Effect of Family Planning Education on Birth-Spacing Practices among Women of Reproductive Age in Nigeria

Abstract

Birth spacing is an important component of reproductive, maternal, newborn, and child health and can contribute to improved health outcomes for women and children. Closely spaced pregnancies may increase the risk of adverse maternal and child health outcomes, including maternal depletion, preterm birth, low birth weight, and other pregnancy-related complications. Family planning education provides women of reproductive age with information about the benefits of birth spacing, available family planning methods, contraceptive effectiveness, possible side effects, eligibility, correct use, management of common concerns, and access to appropriate reproductive health services. Birth-spacing practices refer to behaviours and decisions that enable women and couples to maintain an appropriate interval between pregnancies through informed fertility planning and use of suitable family planning methods. In Nigeria, factors such as inadequate knowledge, misconceptions about contraception, fear of side effects, partner influence, cultural and religious beliefs, limited access to reproductive health services, and concerns about contraceptive effectiveness may affect birth-spacing practices among women of reproductive age. Effective family planning education may improve women's knowledge, confidence, decision-making, and utilization of appropriate family planning methods for birth spacing. Against this background, this study investigates the effect of family planning education on birth-spacing practices among women of reproductive age in Nigeria. The study is anchored on the Health Belief Model, Theory of Planned Behavior, and Social Cognitive Theory. The Health Belief Model explains preventive and reproductive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding women's decisions regarding family planning and birth spacing. The Theory of Planned Behavior explains how attitudes, subjective norms, and perceived behavioural control influence women's intentions and subsequent reproductive health practices. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping reproductive health behaviour. Collectively, these theoretical perspectives provide a suitable framework for explaining how family planning education may influence birth-spacing practices among women of reproductive age in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from women of reproductive age in selected communities and healthcare facilities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, communities, healthcare facilities, and eligible women. Where feasible, selected participant groups or communities will be assigned to intervention and comparison groups. Women in the intervention group will receive structured family planning education covering the importance and benefits of birth spacing, recommended intervals between pregnancies, available contraceptive methods, effectiveness, correct use, possible side effects, management of common concerns, fertility awareness, partner communication, informed choice, and access to appropriate family planning services. Birth-spacing practices will be assessed using indicators such as pregnancy intervals, current and previous contraceptive use, consistency of contraceptive use where applicable, intention to space future pregnancies, and utilization of appropriate family planning services. Descriptive statistics will be used to summarize participants' sociodemographic and reproductive characteristics, family planning knowledge, exposure to education, and birth-spacing practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression or logistic regression analysis, will be used to determine the effect of family planning education on birth-spacing practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that family planning education has a significant positive effect on birth-spacing practices among women of reproductive age in Nigeria. Women exposed to structured family planning education are expected to demonstrate improved knowledge of contraceptive methods and greater awareness of the maternal and child health benefits of appropriate birth spacing. Education may encourage informed contraceptive choices, improve utilization of family planning services, increase confidence in discussing reproductive intentions with partners, and promote longer and healthier intervals between pregnancies. However, education alone may not guarantee appropriate birth-spacing practices because women may continue to experience barriers such as contraceptive side effects, limited availability of preferred methods, financial constraints, partner opposition, misconceptions, cultural influences, and inadequate access to quality reproductive health services. Consequently, family planning education is expected to be most effective when supported by accessible, affordable, confidential, and high-quality family planning services with a reliable range of contraceptive options. The study is expected to contribute to the literature on family planning, birth spacing, reproductive health, maternal and child health, health education, and public health in Nigeria by providing empirical evidence on the effect of family planning education on birth-spacing practices among women of reproductive age. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, primary healthcare facilities, reproductive health programmes, family planning providers, community health workers, development partners, and policymakers regarding strategies for improving healthy birth-spacing practices. The study will also provide evidence-based recommendations for strengthening family planning education, improving contraceptive counselling, addressing misconceptions and concerns about contraceptive methods, expanding access to quality family planning services, strengthening community-based reproductive health programmes, and promoting informed birth-spacing practices among women of reproductive age across Nigeria.
