Impact of Community Disability Screening on Early Identification of Functional Disabilities among Adults in Nigeria

 

Abstract

Functional disability is an important public health concern that may affect an individual's mobility, self-care, communication, cognition, hearing, vision, and ability to participate fully in family, social, educational, and economic activities. Functional difficulties may remain unidentified when individuals have limited access to routine health assessments, disability screening, rehabilitation services, or appropriate assistive technologies. In Nigeria, geographical barriers, financial constraints, limited disability awareness, stigma, inadequate rehabilitation services, and shortages of trained healthcare and rehabilitation professionals may contribute to delayed identification of functional disabilities among adults. Community disability screening provides an opportunity to bring preliminary functional assessments closer to residents and identify individuals who may require further clinical, rehabilitation, or social-support evaluation. Against this background, this study investigates the impact of community disability screening on early identification of functional disabilities among adults in Nigeria. The study will be anchored on the Social Model of Disability, Health Belief Model, and Social Ecological Model. The Social Model of Disability emphasizes the role of environmental, social, and institutional barriers in limiting participation among people with functional difficulties, rather than viewing disability solely as an individual medical condition. The Health Belief Model explains how adults' perceptions of susceptibility to functional difficulties, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in disability screening. The Social Ecological Model emphasizes the interaction between individual, family, community, healthcare, environmental, and societal factors in determining recognition of functional disabilities and access to appropriate services. Collectively, these theoretical perspectives provide a suitable framework for explaining how community disability screening may influence early identification of functional disabilities among adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above residing in selected urban, semi-urban, and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible adults. Community disability screening will be assessed using indicators such as availability and frequency of screening activities, screening coverage, use of standardized functional-assessment tools, availability of trained healthcare or rehabilitation personnel, screening location accessibility, duration of assessment, disability-health education, referral arrangements, and follow-up procedures. Early identification of functional disabilities will be assessed using indicators such as newly identified difficulties with mobility, vision, hearing, communication, cognition, self-care, or other relevant functional domains; abnormal screening results; suspected functional disability; referrals for comprehensive clinical or rehabilitation assessment; confirmation of functional limitations where appropriate; and recommendations for assistive devices, rehabilitation, social support, or other appropriate services. Data will be collected using structured questionnaires, standardized functional-disability screening instruments, screening registers, community outreach records, referral registers, healthcare and rehabilitation records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, functional assessment findings, and referral patterns. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of community disability screening on early identification of functional disabilities. Where a quasi-experimental design is adopted, identification rates before and after implementation of community disability screening may be compared with those of a comparison community or group to determine changes associated with the intervention. Diagnostic performance measures such as sensitivity, specificity, positive predictive value, and negative predictive value may also be assessed where an appropriate clinical reference standard is available. Diagnostic tests will further be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community disability screening has a significant positive impact on early identification of functional disabilities among adults in Nigeria. Adults who participate in community disability screening are expected to have a greater likelihood of previously unrecognized functional difficulties being identified than adults without comparable access to screening. Community-based assessment may reduce geographical, financial, and transportation barriers and provide opportunities for individuals who do not routinely seek specialized rehabilitation or disability services to receive preliminary assessment. Early identification may facilitate timely referral for comprehensive clinical assessment, rehabilitation, assistive devices, occupational or physical therapy, communication support, visual or hearing services, and appropriate social or community-based support. Community screening may also improve awareness of functional disability and reduce misconceptions and stigma surrounding disability. However, inadequate screening equipment, shortages of trained rehabilitation personnel, stigma, low awareness, inaccessible screening environments, limited referral facilities, financial barriers, and weak follow-up systems may reduce effectiveness. The study therefore expects accessible, standardized, culturally appropriate, and professionally supervised community disability screening to contribute significantly to improved early identification of functional disabilities among adults in Nigeria. The study is expected to contribute to the literature on disability screening, functional disability, rehabilitation, community health, disability prevention, assistive technology, health equity, inclusive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, National Commission for Persons with Disabilities, state ministries of health, primary healthcare centres, hospitals, rehabilitation centres, physiotherapists, occupational therapists, audiologists, optometrists, community health workers, disability organizations, development partners, and policymakers regarding strategies for improving early identification and referral of functional disabilities. The study will also provide evidence-based recommendations for integrating functional-disability screening into community and primary healthcare programmes, strengthening referral and rehabilitation services, training healthcare workers in standardized functional assessments, improving access to assistive devices, reducing disability-related stigma, and expanding inclusive health services for adults with functional difficulties across Nigeria.
