Effect of Oral Health Education on Dental Service Utilization among University Students in Nigeria

 

Abstract

Oral health is an important component of general health and wellbeing, yet dental problems such as dental caries, periodontal disease, oral infections, and other oral conditions remain common among young adults. University students may experience oral health challenges because of dietary habits, inadequate oral hygiene practices, academic demands, financial constraints, limited awareness, fear of dental procedures, and infrequent use of professional dental services. Oral health education provides students with knowledge and practical information about oral hygiene, prevention of dental diseases, appropriate tooth-brushing techniques, use of fluoride-containing toothpaste, dietary practices, recognition of oral health problems, importance of routine dental examinations, and appropriate utilization of professional dental services. Dental service utilization refers to the use of professional oral healthcare services, including routine dental examinations, preventive dental care, treatment of oral conditions, professional cleaning, dental consultation, and follow-up care. In Nigeria, low utilization of dental services among university students may be associated with limited awareness, misconceptions about dental care, perceived absence of symptoms, fear of dental procedures, cost of services, limited availability of dental facilities, and preference for seeking care only when oral problems become severe. Effective oral health education may improve students' knowledge and encourage timely and preventive use of dental services. Against this background, this study investigates the effect of oral health education on dental service utilization among university students in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding students' decisions to utilize dental services through perceived susceptibility to oral diseases, perceived severity of oral health problems, perceived benefits of preventive dental care, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, environmental conditions, and health-related behaviour in shaping dental service utilization. The Knowledge-Attitude-Practice framework provides a basis for examining how oral health education may improve students' knowledge and attitudes and influence their utilization of dental services. Collectively, these theoretical perspectives provide a suitable framework for explaining how oral health education may influence dental service utilization among university students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from undergraduate students in selected Nigerian universities using a structured self-administered questionnaire, oral health knowledge assessment, and dental service utilization assessment tool. A multistage sampling technique will be employed to select universities, faculties or departments, academic levels, and eligible students. Where feasible, selected student groups or university settings will be assigned to intervention and comparison groups. Students in the intervention group will receive structured oral health education covering the importance of oral health, common dental diseases, oral hygiene practices, effective tooth-brushing techniques, use of fluoride toothpaste, dietary practices, signs and symptoms requiring professional attention, benefits of preventive dental visits, available dental services, and appropriate healthcare-seeking pathways. Dental service utilization will be assessed using indicators such as attendance for routine dental examinations, professional dental cleaning, dental consultation, treatment of oral conditions, preventive dental services, and follow-up appointments. Where feasible, self-reported utilization will be complemented by available dental clinic attendance records. Descriptive statistics will be used to summarize students' sociodemographic characteristics, previous exposure to oral health education, oral health knowledge, dental attendance history, and utilization patterns. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the effect of oral health education on dental service utilization. Where appropriate, dental service utilization before and after the intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that oral health education has a significant positive effect on dental service utilization among university students in Nigeria. Students exposed to structured oral health education are expected to demonstrate improved understanding of oral diseases, preventive dental care, appropriate oral hygiene, and the importance of routine dental visits. Education may encourage students to seek professional dental care before oral problems become severe and increase utilization of preventive services such as routine examinations and professional cleaning. It may also reduce misconceptions about dental care and improve awareness of available dental services within university and community healthcare facilities. However, education alone may not guarantee increased utilization because students may continue to experience barriers such as cost of dental services, fear of dental procedures, limited availability of dental facilities, long waiting times, academic schedules, transportation difficulties, and reliance on self-medication or informal treatment. Consequently, oral health education may be most effective when supported by accessible and affordable university dental services, preventive dental programmes, student-friendly appointment systems, and continuous oral health promotion. The study is expected to contribute to the literature on oral health education, dental service utilization, university student health, preventive dentistry, health-seeking behaviour, health promotion, and public health in Nigeria by providing empirical evidence on the effect of oral health education on dental service utilization among university students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, universities, university health centres, dental clinics, dental professionals, public health practitioners, health educators, development partners, and policymakers regarding strategies for improving oral healthcare utilization among young adults. The study will also provide evidence-based recommendations for integrating oral health education into university health programmes, strengthening preventive dental services, improving access to affordable dental care, promoting routine dental examinations, reducing fear and misconceptions about dental treatment, and encouraging timely utilization of professional dental services among university students in Nigeria.
