Impact of Digital Appointment Booking Systems on Missed Hospital Appointments in Nigeria

 

Abstract

Missed hospital appointments are an important healthcare management challenge because they may disrupt continuity of care, reduce efficient utilization of healthcare resources, increase waiting lists, and contribute to delays in diagnosis, treatment, and disease monitoring. In Nigeria, missed appointments may be associated with forgetfulness, poor communication between healthcare facilities and patients, transportation difficulties, long distances, financial constraints, inconvenient appointment schedules, limited awareness of appointment dates, and inadequate reminder systems. Digital appointment booking systems provide an opportunity to improve appointment management through electronic scheduling, automated reminders, appointment confirmation, rescheduling options, and improved communication between healthcare facilities and patients. However, limited digital literacy, unreliable internet connectivity, high data costs, inadequate digital infrastructure, technological challenges, and limited patient access to smartphones may affect the effectiveness of digital appointment systems. Against this background, this study investigates the impact of digital appointment booking systems on missed hospital appointments in Nigeria. The study will be anchored on the Technology Acceptance Model, Queuing Theory, and Systems Theory. The Technology Acceptance Model explains how perceived usefulness and perceived ease of use of digital appointment booking systems may influence patients' acceptance and utilization of the technology. Queuing Theory explains how appointment scheduling and organized patient arrivals can improve the management of healthcare demand and reduce inefficiencies associated with unplanned patient attendance. Systems Theory emphasizes the interdependence between patients, healthcare workers, hospital information systems, appointment scheduling, communication systems, and administrative processes in determining appointment outcomes. Collectively, these theoretical perspectives provide a suitable framework for explaining how digital appointment booking systems may influence missed hospital appointments in Nigeria. The study will adopt a quantitative analytical retrospective, cross-sectional, or quasi-experimental research design. The study population will comprise patients attending selected public and private hospitals in Nigeria. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, clinics, and eligible patients or appointment records. Digital appointment booking systems will be assessed using indicators such as availability of online booking platforms, electronic scheduling, appointment confirmation, automated reminders, SMS or email notifications, mobile application functionality, appointment rescheduling options, cancellation facilities, patient communication, and integration with hospital information systems. Missed hospital appointments will be assessed using indicators such as the proportion of scheduled appointments not attended, frequency of missed appointments, repeated missed appointments, late cancellations, failure to reschedule, and appointment attendance rates. Data will be collected using structured questionnaires, electronic appointment records, hospital registers, appointment schedules, reminder logs, patient medical records, and relevant hospital information-system reports. Descriptive statistics will be used to summarize patients' characteristics, digital appointment system utilization, appointment attendance, and patterns of missed appointments. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of digital appointment booking systems on missed hospital appointments. Where a quasi-experimental design is adopted, missed appointment rates before and after implementation of a digital appointment booking system may be compared, while relevant demographic, socioeconomic, clinical, technological, and hospital-level factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective digital appointment booking systems have a significant negative impact on missed hospital appointments, indicating that improved digital appointment management is associated with fewer missed appointments. Patients using digital booking systems are expected to demonstrate higher appointment attendance and lower rates of missed appointments because electronic scheduling, automated reminders, appointment confirmations, and convenient rescheduling options may reduce forgetfulness and communication-related barriers. Digital systems may also improve coordination between patients and healthcare facilities and allow hospitals to monitor appointment attendance more effectively. However, the effectiveness of digital appointment systems may be limited by poor internet connectivity, limited digital literacy, lack of access to smartphones, high data costs, inaccurate patient contact information, technical failures, inadequate staff training, and patients' inability to afford or access transportation and healthcare services. The study therefore expects accessible, reliable, user-friendly, and well-integrated digital appointment booking systems to contribute significantly to reducing missed hospital appointments in Nigeria. The study is expected to contribute to the literature on digital appointment booking, missed hospital appointments, healthcare information systems, patient attendance, hospital management, digital health, healthcare efficiency, and patient-centred healthcare in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, public and private hospitals, hospital administrators, health information managers, healthcare providers, digital health organizations, development partners, and policymakers regarding strategies for improving appointment management. The study will also provide evidence-based recommendations for expanding digital appointment booking systems, strengthening automated appointment reminders, improving patient communication, providing convenient rescheduling options, enhancing digital literacy, maintaining accurate patient contact information, training hospital staff, improving system reliability, and integrating digital appointment management into broader hospital quality-improvement and digital health programmes across Nigeria.
