Impact of Community Polio Immunization Campaigns on Polio Vaccination Uptake among Children in Rural Nigeria

 

Abstract

Poliomyelitis remains an important public health concern in Nigeria because poliovirus transmission can result in paralysis, permanent disability, and, in severe cases, death. Although vaccination is highly effective in preventing poliomyelitis, children in rural communities may remain vulnerable because of missed vaccination opportunities, limited access to healthcare facilities, inadequate awareness, vaccine misinformation, geographical barriers, population mobility, and difficulties reaching underserved households. Community polio immunization campaigns provide an opportunity to bring vaccination services closer to households and increase awareness and acceptance of polio vaccination. Effective community-based campaigns may improve access to vaccination, identify children who have missed previous doses, and strengthen community participation in polio eradication activities. Against this background, this study investigates the impact of community polio immunization campaigns on polio vaccination uptake among children in rural Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Theory of Planned Behavior. The Health Belief Model explains how parents' and caregivers' perceptions of children's susceptibility to poliomyelitis, perceived severity of the disease, perceived benefits of vaccination, perceived barriers, and cues to action may influence acceptance of polio vaccination. The Social Ecological Model emphasizes the interaction of individual, family, community, healthcare, and environmental factors in shaping childhood vaccination behaviour. The Theory of Planned Behavior explains how caregivers' attitudes, subjective norms, perceived behavioural control, and vaccination intentions may influence their decision to allow children to receive polio vaccines. Collectively, these theoretical perspectives provide a suitable framework for explaining how community polio immunization campaigns may influence vaccination uptake among children in rural Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise children within the applicable polio immunization age range residing in selected rural communities in Nigeria and their parents or primary caregivers. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, rural communities, households, and eligible child-caregiver pairs. Community polio immunization campaigns will be assessed using indicators such as campaign frequency, duration, geographical coverage, household visits, vaccination-team availability, community mobilization, caregiver sensitization, involvement of community leaders, availability of vaccines, vaccination-point accessibility, identification of missed children, reminder activities, and follow-up mechanisms. Polio vaccination uptake will be assessed using indicators such as receipt of polio vaccine doses, vaccination status verified through vaccination cards or programme records where available, participation during campaign activities, completion of recommended doses according to the applicable programme schedule, number of previously missed children vaccinated, and timely receipt of scheduled doses. Data will be collected using structured questionnaires, child vaccination cards, immunization registers, household campaign records, vaccination-team records, community mobilization records, and relevant immunization programme documents. Descriptive statistics will be used to summarize caregivers' characteristics, campaign exposure, vaccination knowledge, and children's polio vaccination status. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of community polio immunization campaigns on polio vaccination uptake. Where a quasi-experimental design is adopted, vaccination uptake before and after implementation of community campaigns may be compared with that of a comparison community or group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community polio immunization campaigns have a significant positive impact on polio vaccination uptake among children in rural Nigeria. Children living in communities exposed to well-organized polio immunization campaigns are expected to have higher vaccination uptake than children in communities with limited campaign exposure. Household vaccination activities may reduce geographical and financial barriers by bringing vaccination services directly to families, while community sensitization may improve caregivers' understanding of poliomyelitis and the importance of vaccination. Campaigns may also identify children who have previously missed vaccination opportunities and provide additional opportunities for immunization. Engagement of traditional leaders, religious leaders, community health workers, and other trusted community members may increase acceptance and reduce vaccine-related misconceptions. However, vaccine misinformation, caregiver refusal, population mobility, insecurity in some areas, inadequate campaign coverage, poor access to remote households, insufficient vaccination personnel, and weak follow-up systems may reduce campaign effectiveness. The study therefore expects accessible, community-oriented, well-coordinated, and adequately supported polio immunization campaigns to contribute significantly to improved polio vaccination uptake among children in rural Nigeria. The study is expected to contribute to the literature on poliomyelitis prevention, polio immunization campaigns, childhood vaccination, vaccine uptake, community mobilization, immunization coverage, infectious disease prevention, rural healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, Nigeria Centre for Disease Control and Prevention, state primary healthcare boards, local government health authorities, primary healthcare centres, community health workers, traditional and religious leaders, development partners, and policymakers regarding strategies for strengthening polio eradication activities. The study will also provide evidence-based recommendations for improving community-based vaccination campaigns, strengthening household vaccination activities, enhancing caregiver education, addressing vaccine misinformation, improving identification and follow-up of missed children, strengthening community mobilization, and improving access to routine and campaign-based polio vaccination services across rural Nigeria.
