Effect of Food Fortification Education on Micronutrient-Rich Food Consumption among Women of Reproductive Age in Nigeria
Abstract
Micronutrient deficiencies remain an important public health concern in Nigeria, particularly among women of reproductive age who have increased nutritional requirements and may be vulnerable to deficiencies of iron, folate, vitamin A, iodine, zinc, and other essential micronutrients. Inadequate intake of micronutrient-rich foods may contribute to anaemia, impaired immunity, poor maternal health, adverse pregnancy outcomes, and reduced quality of life. Food fortification is an important nutrition intervention through which essential vitamins and minerals are added to commonly consumed foods to improve their nutritional value and reduce micronutrient deficiencies. Food fortification education provides women with information about fortified foods, their nutritional benefits, methods of identifying fortified products, appropriate food choices, dietary diversity, and the importance of consuming foods that provide essential micronutrients. Micronutrient-rich food consumption refers to the regular intake of foods naturally rich in or fortified with essential vitamins and minerals, including vegetables, fruits, legumes, animal-source foods, fortified cereals, fortified flour, and other appropriately fortified staple foods. In Nigeria, limited knowledge of food fortification, inadequate awareness of fortified products, food affordability, dietary preferences, and limited access to nutritious foods may influence dietary practices among women of reproductive age. Effective food fortification education may improve nutritional knowledge and encourage greater consumption of micronutrient-rich foods. Against this background, this study investigates the effect of food fortification education on micronutrient-rich food consumption among women of reproductive age in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding women's decisions to consume micronutrient-rich and fortified foods. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping dietary behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from food fortification education may influence dietary practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how food fortification education may influence micronutrient-rich food consumption among women of reproductive age in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from women of reproductive age in selected communities, healthcare facilities, workplaces, markets, and other appropriate settings in Nigeria using a structured interviewer-administered questionnaire and an appropriate dietary assessment instrument. A multistage sampling technique will be employed to select states, local government areas, communities, facilities, and eligible participants. Where feasible, selected participant groups or communities will be assigned to intervention and comparison groups. Women in the intervention group will receive structured food fortification education covering micronutrient deficiencies, the importance of essential vitamins and minerals, food fortification concepts, benefits of fortified foods, identification of fortified food products, dietary diversity, appropriate food selection, food preparation practices, and the importance of consuming micronutrient-rich foods. Micronutrient-rich food consumption will be assessed using indicators such as dietary diversity, frequency of consumption of fruits and vegetables, legumes, animal-source foods, fortified staple foods, fortified cereals, and other relevant micronutrient-rich food groups. Descriptive statistics will be used to summarize participants' sociodemographic characteristics, nutritional knowledge, exposure to education, and dietary practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the effect of food fortification education on micronutrient-rich food consumption. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that food fortification education has a significant positive effect on micronutrient-rich food consumption among women of reproductive age in Nigeria. Women exposed to structured food fortification education are expected to demonstrate improved knowledge of micronutrients, greater awareness of fortified food products, and increased consumption of micronutrient-rich foods. Education may improve dietary diversity, encourage greater consumption of fruits and vegetables, legumes, animal-source foods, and appropriately fortified staple foods, and increase women's ability to identify and select fortified food products. However, education alone may not guarantee sustained dietary improvement because women may continue to face barriers such as food prices, household income, availability of nutritious foods, cultural food preferences, seasonal variation, and limited access to fortified products. Consequently, food fortification education is expected to be most effective when supported by affordable nutritious foods, adequate availability of fortified products, appropriate food labelling, nutrition-sensitive policies, and broader community-based nutrition interventions. The study is expected to contribute to the literature on food fortification, micronutrient nutrition, dietary practices, maternal and reproductive health, nutrition education, and public health in Nigeria by providing empirical evidence on the effect of food fortification education on micronutrient-rich food consumption among women of reproductive age. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Agency for Food and Drug Administration and Control (NAFDAC), National Primary Health Care Development Agency (NPHCDA), nutrition programmes, state ministries of health, healthcare providers, nutritionists, food manufacturers, community health workers, development partners, and policymakers regarding strategies for improving micronutrient intake among women. The study will also provide evidence-based recommendations for strengthening nutrition education, improving awareness and identification of fortified foods, promoting dietary diversity, increasing access to affordable micronutrient-rich foods, strengthening food fortification programmes, and promoting healthy dietary practices among women of reproductive age across Nigeria.
