assessed using indicators such as ambulance availability, number of functional ambulances, service operating hours, response time, geographical coverage, availability of trained drivers or emergency personnel, communication systems, fuel availability, maintenance practices, emergency call systems, and coordination between ambulance providers and healthcare facilities. Emergency referral completion will be assessed using indicators such as successful arrival at the receiving facility, referral completion within the required time, transfer of the patient with appropriate referral documentation, communication between referring and receiving facilities, arrival without referral cancellation or abandonment, and appropriate continuation of care at the receiving facility. Structured questionnaires, facility assessment checklists, ambulance service records, and referral registers will be used for data collection. Descriptive statistics will be used to summarize participants' characteristics, ambulance service availability, and referral completion patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of community ambulance services on emergency referral completion. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that the availability and effective operation of community ambulance services have a significant positive effect on emergency referral completion in rural Nigeria. Rural patients who have access to functional community ambulance services are expected to have a greater likelihood of completing emergency referrals than patients who depend on informal or privately arranged transportation. Ambulance services may reduce transportation delays, improve coordination between communities and healthcare facilities, and facilitate safer movement of patients requiring urgent medical attention. Shorter response and transfer times may also increase the likelihood that patients reach appropriate referral facilities before their conditions deteriorate. Conversely, inadequate ambulance coverage, frequent vehicle breakdowns, fuel shortages, poor road conditions, long distances, and weak communication systems may reduce referral completion and contribute to delays in receiving appropriate care. However, community ambulance services alone may not fully determine emergency referral completion because receiving-facility capacity, referral decision-making, healthcare costs, road infrastructure, communication networks, availability of healthcare workers, and the severity of patients' conditions may also influence referral outcomes. The study is expected to contribute to the literature on emergency referral systems, community ambulance services, rural healthcare, emergency medical transportation, primary healthcare, maternal and child health, health-services research, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare facilities, emergency medical service providers, community health organizations, development partners, and policymakers regarding strategies for strengthening rural emergency referral systems. The study will also provide evidence-based recommendations for expanding community ambulance coverage, ensuring availability of functional ambulances, improving ambulance maintenance and fuel management, strengthening emergency communication systems, training ambulance personnel, improving coordination between referring and receiving facilities, and integrating community ambulance services into rural primary healthcare and emergency referral systems across Nigeria.
