Effect of Nurse-Patient Ratio on Patient Safety in Nigerian Public Hospitals

Abstract

The nurse-patient ratio is an important indicator of nursing workforce adequacy and may have significant implications for the quality and safety of patient care. Nurse-patient ratio refers to the number of patients assigned to or cared for by a nurse within a specified period, particularly during a clinical shift. Patient safety refers to the prevention of avoidable harm, errors, adverse events, and unnecessary risks arising from healthcare delivery. Adequate nurse staffing may enable nurses to provide timely monitoring, medication administration, patient assessment, documentation, communication, and appropriate responses to changes in patients' clinical conditions. In Nigerian public hospitals, inadequate numbers of nurses relative to patient demand, increasing patient volumes, staff shortages, prolonged working hours, emergency admissions, and uneven distribution of nursing personnel may result in high nurse-patient ratios. Excessively high nurse-patient ratios may increase workload, reduce the time available for individual patients, delay nursing interventions, increase the likelihood of missed care, and compromise patient safety. Conversely, adequate nurse staffing may improve patient monitoring, responsiveness, continuity of care, and compliance with patient-safety procedures. Against this background, this study investigates the effect of nurse-patient ratio on patient safety in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, the Nursing Care Quality Model, and Systems Theory. Donabedian's Model identifies staffing levels as an important structural component of healthcare delivery that influences care processes and patient outcomes. The Nursing Care Quality Model emphasizes the relationship between nursing resources, nursing care processes, and patient outcomes, providing a suitable basis for examining how nurse staffing levels may influence safety. Systems Theory views patient safety as the outcome of interactions among nursing personnel, patients, hospital management, clinical processes, equipment, communication systems, and organizational resources. Collectively, these theoretical perspectives provide a suitable framework for explaining how nurse-patient ratios may influence patient safety in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected Nigerian public hospitals using structured nurse questionnaires, staffing records, duty rosters, patient census records, nursing workload assessment tools, incident-reporting records, and patient-safety assessment checklists. A multistage sampling technique will be employed to select states, public hospitals, clinical departments, nurses, and relevant patient-care units. Nurse-patient ratio will be assessed using indicators such as average number of patients assigned to each nurse per shift, day-to-night staffing ratios, nurse staffing adequacy, number of nurses available relative to patient census, frequency of understaffed shifts, overtime frequency, and workload variation across departments. Objective staffing records and duty rosters will be used where available to calculate actual nurse-patient ratios rather than relying solely on nurses' perceptions. Patient safety will be assessed using indicators such as medication errors, patient falls, healthcare-associated infections, pressure injuries, delayed nursing interventions, missed nursing care, patient identification errors, documentation errors, adverse events, near misses, and compliance with established patient-safety procedures. Hospital incident reports, quality-improvement records, nursing records, and patient-safety registers will be reviewed where available to complement questionnaire data. Descriptive statistics will be used to summarize nurse-patient ratios, staffing patterns, patient characteristics, and patient-safety indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of nurse-patient ratio on patient safety. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that high nurse-patient ratios have a significant negative effect on patient safety in Nigerian public hospitals. Units where individual nurses are responsible for a large number of patients are expected to experience greater patient-safety risks than units with more adequate nurse staffing. High nurse-patient ratios may reduce the time available for patient assessment and monitoring, delay medication administration and other nursing interventions, increase missed nursing care, and contribute to documentation and communication errors. Excessive patient assignments may also make it more difficult for nurses to identify early signs of clinical deterioration and respond promptly to emergencies. Conversely, adequate nurse-patient ratios are expected to support closer patient monitoring, timely intervention, improved communication, better documentation, and greater adherence to patient-safety procedures. However, nurse-patient ratio alone may not fully determine patient safety because patient acuity, nurse competence, availability of equipment and medicines, hospital workload, organizational culture, supervision, and clinical protocols may also influence safety outcomes. Therefore, appropriate nurse staffing should form part of a broader patient-safety and healthcare-quality strategy. The study is expected to contribute to the literature on nurse-patient ratios, nursing workforce adequacy, patient safety, nursing care quality, healthcare quality, hospital administration, nursing management, and public health in Nigeria by providing empirical evidence on the relationship between nurse staffing levels and patient-safety outcomes. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, nursing management departments, chief nursing officers, human-resource departments, Nursing and Midwifery Council of Nigeria, healthcare quality-improvement teams, professional organizations, development partners, and policymakers regarding strategies for improving nurse staffing. The study will also provide evidence-based recommendations for establishing appropriate nurse-patient staffing levels, improving workforce planning, reducing understaffed shifts, strengthening nurse recruitment and deployment, monitoring nurse-patient ratios across hospital departments, reducing excessive overtime, and integrating nurse staffing indicators into hospital patient-safety monitoring systems.
