Effect of Community-Based Blood Donation Education on Voluntary Blood Donation Practices among Adults in Nigeria
Abstract
Adequate availability of safe blood is an essential component of effective healthcare delivery and emergency medical services. Blood is required for the management of obstetric complications, severe anaemia, trauma, surgical conditions, childhood illnesses, cancer treatment, and other medical emergencies. However, shortages of voluntary blood donors may limit the availability of safe blood for patients requiring transfusion in Nigeria. Factors such as inadequate knowledge, fear of blood donation, misconceptions about its health effects, concerns about pain or weakness, cultural beliefs, lack of awareness of donation requirements, and limited access to convenient blood donation centres may discourage adults from donating voluntarily. Community-based blood donation education provides an opportunity to improve knowledge about the importance, safety, eligibility requirements, and benefits of voluntary blood donation while addressing common misconceptions and encouraging community participation. Against this background, this study investigates the effect of community-based blood donation education on voluntary blood donation practices among adults in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Social Ecological Model. The Health Belief Model explains how adults' perceptions of susceptibility, perceived benefits, perceived barriers, perceived severity, and cues to action may influence their willingness to donate blood. Social Cognitive Theory emphasizes observational learning, modelling, self-efficacy, reinforcement, and social interaction in shaping voluntary blood donation behaviour. The Social Ecological Model highlights the influence of individual, interpersonal, community, organizational, and broader environmental factors on access to and participation in blood donation programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based blood donation education may influence voluntary blood donation practices among adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above residing in selected urban, semi-urban, and rural communities across Nigeria who meet applicable blood-donation eligibility requirements. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible participants. Community-based blood donation education will be assessed using indicators such as frequency and duration of educational sessions, information on blood donation eligibility, donation procedures, safety requirements, benefits of voluntary donation, management of common concerns, myths and misconceptions addressed, community mobilization activities, availability of educational materials, and exposure to trained health educators or blood-donation personnel. Voluntary blood donation practices will be assessed using indicators such as history of voluntary blood donation, frequency of donation, recent voluntary donation, willingness to donate in the future, participation in community blood drives, repeat donation, compliance with donor eligibility requirements, attendance at blood-donation centres, and intention to encourage others to donate. Data will be collected using structured questionnaires, blood-donation knowledge and practice assessment tools, blood donor registers, donation records where available, community mobilization records, and relevant blood-donation programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to community-based education, blood-donation knowledge, and donation practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of community-based blood donation education on voluntary blood donation practices. Where a quasi-experimental design is adopted, voluntary donation practices before and after the educational intervention may be compared with those of a comparison group or community to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based blood donation education has a significant positive effect on voluntary blood donation practices among adults in Nigeria. Adults exposed to structured community-based blood donation education are expected to demonstrate improved knowledge of blood donation requirements, greater willingness to donate, and higher participation in voluntary blood donation activities than adults without comparable exposure. Education may help correct misconceptions about blood donation, reduce fear and anxiety, clarify eligibility requirements, and improve understanding of the importance of regular voluntary donation. Community-based education may also increase awareness of local blood-donation opportunities and encourage social support for voluntary donation. Repeated education and community mobilization may be particularly useful in encouraging first-time donors to become repeat voluntary donors. However, inadequate access to blood-donation centres, inconvenient donation schedules, fear of needles, cultural beliefs, misconceptions, concerns about weakness after donation, limited community mobilization, and negative previous experiences may reduce the effectiveness of educational interventions. The study therefore expects accessible, culturally appropriate, practical, and regularly conducted community-based blood donation education to contribute significantly to improved voluntary blood donation practices among adults in Nigeria. The study is expected to contribute to the literature on voluntary blood donation, blood safety, community health education, health promotion, blood transfusion services, preventive healthcare, community participation, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Agency, state ministries of health, hospitals, blood transfusion centres, primary healthcare facilities, community health workers, blood-donor organizations, development partners, and policymakers regarding strategies for strengthening voluntary blood donation. The study will also provide evidence-based recommendations for integrating blood donation education into community health programmes, strengthening community blood drives, improving public awareness of blood donation eligibility and safety, developing culturally appropriate educational materials, establishing convenient blood-donation opportunities, encouraging repeat voluntary donation, and strengthening collaboration between communities and blood transfusion services to improve the availability of safe blood across Nigeria.
