Effect of Healthcare Provider Communication on Patient Satisfaction in Nigerian Public Hospitals
Abstract
Effective communication between healthcare providers and patients is an important component of quality healthcare delivery because it influences patients' understanding of their health conditions, treatment options, healthcare instructions, and overall experiences within healthcare facilities. Healthcare provider communication encompasses the clarity, accuracy, completeness, timeliness, empathy, respect, responsiveness, and interpersonal manner with which healthcare professionals communicate with patients during healthcare encounters. Patient satisfaction refers to the extent to which patients' expectations regarding healthcare services are met, including their perceptions of provider interaction, communication, responsiveness, quality of care, waiting time, privacy, and overall healthcare experience. In Nigeria, communication challenges within public hospitals may arise from high patient volumes, workload pressures, limited consultation time, language differences, inadequate communication skills, poor provider-patient interaction, and insufficient attention to patient concerns. Ineffective communication may result in misunderstanding of diagnoses and treatment instructions, reduced patient trust, poor adherence to recommended care, and dissatisfaction with healthcare services. Conversely, respectful, clear, patient-centred, and empathetic communication may improve patients' understanding, trust, confidence, and satisfaction. Against this background, this study investigates the effect of healthcare provider communication on patient satisfaction in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, Expectancy-Disconfirmation Theory, and Patient-Centred Care Theory. Donabedian's Model provides a framework for understanding healthcare provider communication as an important component of the care process that can influence patient experiences and outcomes. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing patients' expectations of healthcare interactions with their actual experiences. Patient-Centred Care Theory emphasizes respect for patients' preferences, needs, values, participation, information needs, and effective communication between patients and healthcare providers. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare provider communication may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from patients attending selected public hospitals in Nigeria using a structured patient questionnaire and healthcare provider communication assessment scale. A multistage sampling technique will be employed to select states, public hospitals, departments or service units, and eligible patients. Healthcare provider communication will be assessed using indicators such as clarity of explanations, active listening, respect, empathy, courtesy, responsiveness to questions, explanation of diagnosis and treatment, communication of medication instructions, involvement of patients in healthcare decisions, privacy during communication, and opportunity for patients to express concerns. Where feasible, a validated communication observation checklist or structured assessment by trained observers may complement patient-reported measures. Patient satisfaction will be assessed using indicators such as satisfaction with provider interaction, quality of information received, responsiveness, respect, waiting experience, perceived quality of care, confidence in the provider, and overall satisfaction with hospital services. Descriptive statistics will be used to summarize patients' sociodemographic characteristics, healthcare experiences, provider communication patterns, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of healthcare provider communication on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that effective healthcare provider communication has a significant positive effect on patient satisfaction in Nigerian public hospitals. Patients who experience clear, respectful, empathetic, responsive, and patient-centred communication are expected to report higher levels of satisfaction with healthcare services. Clear explanations of diagnoses, treatment options, medication instructions, expected outcomes, and follow-up requirements are expected to improve patients' understanding and confidence in their healthcare providers. Opportunities for patients to ask questions and participate in healthcare decisions are also expected to strengthen trust and improve their overall healthcare experience. Conversely, poor communication, inadequate explanations, dismissive attitudes, limited opportunity for questions, and lack of empathy are expected to contribute to dissatisfaction. However, communication alone may not fully determine patient satisfaction because patients' experiences may also be influenced by waiting time, availability of medicines, cost of care, hospital cleanliness, staffing levels, infrastructure, and perceived clinical outcomes. Consequently, strengthening provider communication should form part of a broader patient-centred quality-improvement strategy. The study is expected to contribute to the literature on healthcare provider communication, patient satisfaction, patient-centred care, healthcare quality, health communication, public hospitals, healthcare management, and health-services delivery in Nigeria by providing empirical evidence on the effect of provider-patient communication on patients' satisfaction. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, healthcare providers, professional healthcare associations, quality-improvement teams, health educators, public health practitioners, and policymakers regarding strategies for strengthening patient-centred healthcare delivery. The study will also provide evidence-based recommendations for incorporating communication-skills training into healthcare professional education and continuing professional development, improving provider-patient interaction, strengthening respectful and empathetic communication, encouraging shared decision-making, improving communication of treatment information, and establishing routine patient-feedback mechanisms in Nigerian public hospitals.
