Effect of Health Worker Training on Infection Prevention Practices in Nigerian Hospitals
Abstract
Healthcare-associated infections remain an important public health and patient-safety concern because they can increase morbidity, mortality, length of hospital stay, healthcare costs, and antimicrobial use. Effective infection prevention and control practices are essential for reducing the transmission of infectious agents within healthcare facilities and protecting patients, healthcare workers, visitors, and communities. Health worker training refers to structured educational, practical, and competency-based activities designed to improve healthcare workers' knowledge and skills in infection prevention and control. Infection prevention practices refer to the routine measures adopted by healthcare workers to prevent the transmission of infectious agents within healthcare settings. These practices include hand hygiene, appropriate use of personal protective equipment, safe injection practices, proper handling and disposal of healthcare waste, environmental cleaning, sterilization and disinfection, respiratory hygiene, safe handling of specimens, appropriate isolation procedures, and adherence to standard precautions. In Nigerian hospitals, inadequate training, irregular refresher education, limited practical exercises, shortages of infection prevention resources, heavy workloads, inadequate supervision, and inconsistent implementation of infection prevention policies may affect compliance with recommended practices. Regular and competency-based health worker training may improve knowledge, practical skills, awareness of infection risks, and adherence to standard infection prevention procedures. Against this background, this study investigates the effect of health worker training on infection prevention practices in Nigerian hospitals. The study is anchored on Human Capital Theory, the Health Belief Model, and Donabedian's Structure-Process-Outcome Model. Human Capital Theory views training as an investment in knowledge and skills that improves individual competence and organizational performance. The Health Belief Model explains preventive behaviours through perceptions of susceptibility to infection, severity of consequences, benefits of preventive actions, perceived barriers, and cues to action. Donabedian's Model identifies healthcare workforce capacity, infrastructure, equipment, and organizational resources as structural factors that influence healthcare processes and outcomes. Collectively, these theoretical perspectives provide a suitable framework for explaining how health worker training may influence infection prevention practices in Nigerian hospitals. The study will adopt a quantitative quasi-experimental research design. Healthcare workers will be selected from participating Nigerian hospitals and assessed before and after exposure to a structured infection prevention and control training programme. A multistage sampling technique will be employed to select states, hospitals, departments, professional categories, and eligible healthcare workers. The training programme will cover standard precautions, hand hygiene, personal protective equipment, injection safety, healthcare waste management, environmental cleaning, sterilization and disinfection, respiratory hygiene, specimen handling, isolation precautions, occupational exposure prevention, and appropriate management of sharps and infectious materials. Health worker training will be assessed using indicators such as participation in infection prevention training, training frequency, training duration, recency of training, practical demonstrations, simulation exercises, refresher training, and relevance of training content. Infection prevention practices will be assessed using indicators such as hand hygiene compliance, appropriate use of personal protective equipment, safe injection practices, proper segregation and disposal of healthcare waste, safe sharps handling, environmental cleaning, sterilization and disinfection practices, respiratory hygiene, specimen handling, isolation procedures, and compliance with standard precautions. Structured observation checklists will be used where feasible to complement self-reported information and provide more objective measures of infection prevention practices. Descriptive statistics will be used to summarize participants' characteristics, training exposure, and infection prevention practices. Inferential statistical techniques, including paired-sample tests, chi-square tests, correlation analysis, and regression analysis, will be used to determine the effect of health worker training on infection prevention practices. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that structured and regular health worker training has a significant positive effect on infection prevention practices in Nigerian hospitals. Healthcare workers who receive appropriate infection prevention and control training are expected to demonstrate improved compliance with hand hygiene, personal protective equipment use, safe injection practices, healthcare waste management, environmental cleaning, sterilization and disinfection, respiratory hygiene, and other standard precautions. Practical demonstrations and simulation exercises are expected to strengthen the ability of healthcare workers to apply infection prevention procedures correctly in routine clinical situations. Training may also improve awareness of occupational infection risks and encourage healthcare workers to adopt appropriate protective behaviours. Conversely, inadequate or outdated training may contribute to knowledge gaps, inconsistent compliance, inappropriate use of protective equipment, poor hand hygiene, unsafe waste disposal, and other practices that increase infection risks. However, training alone may not fully determine infection prevention practices because compliance is also influenced by availability of personal protective equipment, handwashing facilities, water supply, staffing levels, workload, supervision, leadership commitment, infection prevention policies, and availability of essential supplies. Therefore, training should be integrated with adequate institutional resources and strong infection prevention and control systems. The study is expected to contribute to the literature on health worker training, infection prevention and control, healthcare-associated infection prevention, hospital safety, nursing, epidemiology, and public health in Nigeria by providing empirical evidence on the role of workforce education in improving infection prevention practices. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, state ministries of health, hospital management boards, healthcare facility administrators, infection prevention and control committees, nursing departments, medical and laboratory professional bodies, development partners, and policymakers regarding strategies for strengthening infection prevention. The study will also provide evidence-based recommendations for establishing regular infection prevention training, strengthening refresher programmes, incorporating practical demonstrations and simulation exercises, conducting routine compliance assessments, improving supervision, ensuring adequate infection prevention supplies and infrastructure, strengthening infection prevention and control committees, and integrating infection prevention competencies into continuing professional development programmes for healthcare workers across Nigerian hospitals.
