Effect of Community Health Insurance Enrollment on Out-of-Pocket Healthcare Expenditure among Rural Households in Nigeria

 

Abstract

Out-of-pocket healthcare expenditure remains a major challenge to equitable access to healthcare in Nigeria, particularly among rural households that may have limited and unstable sources of income. High direct payments for healthcare services can discourage timely healthcare utilization, increase household financial burden, and expose families to financial hardship and catastrophic health expenditure. Community health insurance provides a potential mechanism for improving financial protection by pooling healthcare risks and reducing the amount that households pay directly at the point of service. Through community-based financing arrangements, enrolled households may gain access to covered healthcare services while contributing relatively predictable premiums or other forms of prepayment. However, participation in community health insurance remains limited in many rural communities because of inadequate awareness, affordability concerns, limited availability of insurance schemes, low trust, restricted benefit packages, and geographical barriers to enrollment and service utilization. Out-of-pocket healthcare expenditure refers to direct payments made by households for healthcare services, medicines, consultations, investigations, transportation related to care, and other health-related expenses that are not reimbursed through insurance or other financing mechanisms. Against this background, this study investigates the effect of community health insurance enrollment on out-of-pocket healthcare expenditure among rural households in Nigeria. The study will be anchored on the Theory of Health Insurance, Andersen's Behavioral Model of Health Services Use, and the Health Systems Framework. The Theory of Health Insurance explains how risk pooling and prepayment mechanisms can protect households against the financial consequences of illness and reduce direct healthcare payments. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how insurance coverage influences healthcare-seeking behaviour. The Health Systems Framework emphasizes health financing, service delivery, health workforce, medical resources, and accessibility as important components of an effective healthcare system. Collectively, these theoretical perspectives provide a suitable framework for explaining how community health insurance enrollment may influence household healthcare expenditure in rural Nigeria. The study will adopt a quantitative cross-sectional or comparative analytical research design. The study population will comprise rural households in selected communities across Nigeria, including households enrolled in community health insurance schemes and comparable households that are not enrolled. A multistage sampling technique will be used to select states, local government areas, rural communities, and eligible households. Community health insurance enrollment will be measured using indicators such as insurance enrollment status, duration of enrollment, household membership coverage, continuity of enrollment, and utilization of covered services. Out-of-pocket healthcare expenditure will be assessed using household expenditure on consultations, medicines, diagnostic investigations, hospital services, transportation associated with healthcare utilization, and other direct health-related payments over a specified recall period. Additional information will be collected on healthcare utilization, household size, income sources, and healthcare financing patterns. Data will be collected using structured household questionnaires, insurance enrollment records where available, healthcare utilization records, and relevant programme documents. Descriptive statistics will be used to summarize household characteristics, insurance enrollment patterns, and healthcare expenditure. Inferential techniques, including chi-square tests, t-tests or non-parametric comparisons, correlation analysis, and multiple regression analysis, will be used to determine the effect of community health insurance enrollment on out-of-pocket healthcare expenditure. Where appropriate, propensity score methods or other comparative techniques may be used to reduce differences between insured and uninsured households. Diagnostic tests will be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community health insurance enrollment has a significant negative effect on out-of-pocket healthcare expenditure among rural households in Nigeria. Enrolled households are expected to spend less directly on covered healthcare services than households without insurance coverage because insurance reduces the amount payable at the point of service and provides greater financial risk protection. Community health insurance may also encourage earlier healthcare utilization by reducing concerns about immediate healthcare costs. However, the extent of financial protection may depend on the range of services included in the benefit package, availability of participating healthcare facilities, reimbursement arrangements, co-payments, excluded services, medicine availability, and continuity of insurance enrollment. Households may continue to incur substantial out-of-pocket expenditure when required services or medicines are not covered or when insured services are unavailable locally. Limited awareness, weak scheme administration, delayed reimbursement, and low provider participation may also reduce the financial protection provided by community health insurance. The study is expected to contribute to the literature on community health insurance, health financing, out-of-pocket healthcare expenditure, financial risk protection, universal health coverage, rural healthcare, health economics, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Health Insurance Authority, state health insurance agencies, community health insurance programme managers, rural healthcare providers, development partners, and policymakers regarding strategies for reducing the financial burden of healthcare among rural households. The study will also provide evidence-based recommendations for expanding community health insurance enrollment, improving awareness and public trust, strengthening benefit packages, reducing unnecessary co-payments, increasing participation of rural healthcare providers, improving claims administration, ensuring availability of covered medicines and services, and strengthening community-based health financing mechanisms as part of Nigeria's efforts toward universal health coverage.
