Impact of Emergency Medical Transport Services on Emergency Care Access among Road Traffic Accident Victims in Nigeria

Abstract

Road traffic accidents remain a major public health problem in Nigeria and contribute substantially to preventable deaths, injuries, disabilities, and loss of productivity. Victims of road traffic accidents often require immediate medical assessment and emergency treatment, particularly those with severe bleeding, fractures, head injuries, spinal injuries, and other life-threatening conditions. However, timely access to emergency care may be hindered by inadequate emergency medical transport systems, limited ambulance availability, poor road infrastructure, traffic congestion, long distances to healthcare facilities, inadequate communication systems, and delays in dispatching and transferring injured persons. Emergency medical transport services provide an important link between accident scenes and healthcare facilities by facilitating rapid transportation, basic prehospital care, stabilization, and referral of injured persons to appropriate emergency departments. Effective emergency medical transport may therefore improve timely access to emergency care and increase the likelihood that road traffic accident victims receive appropriate treatment. Against this background, this study investigates the impact of emergency medical transport services on emergency care access among road traffic accident victims in Nigeria. The study will be anchored on the Andersen Behavioral Model of Health Services Use, the Donabedian Model of Healthcare Quality, and the Health Systems Framework. The Andersen Behavioral Model explains access to emergency healthcare through predisposing, enabling, and need factors, with emergency medical transport representing an important enabling factor. The Donabedian Model emphasizes healthcare quality through structure, process, and outcome, with emergency transport infrastructure and service delivery representing key structural and process components and timely emergency care access serving as an important outcome. The Health Systems Framework emphasizes service delivery, health workforce, medical products and technologies, health information, financing, governance, and accessibility as essential components of an effective emergency healthcare system. Collectively, these theoretical perspectives provide a suitable framework for explaining how emergency medical transport services may influence emergency care access among road traffic accident victims in Nigeria. The study will adopt a quantitative analytical cross-sectional, retrospective cohort, or quasi-experimental research design, depending on the availability of emergency transport records and study sites. The study population will comprise road traffic accident victims receiving emergency care at selected hospitals and emergency medical facilities in Nigeria. A multistage sampling technique will be used to select states, local government areas, emergency medical transport providers, accident locations, healthcare facilities, and eligible road traffic accident victims or their caregivers. Emergency medical transport services will be assessed using indicators such as ambulance availability, accessibility of emergency transport, dispatch time, response time, travel time, availability of trained emergency medical personnel, prehospital first aid, patient stabilization, communication between ambulance personnel and receiving facilities, ambulance equipment, referral arrangements, and continuity of transport services. Emergency care access will be assessed using indicators such as time from accident occurrence to arrival at a healthcare facility, time from arrival to initial medical assessment, ability to reach an appropriate emergency facility, receipt of emergency treatment, availability of appropriate emergency services, referral completion, emergency department admission, and delays in receiving definitive care. Data will be collected using structured questionnaires, ambulance dispatch records, emergency medical service records, hospital emergency registers, patient medical records, accident reports, referral records, and interviews with emergency medical personnel and caregivers where appropriate. Descriptive statistics will be used to summarize victims' demographic and injury characteristics, emergency transport utilization, response patterns, and access to emergency care. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of emergency medical transport services on emergency care access. Where a quasi-experimental or comparative design is adopted, emergency care access among victims transported through organized emergency medical transport services may be compared with victims transported through informal or conventional means. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective emergency medical transport services have a significant positive impact on emergency care access among road traffic accident victims in Nigeria. Victims transported through organized emergency medical transport systems are expected to reach appropriate healthcare facilities more rapidly and have shorter delays before receiving emergency assessment and treatment than victims transported through informal or non-specialized means. Timely ambulance dispatch, appropriate prehospital care, patient stabilization, effective communication with receiving hospitals, and coordinated referral may improve continuity between the accident scene and hospital emergency services. However, inadequate ambulance coverage, poor road conditions, traffic congestion, limited emergency personnel, inadequate ambulance equipment, communication failures, insufficient funding, and long distances between accident locations and appropriate emergency facilities may reduce the effectiveness of emergency medical transport services. The study therefore expects accessible, adequately equipped, well-coordinated, and responsive emergency medical transport systems to contribute significantly to improved emergency care access among road traffic accident victims in Nigeria. The study is expected to contribute to the literature on emergency medical transport, emergency care access, road traffic injuries, prehospital care, ambulance services, trauma care, emergency healthcare systems, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Emergency Medical Treatment Committee, state ministries of health, emergency medical service agencies, hospitals, road safety authorities, local government authorities, emergency care providers, development partners, and policymakers regarding strategies for strengthening emergency response and trauma care. The study will also provide evidence-based recommendations for expanding ambulance coverage, improving emergency dispatch systems, strengthening prehospital care capacity, equipping ambulances appropriately, training emergency medical personnel, improving communication between ambulance services and hospitals, establishing coordinated referral networks, addressing transportation barriers, and integrating emergency medical transport into a comprehensive national emergency care system across Nigeria.
