Effect of Osteoporosis Education on Bone Health Knowledge among Postmenopausal Women in Nigeria

 

Abstract

Osteoporosis is an important public health concern among postmenopausal women because the decline in oestrogen levels after menopause can accelerate bone loss and increase the risk of osteoporosis, fractures, disability, and reduced quality of life. Adequate knowledge of bone health can help postmenopausal women understand risk factors for osteoporosis and adopt appropriate preventive practices, including adequate calcium and vitamin D intake, regular weight-bearing and muscle-strengthening physical activity, avoidance of tobacco use and excessive alcohol consumption, fall-prevention practices, and appropriate healthcare assessment. However, some postmenopausal women in Nigeria may have limited knowledge of osteoporosis and its prevention because of inadequate health education, misconceptions about ageing and bone health, limited access to healthcare information, and insufficient emphasis on osteoporosis prevention within routine women's health services. Osteoporosis education provides an opportunity to improve women's understanding of osteoporosis, its risk factors, prevention, early assessment, and appropriate bone-health practices. Against this background, this study investigates the effect of osteoporosis education on bone health knowledge among postmenopausal women in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Promotion Model. The Health Belief Model explains how women's perceptions of their susceptibility to osteoporosis, perceived severity of osteoporosis and fractures, perceived benefits of preventive practices, perceived barriers, and cues to action may influence their acquisition and application of bone health knowledge. Social Cognitive Theory emphasizes knowledge, self-efficacy, observational learning, social support, reinforcement, and environmental influences in developing appropriate bone-health behaviours. The Health Promotion Model emphasizes individual experiences, perceived benefits and barriers, interpersonal influences, and behavioural factors that influence participation in health-promoting activities. Collectively, these theoretical perspectives provide a suitable framework for explaining how osteoporosis education may influence bone health knowledge among postmenopausal women in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise postmenopausal women attending selected hospitals, primary healthcare centres, women's health clinics, community health programmes, and other relevant healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, clinics, and eligible postmenopausal women. Osteoporosis education will be assessed using indicators such as exposure to osteoporosis education, frequency and duration of education sessions, information on osteoporosis risk factors, effects of menopause on bone health, calcium and vitamin D requirements, physical activity, fall prevention, lifestyle risk factors, bone-health assessment, osteoporosis screening where clinically indicated, treatment options, use of educational materials, individualized counselling, and follow-up support. Bone health knowledge will be assessed using indicators such as understanding of osteoporosis, knowledge of osteoporosis risk factors, awareness of postmenopausal bone loss, knowledge of calcium and vitamin D sources, understanding of appropriate physical activity, awareness of fall-prevention measures, recognition of osteoporosis-related fracture risks, knowledge of lifestyle risk factors, and awareness of appropriate healthcare assessment. A structured bone health knowledge questionnaire or validated osteoporosis knowledge assessment instrument may be used where appropriate. Data will be collected using structured questionnaires, bone health knowledge assessment tools, clinic records, health education records, and relevant women's health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to osteoporosis education, and levels of bone health knowledge. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of osteoporosis education on bone health knowledge. Where a quasi-experimental design is adopted, bone health knowledge scores before and after structured osteoporosis education may be compared, while relevant demographic, socioeconomic, educational, health, and healthcare-access factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that osteoporosis education has a significant positive effect on bone health knowledge among postmenopausal women in Nigeria. Women who receive structured and comprehensive osteoporosis education are expected to demonstrate higher bone health knowledge scores than women with limited exposure to osteoporosis education. Education may improve women's understanding of osteoporosis risk factors, the relationship between menopause and bone loss, appropriate calcium and vitamin D intake, physical activity, fall prevention, and the importance of seeking appropriate healthcare assessment. Interactive education and repeated exposure to bone-health information may also correct misconceptions and improve women's confidence in adopting preventive practices. However, limited access to healthcare information, low health literacy, cultural beliefs, financial constraints, inadequate availability of educational resources, and limited access to specialized bone-health services may reduce the effectiveness of education. The study therefore expects accessible, culturally appropriate, practical, and regularly reinforced osteoporosis education to contribute significantly to improved bone health knowledge among postmenopausal women in Nigeria. The study is expected to contribute to the literature on osteoporosis education, bone health knowledge, postmenopausal health, osteoporosis prevention, healthy ageing, women's health, fracture prevention, nutrition education, physical activity, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, women's health clinics, healthcare providers, physiotherapists, nutritionists, community health workers, healthy ageing programmes, development partners, and policymakers regarding strategies for improving bone health among postmenopausal women. The study will also provide evidence-based recommendations for integrating osteoporosis and bone-health education into routine women's health and primary healthcare services, training healthcare providers in osteoporosis prevention education, developing culturally appropriate educational materials, promoting calcium-rich nutrition and appropriate physical activity, strengthening fall-prevention education, and improving awareness of bone-health assessment and osteoporosis prevention among postmenopausal women across Nigeria.
