Effect of Healthcare Facility Security on Patient Satisfaction in Nigerian Public Hospitals

Abstract

Healthcare facility security is an important component of a safe and supportive healthcare environment because patients, healthcare workers, visitors, and hospital property require protection from violence, theft, unauthorized access, harassment, and other security threats. Healthcare facility security refers to the systems, personnel, procedures, infrastructure, and measures established by a hospital to protect patients, staff, visitors, equipment, medicines, records, and other hospital resources. Patient satisfaction refers to the extent to which patients' expectations and perceptions of healthcare services and their overall hospital experience are met. In Nigerian public hospitals, concerns relating to theft, overcrowding, unauthorized access, harassment, violence, poor access control, inadequate security personnel, and insufficient emergency security measures may influence patients' perceptions of safety and the quality of hospital services. A secure hospital environment may promote patients' sense of safety, confidence, privacy, dignity, and trust in the healthcare institution. Conversely, perceived insecurity may generate anxiety, fear, dissatisfaction, and reluctance to use hospital services. Against this background, this study investigates the effect of healthcare facility security on patient satisfaction in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, Maslow's Hierarchy of Needs, and Expectancy-Disconfirmation Theory. Donabedian's Model identifies facility infrastructure, security systems, and organizational resources as structural components that can influence healthcare processes and patient outcomes. Maslow's Hierarchy of Needs emphasizes safety as a fundamental human need, suggesting that patients may be unable to fully appreciate or evaluate healthcare services when they perceive their physical security to be threatened. Expectancy-Disconfirmation Theory explains patient satisfaction as the outcome of comparing patients' expectations concerning healthcare services and the hospital environment with their actual experiences. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare facility security may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected Nigerian public hospitals using structured patient questionnaires, healthcare worker questionnaires, hospital security assessment checklists, incident records, and relevant hospital administrative documents. A multistage sampling technique will be employed to select states, public hospitals, departments or service units, healthcare workers, patients, and hospital security personnel. Healthcare facility security will be assessed using indicators such as availability and adequacy of security personnel, access-control systems, security screening procedures, lighting, surveillance systems, emergency response procedures, visitor-management systems, security signage, protection of patient belongings, incident reporting mechanisms, and security monitoring. Patient satisfaction will be assessed using indicators such as perceived safety, satisfaction with security personnel, privacy and dignity, confidence in the hospital environment, staff responsiveness, service quality, willingness to return to the hospital, willingness to recommend the hospital, and overall satisfaction. Where feasible, hospital security incident records, access-control reports, security duty registers, surveillance records, and documented security procedures will be reviewed to complement patient perceptions. Additional information will be collected on patients' age, sex, educational level, type of service received, frequency of hospital visits, department attended, duration of hospital stay, and previous experience with security incidents. Descriptive statistics will be used to summarize hospital security conditions, patient characteristics, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of healthcare facility security on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that effective healthcare facility security has a significant positive effect on patient satisfaction in Nigerian public hospitals. Patients who perceive hospital environments as safe, well-controlled, and adequately protected are expected to report higher levels of satisfaction than patients who perceive inadequate security or frequent security risks. Adequate security personnel, effective access control, appropriate surveillance, sufficient lighting, and clear emergency-response procedures are expected to strengthen patients' sense of safety and confidence in hospital services. Effective security may also protect patient privacy, reduce fear and anxiety, safeguard personal belongings, and create an environment that supports respectful and patient-centred care. Conversely, inadequate security, poor access control, insufficient security personnel, and frequent security incidents may negatively affect patients' perceptions of hospital quality and reduce satisfaction. However, facility security alone may not fully determine patient satisfaction because satisfaction is also influenced by waiting time, provider communication, availability of medicines and equipment, healthcare costs, clinical outcomes, and physical facility conditions. Therefore, security improvements should form part of a broader strategy for enhancing patient-centred healthcare quality. The study is expected to contribute to the literature on healthcare facility security, patient satisfaction, hospital safety, healthcare quality, patient experience, hospital administration, healthcare management, and public health in Nigeria by providing empirical evidence on the relationship between security conditions and patients' perceptions of hospital services. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, hospital security departments, healthcare quality-improvement teams, healthcare professionals, development partners, and policymakers regarding strategies for improving hospital safety and patient experience. The study will also provide evidence-based recommendations for strengthening hospital security personnel, improving access-control procedures, expanding appropriate surveillance systems, improving lighting and physical security infrastructure, strengthening security incident reporting, developing emergency security-response protocols, training security personnel, and establishing regular monitoring and evaluation of healthcare facility security in Nigerian public hospitals.
