Impact of Blood Donor Screening Procedures on Blood Safety in Nigerian Blood Banks
Abstract
Blood transfusion is an essential component of healthcare delivery and is critical for managing severe anaemia, obstetric complications, trauma, surgical conditions, and other life-threatening medical conditions. Ensuring the safety of donated blood is therefore essential for protecting recipients from transfusion-transmissible infections and other preventable transfusion-related risks. Blood donor screening procedures provide important safeguards through donor selection, medical history assessment, physical assessment, infectious disease testing, blood grouping, compatibility testing, and appropriate documentation. However, challenges such as inadequate screening equipment, shortages of trained laboratory personnel, inconsistent adherence to screening protocols, limited quality-control systems, insufficient reagents, and weak documentation may affect blood safety in Nigerian blood banks. Effective and consistently implemented donor screening procedures may reduce the risk of unsafe blood entering the transfusion system and strengthen the quality of blood services. Against this background, this study investigates the impact of blood donor screening procedures on blood safety in Nigerian blood banks. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and the Health Belief Model. The Donabedian Model explains healthcare quality through structure, process, and outcome, with blood donor screening procedures representing important healthcare processes and blood safety serving as a measurable outcome. Systems Theory views blood safety as the result of interactions among blood donors, blood banks, laboratory personnel, screening technologies, quality-control systems, blood-storage facilities, regulatory bodies, and transfusion services. The Health Belief Model provides a framework for understanding how donor perceptions of infectious disease risks, benefits of screening, perceived barriers, and cues to action may influence compliance with donor-screening requirements and accurate disclosure of relevant health information. Collectively, these theoretical perspectives provide a suitable framework for explaining how blood donor screening procedures may influence blood safety in Nigerian blood banks. The study will adopt a quantitative analytical cross-sectional or retrospective research design. The study population will comprise selected public and private blood banks and blood donation centres in Nigeria, blood donors, laboratory scientists, blood-bank personnel, and available blood screening records. A multistage sampling technique will be used to select states, local government areas, blood banks, blood donation centres, screening units, personnel, and eligible donor records. Blood donor screening procedures will be assessed using indicators such as donor registration, donor eligibility assessment, medical history screening, risk-factor assessment, physical examination, haemoglobin assessment where applicable, blood grouping, testing for transfusion-transmissible infections, screening for HIV, hepatitis B, hepatitis C, syphilis and other infections according to applicable national requirements, quality of screening reagents, equipment availability, adherence to standard operating procedures, internal quality control, external quality assessment, repeat or confirmatory testing where required, documentation, confidentiality, staff training, and referral of deferred donors. Blood safety will be assessed using indicators such as proportion of donated blood units that meet established safety requirements, prevalence of reactive screening results, detection of transfusion-transmissible infections, accuracy and completeness of screening records, proportion of unsafe units appropriately discarded, compliance with quality-control requirements, and occurrence of screening-related errors. Data will be collected using structured questionnaires, donor-screening records, laboratory registers, blood-bank records, quality-control reports, screening test results, standard operating procedures, and relevant regulatory or institutional documents. Descriptive statistics will be used to summarize donor characteristics, screening procedures, laboratory findings, and blood-safety indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of blood donor screening procedures on blood safety. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective and consistently implemented blood donor screening procedures have a significant positive impact on blood safety in Nigerian blood banks. Blood banks with comprehensive donor-selection and screening procedures are expected to demonstrate improved detection of transfusion-transmissible infections, better identification of unsuitable donors, fewer screening errors, stronger documentation, and improved compliance with blood-safety standards. Adequate laboratory equipment, reliable reagents, trained personnel, standardized procedures, and effective quality-control mechanisms are expected to strengthen the accuracy and reliability of screening results. Appropriate donor education and confidential risk assessment may also encourage accurate disclosure of relevant health information and support safer donor selection. However, inadequate equipment, reagent shortages, poor infrastructure, insufficient staff training, weak quality-control systems, inconsistent protocol compliance, and documentation gaps may compromise blood safety. The study therefore expects comprehensive, standardized, adequately resourced, and quality-controlled blood donor screening procedures to contribute significantly to improved blood safety in Nigerian blood banks. The study is expected to contribute to the literature on blood donor screening, blood safety, transfusion-transmissible infections, blood-bank quality management, donor selection, laboratory screening, transfusion medicine, patient safety, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Agency, state ministries of health, blood banks, blood donation centres, medical laboratory scientists, transfusion specialists, healthcare administrators, regulatory bodies, professional organizations, development partners, and policymakers regarding strategies for strengthening blood safety. The study will also provide evidence-based recommendations for standardizing donor-screening procedures, improving laboratory infrastructure and equipment, ensuring consistent availability of quality reagents, strengthening staff training and competency assessment, improving internal and external quality-control systems, strengthening documentation and traceability, monitoring screening performance, and ensuring compliance with national blood-safety standards across Nigerian blood banks.
