Effect of Menopause Education on Menopausal Health-Seeking Practices among Middle-Aged Women in Nigeria
Abstract
Menopause is a natural biological stage in a woman's life that is associated with hormonal changes and may produce a range of physical, psychological, and social experiences. Menopausal symptoms may include hot flashes, night sweats, sleep disturbances, mood changes, vaginal dryness, menstrual changes, and other symptoms that may affect women's quality of life and daily functioning. Appropriate health-seeking practices during menopause are important for identifying symptoms that require professional attention, managing troublesome symptoms, maintaining bone and cardiovascular health, and addressing other age-related health concerns. Menopause education provides women with accurate information about the physiological changes associated with menopause, common symptoms, healthy lifestyle practices, available management options, warning signs requiring medical attention, and appropriate healthcare services. In Nigeria, limited awareness, misconceptions that menopausal symptoms must simply be endured, cultural beliefs, stigma surrounding reproductive health, fear of medical treatment, financial constraints, and limited access to appropriate women's health services may discourage middle-aged women from seeking professional care when necessary. Effective menopause education may improve women's understanding of menopause and encourage appropriate health-seeking practices. Against this background, this study investigates the effect of menopause education on menopausal health-seeking practices among middle-aged women in Nigeria. The study is anchored on the Health Belief Model (HBM), Andersen's Behavioral Model of Health Services Use, and the Knowledge-Attitude-Practice (KAP) framework. The Health Belief Model explains health-seeking behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding women's decisions to seek professional care for menopausal concerns. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for examining how knowledge, access, affordability, and perceived need influence health-seeking behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from menopause education may influence healthcare-seeking practices. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how menopause education may influence menopausal health-seeking practices among middle-aged women in Nigeria. The study adopts a quantitative quasi-experimental research design. Primary data will be collected from middle-aged women in selected communities and healthcare facilities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, communities, healthcare facilities, and eligible participants. Where feasible, selected participants or communities will be assigned to intervention and comparison groups. Women in the intervention group will receive structured menopause education covering the physiological changes associated with menopause, common menopausal symptoms, healthy lifestyle practices, nutrition, physical activity, sleep management, mental well-being, available healthcare and symptom-management options, warning signs requiring medical attention, and the importance of appropriate health-seeking behaviour. Menopausal health-seeking practices will be assessed using indicators such as consultation with qualified healthcare professionals for troublesome symptoms, utilization of appropriate healthcare facilities, participation in recommended health assessments, adherence to professional advice, and timely seeking of care for concerning symptoms. Descriptive statistics will be used to summarize participants' sociodemographic and reproductive characteristics, menopause knowledge, and health-seeking practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple logistic regression analysis, will be employed to determine the effect of menopause education on menopausal health-seeking practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study anticipates that menopause education will have a significant positive effect on menopausal health-seeking practices among middle-aged women in Nigeria. Women who receive structured menopause education are expected to demonstrate improved knowledge of menopausal changes, better recognition of symptoms requiring professional attention, and greater utilization of appropriate healthcare services. Education may reduce misconceptions and stigma surrounding menopause, improve women's confidence in discussing menopausal concerns with healthcare professionals, and encourage timely management of troublesome or concerning symptoms. However, education alone may not guarantee appropriate health-seeking practices because women may continue to experience barriers such as healthcare costs, limited access to specialized women's health services, distance to health facilities, cultural beliefs, reliance on informal remedies, and concerns about treatment options. Consequently, menopause education is expected to be most effective when combined with accessible, affordable, respectful, and age-appropriate healthcare services. This study is expected to contribute to the literature on women's health, menopause, health education, healthcare utilization, reproductive health, and public health in Nigeria by providing empirical evidence on the effect of menopause education on menopausal health-seeking practices among middle-aged women. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, state ministries of health, primary healthcare facilities, hospitals, women's health professionals, community health workers, reproductive health programmes, development partners, and policymakers regarding strategies for improving menopausal health. The study will also provide evidence-based recommendations for strengthening menopause education, integrating menopause counselling into routine women's health services, improving community awareness, addressing cultural misconceptions and stigma, strengthening referral pathways, and increasing access to appropriate and affordable healthcare services for middle-aged women across Nigeria.
