Effect of School-Based Vision Screening on Detection of Visual Impairment among Secondary School Students in Nigeria
Abstract
Visual impairment is an important public health and educational concern among secondary school students because undetected vision problems may affect reading, classroom participation, academic performance, concentration, social interaction, and overall wellbeing. Refractive errors and other visual conditions may remain undetected when students do not recognize their symptoms, do not report difficulties with vision, or have limited access to routine eye examinations. School-based vision screening provides an opportunity to identify students with suspected visual impairment within the school environment and facilitate timely referral for comprehensive eye examination, corrective treatment, and appropriate follow-up. However, inadequate screening programmes, limited availability of trained eye-care personnel, insufficient screening equipment, low awareness, financial constraints, and weak referral systems may limit access to school eye-health services in Nigeria. Against this background, this study investigates the effect of school-based vision screening on the detection of visual impairment among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Andersen's Behavioral Model of Health Services Use. The Health Belief Model explains how students' and parents' perceptions of susceptibility to visual impairment, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in vision screening. The Social Ecological Model emphasizes the interaction between individual, family, school, community, healthcare-system, and broader societal factors that influence access to preventive eye-care services. Andersen's Behavioral Model explains how predisposing characteristics, enabling resources, and perceived healthcare needs influence utilization of vision screening and eye-care services. Collectively, these theoretical perspectives provide a suitable framework for explaining how school-based vision screening may influence the detection of visual impairment among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise students enrolled in selected public and private secondary schools across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, schools, classes, and eligible students. School-based vision screening will be assessed using indicators such as availability of screening programmes, screening coverage, frequency of screening, availability of trained eye-care personnel, availability and functionality of visual acuity charts and other screening equipment, standardized screening procedures, parental consent, documentation of screening results, referral arrangements, and follow-up systems. Detection of visual impairment will be assessed using indicators such as abnormal visual acuity results, suspected refractive errors, reduced distance or near vision, newly identified visual impairment, referral for comprehensive eye examination, confirmed diagnosis, prescription of corrective lenses where required, and follow-up attendance. Data will be collected using structured questionnaires, school vision screening records, visual acuity assessment results, referral registers, school health records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, visual assessment findings, and patterns of visual impairment detection. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of school-based vision screening on the detection of visual impairment. Where a quasi-experimental design is adopted, detection rates before and after implementation of school-based vision screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school-based vision screening has a significant positive effect on the detection of visual impairment among secondary school students in Nigeria. Students who participate in school-based vision screening are expected to have a higher likelihood of previously undetected visual difficulties being identified than students without access to routine screening. Conducting screening within schools may reduce geographical and financial barriers while allowing students with abnormal findings to be referred for comprehensive eye assessment and appropriate management. Early detection may facilitate timely provision of spectacles or other corrective interventions, improve classroom participation, and reduce the educational consequences associated with untreated visual impairment. School-based programmes may also increase awareness among students, parents, and teachers regarding the importance of eye health and regular vision assessment. However, shortages of trained eye-care professionals, inadequate screening equipment, limited access to affordable spectacles, poor parental follow-up, financial barriers, and weak referral systems may reduce the effectiveness of school-based vision screening. The study therefore expects accessible, regular, standardized, and well-coordinated school-based vision screening to contribute significantly to improved detection and early management of visual impairment among secondary school students in Nigeria. The study is expected to contribute to the literature on school-based vision screening, visual impairment detection, adolescent eye health, refractive errors, school health services, preventive eye care, educational performance, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Primary Health Care Development Agency, state ministries of health and education, secondary schools, ophthalmologists, optometrists, ophthalmic nurses, school health personnel, parents, development partners, and policymakers regarding strategies for improving eye health among students. The study will also provide evidence-based recommendations for expanding school-based vision screening programmes, improving access to trained eye-care personnel and screening equipment, strengthening referral and follow-up systems, increasing parental and teacher awareness, improving access to affordable corrective lenses, and integrating vision screening into routine school health programmes across Nigeria.
