Effect of School-Based Hearing Screening on Hearing Impairment Detection among Primary School Pupils in Nigeria

Abstract

Hearing impairment remains an important public health and educational concern among primary school pupils in Nigeria and may affect children's communication, language development, classroom participation, academic performance, social interaction, and overall development. Hearing difficulties may result from congenital conditions, ear infections, excessive noise exposure, genetic factors, birth-related complications, and other health conditions. Some forms of hearing impairment may remain undetected during early childhood because pupils may not recognize or report hearing difficulties and parents or teachers may not immediately identify the signs. Delayed detection may result in difficulties with speech and language development, learning, social participation, and educational achievement. School-based hearing screening provides an opportunity to identify hearing impairment among pupils through systematic hearing assessments conducted within the school environment. Early detection may facilitate further audiological assessment, medical evaluation, treatment, hearing rehabilitation, assistive devices, and appropriate educational support. However, inadequate screening programmes, limited availability of audiometric equipment, shortage of trained audiologists and healthcare personnel, low awareness, financial barriers, and weak referral and follow-up systems may limit the effectiveness of school-based hearing screening in Nigeria. Against this background, this study investigates the effect of school-based hearing screening on hearing impairment detection among primary school pupils in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and the Health Systems Framework. The Health Belief Model explains how parents', teachers', and pupils' perceptions of susceptibility to hearing problems, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in school-based hearing screening. The Social Ecological Model emphasizes the interaction between individual, family, school, community, and broader societal factors in shaping children's health and access to preventive services. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective hearing screening and child healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how school-based hearing screening may influence hearing impairment detection among primary school pupils in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise pupils enrolled in selected public and private primary schools across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, schools, classes, and eligible pupils. School-based hearing screening will be measured using indicators such as availability of hearing screening programmes, screening coverage, frequency of screening activities, availability of trained healthcare or audiological personnel, availability of hearing screening equipment, hearing assessment procedures, parental consent, teacher participation, and referral arrangements. Hearing impairment detection will be assessed using indicators such as abnormal hearing screening results, hearing thresholds, suspected hearing impairment, newly identified cases, degree of hearing impairment, identification of conductive or sensorineural hearing abnormalities where appropriate, referral for comprehensive audiological assessment, confirmed cases, and recommendations for treatment or hearing support. Data will be collected using structured questionnaires, school hearing screening registers, audiometric screening records, school health records, referral registers, medical records, and relevant programme documents. Descriptive statistics will be used to summarize pupils' characteristics, screening coverage, hearing assessment findings, and patterns of hearing impairment detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of school-based hearing screening on hearing impairment detection. Where appropriate, detection rates before and after implementation of school-based hearing screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school-based hearing screening has a significant positive effect on hearing impairment detection among primary school pupils in Nigeria. Pupils who participate in school-based hearing screening are expected to have a higher likelihood of having previously undetected hearing difficulties identified than pupils without access to routine screening. Systematic hearing assessment within schools may facilitate early identification of children with hearing impairment and provide opportunities for timely audiological evaluation, medical treatment, hearing rehabilitation, assistive hearing devices, and educational support. Conducting screening in schools may reduce geographical and financial barriers while allowing teachers and parents to become more aware of children's hearing needs. Early detection may also support improved communication, classroom participation, learning, and social development. Conversely, inadequate screening coverage, shortage of trained audiologists, limited screening equipment, low parental awareness, concerns about screening, and weak referral and follow-up systems may reduce the effectiveness of school-based hearing screening. The study therefore expects accessible and well-organized school-based hearing screening to contribute significantly to improved detection and early management of hearing impairment among primary school pupils in Nigeria. The study is expected to contribute to the literature on school-based hearing screening, hearing impairment detection, child hearing health, school health services, audiological screening, educational development, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Primary Health Care Development Agency, state ministries of health and education, primary schools, audiologists, otolaryngologists, healthcare providers, school health personnel, community health workers, development partners, and policymakers regarding strategies for improving hearing health among school children. The study will also provide evidence-based recommendations for expanding school-based hearing screening programmes, improving access to audiometric equipment and trained hearing-care personnel, integrating hearing screening into school health services, increasing parental and teacher awareness of childhood hearing problems, strengthening referral and follow-up systems, improving access to appropriate treatment and hearing rehabilitation, and providing educational support for pupils identified with hearing impairment across Nigeria.
