Impact of Community Nutrition Programmes on Dietary Diversity among Children Under Five in Nigeria

 

Abstract

Adequate nutrition during early childhood is essential for healthy growth, cognitive development, immune function, and prevention of childhood illness. Children under five years are particularly vulnerable to malnutrition because their nutritional requirements are high while their diets may be limited by household food insecurity, poverty, inadequate nutrition knowledge, cultural practices, and restricted access to diverse foods. Dietary diversity refers to the variety of food groups consumed by a child over a specified period and is an important indicator of the adequacy and quality of complementary feeding. Community nutrition programmes provide an opportunity to improve child nutrition through nutrition education, growth monitoring, counselling, community-based supplementation, cooking demonstrations, breastfeeding and complementary-feeding promotion, home visits, and other interventions aimed at improving household and child feeding practices. In Nigeria, inadequate dietary diversity among young children remains a public health concern, particularly in communities affected by poverty, food insecurity, limited access to health services, and inadequate nutrition information. Effective community nutrition programmes may improve caregivers' knowledge and practices and increase children's access to a wider range of nutritious foods. Against this background, this study investigates the impact of community nutrition programmes on dietary diversity among children under five in Nigeria. The study will be anchored on the Social Ecological Model, Social Cognitive Theory, and Health Belief Model. The Social Ecological Model recognizes that child feeding and nutritional outcomes are influenced by individual, household, community, organizational, and broader environmental factors. Social Cognitive Theory emphasizes the interaction between knowledge, behavioural practices, social influences, and environmental conditions in shaping health-related behaviours. The Health Belief Model explains preventive and health-promoting behaviours through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action. Collectively, these theoretical perspectives provide a suitable framework for explaining how community nutrition programmes may influence dietary diversity among children under five in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from caregivers of children aged 6–59 months participating in selected community nutrition programmes across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible caregiver-child pairs. Community nutrition programmes will be assessed using indicators such as participation in nutrition education sessions, exposure to complementary-feeding counselling, attendance at growth-monitoring activities, participation in cooking demonstrations, home visits by community health or nutrition workers, access to nutrition information, breastfeeding education, micronutrient education, and participation in community-based nutrition activities. Dietary diversity will be assessed using appropriate child dietary diversity indicators, including the number and variety of food groups consumed by the child during the reference period. Information on consumption of grains, roots and tubers, legumes and nuts, dairy products, flesh foods, eggs, vitamin-A-rich fruits and vegetables, and other relevant food groups will be collected using a structured dietary-recall questionnaire. Descriptive statistics will be used to summarize participants' characteristics, programme participation, and dietary diversity patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of community nutrition programmes on dietary diversity. Diagnostic tests will also be conducted to assess the reliability of the research instrument and the robustness of the findings. The study is expected to find that participation in community nutrition programmes has a significant positive impact on dietary diversity among children under five in Nigeria. Children whose caregivers participate regularly in community nutrition programmes are expected to have more diverse diets than children whose caregivers have limited or no exposure to such programmes. Nutrition education and complementary-feeding counselling may improve caregivers' knowledge of the importance of providing children with foods from different food groups. Cooking demonstrations and practical nutrition education may also increase caregivers' confidence in preparing affordable and nutritious meals using locally available foods. Growth-monitoring activities and home visits may provide additional opportunities for identifying poor feeding practices and providing targeted nutrition advice. Conversely, limited access to community nutrition programmes may contribute to inadequate nutrition knowledge and continued reliance on less diverse feeding practices. However, community nutrition programmes alone may not fully determine dietary diversity because household income, food availability, food prices, household food security, cultural practices, maternal education, access to markets, and seasonal variations may also influence children's diets. The study is expected to contribute to the literature on community nutrition programmes, dietary diversity, child nutrition, complementary feeding, maternal and child health, malnutrition prevention, community health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, nutrition agencies, primary healthcare authorities, community health workers, nutritionists, development partners, and policymakers regarding strategies for improving child nutrition. The study will also provide evidence-based recommendations for strengthening community nutrition education, expanding caregiver counselling, improving cooking demonstrations, increasing community health worker involvement, promoting locally available nutritious foods, integrating dietary-diversity assessment into child health services, and strengthening community-based nutrition programmes to improve the quality of diets among children under five in Nigeria.
