Effect of Lactation Education on Proper Breastfeeding Techniques among First-Time Mothers in Nigeria

 

Abstract

Breastfeeding is an essential component of maternal and child health, providing infants with appropriate nutrition, immune protection, and other health benefits while supporting maternal wellbeing. Proper breastfeeding techniques, including correct positioning, effective attachment and latch, appropriate breast support, and responsive feeding practices, are important for effective milk transfer and comfortable breastfeeding. First-time mothers may experience difficulties with breastfeeding techniques because of limited practical experience, inadequate knowledge, anxiety, misinformation, cultural beliefs, and insufficient professional support. Poor breastfeeding technique may contribute to nipple pain, breast engorgement, ineffective milk transfer, perceived insufficient milk supply, and premature discontinuation of breastfeeding. Lactation education provides an opportunity to equip first-time mothers with practical knowledge and skills needed to initiate and maintain effective breastfeeding. Against this background, this study investigates the effect of lactation education on proper breastfeeding techniques among first-time mothers in Nigeria. The study will be anchored on Social Cognitive Theory, the Health Belief Model, and Self-Efficacy Theory. Social Cognitive Theory explains how observational learning, behavioural reinforcement, social support, knowledge, and environmental influences may shape breastfeeding behaviours. The Health Belief Model explains how mothers' perceptions of the benefits of proper breastfeeding, perceived barriers, health-related concerns, and cues to action may influence their adoption of appropriate breastfeeding techniques. Self-Efficacy Theory emphasizes mothers' confidence in their ability to correctly position the infant, achieve effective attachment, recognize feeding difficulties, and sustain appropriate breastfeeding practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how lactation education may influence proper breastfeeding techniques among first-time mothers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise first-time mothers who have recently delivered and are attending selected hospitals, primary healthcare centres, maternity clinics, postnatal clinics, and community maternal and child health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible first-time mothers. Lactation education will be assessed using indicators such as exposure to structured breastfeeding education, frequency and duration of education sessions, individualized counselling, practical demonstrations, visual teaching materials, hands-on assistance, information on positioning and attachment, breast care, responsive feeding, recognition of breastfeeding difficulties, and follow-up support. Proper breastfeeding techniques will be assessed using indicators such as appropriate maternal and infant positioning, correct infant attachment and latch, effective suckling, adequate breast support, appropriate feeding frequency, recognition of effective milk transfer, and appropriate response to common breastfeeding difficulties. Direct observation using a standardized breastfeeding assessment checklist will be used where feasible to provide an objective assessment of breastfeeding technique. Data will also be collected using structured questionnaires, observation checklists, maternal and child health records, and relevant breastfeeding-support records. Descriptive statistics will be used to summarize participants' characteristics, exposure to lactation education, and breastfeeding technique scores. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of lactation education on proper breastfeeding techniques. Where a quasi-experimental design is adopted, breastfeeding technique scores before and after the educational intervention will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that lactation education has a significant positive effect on proper breastfeeding techniques among first-time mothers in Nigeria. Mothers exposed to structured and practical lactation education are expected to demonstrate better positioning, improved infant attachment and latch, more effective breastfeeding techniques, and greater confidence in managing common breastfeeding challenges than mothers with limited exposure to lactation education. Practical demonstrations and individualized assistance may be particularly beneficial because they allow mothers to observe and practice correct techniques under professional guidance. Improved breastfeeding skills may also reduce maternal discomfort, improve effective milk transfer, and support continued breastfeeding. However, fatigue after childbirth, maternal discomfort, cultural beliefs, inadequate family support, limited access to trained lactation personnel, misinformation, and competing postpartum responsibilities may affect the adoption and maintenance of appropriate breastfeeding techniques. The study therefore expects accessible, practical, culturally appropriate, and continuous lactation education to contribute significantly to improved breastfeeding techniques among first-time mothers in Nigeria. The study is expected to contribute to the literature on lactation education, breastfeeding techniques, first-time mothers, breastfeeding support, maternal and child health, infant feeding, postnatal care, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, midwives, nurses, lactation consultants, community health workers, maternal and child health organizations, development partners, and policymakers regarding strategies for improving breastfeeding outcomes. The study will also provide evidence-based recommendations for integrating practical lactation education into antenatal and postnatal care, strengthening breastfeeding counselling services, training healthcare providers in breastfeeding support, increasing access to individualized breastfeeding assistance, developing culturally appropriate educational materials, and establishing effective follow-up support systems for first-time mothers across Nigeria.
