Effect of Health Facility Accreditation on Quality of Healthcare Services in Nigerian Hospitals

 

Abstract

Health facility accreditation is an important mechanism for promoting quality improvement, patient safety, standardization, and accountability within healthcare organizations. Accreditation involves the formal assessment of healthcare facilities against established standards relating to clinical practice, patient safety, infrastructure, staffing, infection prevention and control, documentation, governance, medication management, and other aspects of healthcare delivery. Quality of healthcare services refers to the extent to which healthcare services are safe, effective, timely, patient-centred, equitable, efficient, and responsive to patients' healthcare needs. In Nigeria, variations in healthcare infrastructure, staffing, clinical practices, infection prevention, documentation, patient safety systems, and institutional management may affect the quality of services provided in hospitals. Health facility accreditation may provide a structured framework for identifying service gaps, enforcing standards, strengthening quality-improvement processes, and promoting continuous monitoring of healthcare delivery. However, the extent to which accreditation contributes to measurable improvements in healthcare quality remains an important empirical question. Against this background, this study investigates the effect of health facility accreditation on the quality of healthcare services in Nigerian hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, Total Quality Management Theory, and Institutional Theory. Donabedian's Model provides a framework for examining how accreditation-related improvements in healthcare structures and processes may influence healthcare outcomes and quality. Total Quality Management Theory emphasizes continuous improvement, adherence to standards, staff participation, customer or patient focus, process improvement, and organizational commitment to quality. Institutional Theory explains how healthcare organizations adopt formal standards and practices in response to regulatory requirements, professional expectations, and pressures for legitimacy and improved performance. Collectively, these theoretical perspectives provide a suitable framework for explaining how health facility accreditation may influence the quality of healthcare services in Nigerian hospitals. The study will adopt a quantitative comparative cross-sectional research design. Primary data will be collected from selected accredited and non-accredited hospitals in Nigeria using structured questionnaires, hospital quality-assessment checklists, patient-experience instruments, and relevant facility records. A multistage sampling technique will be employed to select states, hospitals, healthcare departments, healthcare workers, and patients. Health facility accreditation will be assessed based on accreditation status, duration of accreditation, compliance with applicable standards, frequency of accreditation assessment, and evidence of corrective actions arising from accreditation reviews. Quality of healthcare services will be assessed using indicators such as patient safety practices, infection prevention and control, availability and functionality of essential equipment, clinical documentation, medication management, waiting time, responsiveness, continuity of care, patient-centred care, adherence to clinical standards, patient satisfaction, and selected service outcomes. Where feasible, hospital records, accreditation reports, quality-improvement documents, incident reports, and direct facility observations will complement questionnaire responses. Descriptive statistics will be used to summarize hospital characteristics, accreditation status, compliance levels, healthcare-worker characteristics, and service-quality indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, independent-sample tests, and multiple regression analysis, will be used to determine the effect of health facility accreditation on healthcare service quality. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that health facility accreditation has a significant positive effect on the quality of healthcare services in Nigerian hospitals. Accredited hospitals are expected to demonstrate stronger compliance with established clinical and organizational standards, improved patient safety practices, better infection prevention and control, stronger documentation, improved medication-management processes, and more systematic quality-improvement activities than non-accredited hospitals. Accreditation is also expected to encourage hospital management and healthcare workers to identify service deficiencies, implement corrective measures, monitor performance, and sustain continuous quality improvement. Patients receiving care in accredited facilities are therefore expected to report better perceptions of service quality and patient-centred care. However, accreditation alone may not guarantee sustained quality improvement because the effectiveness of accreditation may depend on the seriousness of compliance monitoring, availability of resources, management commitment, healthcare-worker participation, frequency of assessment, and implementation of corrective actions. Consequently, accreditation should be supported by adequate financing, effective regulation, continuous quality monitoring, staff development, and strong institutional leadership. The study is expected to contribute to the literature on health facility accreditation, healthcare quality, hospital management, patient safety, quality improvement, health-system strengthening, healthcare governance, and public health in Nigeria by providing empirical evidence on the relationship between accreditation and healthcare service quality. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, regulatory and accreditation bodies, hospital management boards, public and private hospitals, healthcare professional organizations, quality-improvement teams, healthcare workers, development partners, and policymakers regarding strategies for strengthening healthcare quality. The study will also provide evidence-based recommendations for improving accreditation standards, strengthening monitoring and enforcement mechanisms, supporting hospitals to address identified quality gaps, improving healthcare-worker training, strengthening patient-safety systems, promoting continuous quality improvement, and ensuring that accreditation produces sustained improvements in the quality and safety of healthcare services in Nigerian hospitals.
