Effect of Postpartum Depression Education on Postpartum Depression Recognition among Mothers in Nigeria

 

Abstract

Postpartum depression is an important maternal mental health concern that may occur during the period following childbirth and can negatively affect mothers' emotional wellbeing, daily functioning, relationships, and ability to care for their infants. When postpartum depression is not recognized and addressed early, it may persist and contribute to impaired maternal-infant bonding, difficulties with breastfeeding and infant care, social withdrawal, and other adverse family outcomes. In Nigeria, postpartum depression may remain underrecognized because of limited awareness, stigma surrounding mental health conditions, cultural beliefs, inadequate screening and counselling services, limited access to mental healthcare professionals, and reluctance to discuss emotional difficulties after childbirth. Postpartum depression education provides an opportunity to improve mothers' knowledge of common symptoms, risk factors, warning signs, available support services, and the importance of seeking professional help. Against this background, this study investigates the effect of postpartum depression education on postpartum depression recognition among mothers in Nigeria. The study will be anchored on the Health Belief Model, Theory of Planned Behavior, and Social Ecological Model. The Health Belief Model explains how mothers' perceptions of susceptibility to postpartum depression, perceived severity, perceived benefits of recognizing symptoms, perceived barriers, and cues to action may influence their ability to recognize and respond to symptoms. The Theory of Planned Behavior explains how attitudes toward mental health, perceived social expectations, perceived behavioural control, and intentions may influence mothers' willingness to identify symptoms and seek appropriate assistance. The Social Ecological Model emphasizes the influence of individual, family, community, healthcare, cultural, and socioeconomic factors on mental health awareness and recognition. Collectively, these theoretical perspectives provide a suitable framework for explaining how postpartum depression education may influence recognition of postpartum depression among mothers in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise mothers within the postpartum period attending selected healthcare facilities, postnatal clinics, or community-based maternal health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, postnatal clinics, communities, and eligible mothers. Postpartum depression education will be assessed using indicators such as exposure to educational sessions, frequency and timing of education, content of education, information on symptoms and risk factors, discussion of available treatment and support services, mental health counselling, healthcare-worker involvement, and educational materials provided. Postpartum depression recognition will be assessed using mothers' ability to identify common symptoms and warning signs, distinguish postpartum depression from normal temporary emotional changes, recognize when symptoms require professional attention, identify available sources of help, and appropriately indicate the need for referral or professional assessment. Where validated screening instruments are used, they will be employed to support standardized assessment of depressive symptoms while distinguishing recognition knowledge from clinical diagnosis. Data will be collected using structured questionnaires, postpartum education records, postnatal care records, maternal health records, and relevant mental health programme documents. Descriptive statistics will be used to summarize mothers' characteristics, exposure to education, mental health knowledge, and recognition patterns. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of postpartum depression education on recognition of postpartum depression. Where a quasi-experimental design is adopted, recognition scores before and after exposure to structured education may be compared, while relevant maternal, socioeconomic, and healthcare-related factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that postpartum depression education has a significant positive effect on postpartum depression recognition among mothers in Nigeria. Mothers who receive structured and comprehensive education are expected to demonstrate better knowledge of postpartum depression symptoms and warning signs and a greater ability to distinguish the condition from normal emotional changes after childbirth. Education may also improve awareness of available mental health services, reduce misconceptions, encourage communication about emotional difficulties, and promote timely help-seeking. However, stigma, cultural beliefs, limited access to mental healthcare services, inadequate healthcare-worker training, low literacy, family attitudes, and fear of social judgment may continue to limit recognition and disclosure of symptoms. The study therefore expects accessible, culturally appropriate, and comprehensive postpartum depression education, integrated into routine maternal and postnatal services, to contribute significantly to improved recognition and early response to postpartum depression in Nigeria. The study is expected to contribute to the literature on postpartum depression education, postpartum depression recognition, maternal mental health, postnatal care, maternal wellbeing, mental health literacy, healthcare utilization, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, maternal and child health programmes, psychiatrists, psychologists, psychiatric nurses, obstetricians, midwives, community health workers, development partners, and policymakers regarding strategies for strengthening maternal mental health services. The study will also provide evidence-based recommendations for integrating postpartum depression education into antenatal and postnatal care, strengthening healthcare-worker capacity for maternal mental health education, improving mental health literacy, reducing stigma, increasing awareness of available support services, strengthening referral pathways, and promoting early identification and professional assessment of postpartum depression across Nigeria.
