Effect of Oral Health Screening on Dental Problems Detection among Pregnant Women in Nigeria
Abstract
Oral health is an important component of maternal health, and pregnancy-related physiological and hormonal changes may increase women's susceptibility to certain oral health problems. Pregnant women may experience conditions such as gingivitis, dental caries, periodontal problems, tooth sensitivity, and other oral health concerns that can affect comfort, nutrition, quality of life, and general wellbeing. However, oral health problems may remain undetected during pregnancy because of inadequate awareness, limited access to dental services, financial constraints, fear of dental treatment, misconceptions about the safety of dental care during pregnancy, and inadequate integration of oral healthcare into antenatal services. Oral health screening during pregnancy provides an opportunity to identify dental problems early and facilitate appropriate counselling, referral, diagnosis, and treatment. Against this background, this study investigates the effect of oral health screening on dental problems detection among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Social Ecological Model. The Health Belief Model explains how pregnant women's perceptions of susceptibility to oral health problems, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in oral health screening. Andersen's Behavioral Model explains how women's predisposing characteristics, enabling resources, and perceived oral healthcare needs influence their utilization of screening and dental services. The Social Ecological Model emphasizes the interaction between individual, family, community, healthcare-system, and broader environmental factors in determining access to and utilization of oral health screening services. Collectively, these theoretical perspectives provide a suitable framework for explaining how oral health screening may influence dental problems detection among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women attending selected antenatal clinics, primary healthcare centres, hospitals, maternity clinics, and community maternal health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible pregnant women. Oral health screening will be assessed using indicators such as availability of screening services, screening coverage, frequency of screening, availability of trained oral health personnel, use of standardized oral health assessment procedures, dental examination, assessment of gingival and periodontal health, identification of dental caries, assessment of tooth pain or sensitivity, oral health counselling, referral arrangements, and follow-up services. Dental problems detection will be assessed using indicators such as newly identified dental caries, gingivitis, periodontal problems, tooth pain, oral infections, other clinically identifiable dental conditions, suspected oral abnormalities requiring further assessment, referral for professional dental evaluation, and confirmed dental diagnoses where appropriate. Data will be collected using structured questionnaires, standardized oral health screening checklists, clinical examination records, antenatal records, dental referral registers, and relevant healthcare records. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, oral health findings, and patterns of dental problems detection. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of oral health screening on dental problems detection. Where a quasi-experimental design is adopted, detection rates before and after implementation of oral health screening may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that oral health screening has a significant positive effect on dental problems detection among pregnant women in Nigeria. Pregnant women who participate in routine oral health screening are expected to have a higher likelihood of having previously undetected dental problems identified than women without access to screening. Screening may facilitate early identification of gingivitis, dental caries, periodontal problems, tooth pain, and other oral health conditions, thereby creating opportunities for timely counselling, professional dental assessment, appropriate treatment, and follow-up. Integrating oral health screening into antenatal services may reduce the need for women to make separate healthcare visits and may improve referral between maternal and dental healthcare providers. However, limited availability of dental professionals, inadequate screening equipment, financial barriers, fear of dental treatment, misconceptions about dental care during pregnancy, and weak referral systems may reduce screening effectiveness. The study therefore expects accessible, routine, safe, and well-coordinated oral health screening within maternal healthcare services to contribute significantly to improved detection of dental problems among pregnant women in Nigeria. The study is expected to contribute to the literature on oral health screening during pregnancy, dental problems detection, maternal oral health, antenatal care, preventive dentistry, pregnancy-related oral health, dental healthcare utilization, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, dental clinics, dentists, dental therapists, nurses, midwives, maternal health organizations, development partners, and policymakers regarding strategies for improving oral health during pregnancy. The study will also provide evidence-based recommendations for integrating oral health screening into antenatal care, strengthening referral pathways between maternal and dental services, improving access to trained oral health professionals, providing appropriate screening equipment, increasing pregnant women's awareness of oral health, and ensuring timely dental evaluation and treatment for pregnant women across Nigeria.
