Impact of Essential Medicine Stock Availability on Treatment Continuity in Nigerian Primary Healthcare Centres

 

Abstract

Availability of essential medicines is a critical component of effective primary healthcare delivery and is particularly important for ensuring that patients receive uninterrupted treatment for acute and chronic health conditions. In Nigeria, primary healthcare centres may experience medicine stock-outs resulting from inadequate procurement, funding constraints, supply-chain challenges, inaccurate forecasting, transportation difficulties, and weak inventory management systems. Frequent or prolonged stock-outs may force patients to interrupt prescribed treatment, purchase medicines from alternative sources, change prescribed medications, or delay further care. These interruptions may negatively affect treatment outcomes, patient satisfaction, healthcare utilization, and the overall effectiveness of primary healthcare services. Ensuring adequate and continuous availability of essential medicines may therefore contribute to improved treatment continuity in primary healthcare centres. Against this background, this study investigates the impact of essential medicine stock availability on treatment continuity in Nigerian primary healthcare centres. The study will be anchored on the Health Systems Framework, Andersen's Behavioral Model of Health Services Use, and the Donabedian Model of Healthcare Quality. The Health Systems Framework emphasizes the importance of medical products, health financing, supply systems, service delivery, and health information in ensuring effective healthcare services. Andersen's Behavioral Model explains how enabling resources, including the availability and accessibility of medicines, influence healthcare utilization and continued use of health services. The Donabedian Model provides a framework for understanding how healthcare structures and resources influence service processes and patient outcomes, with medicine availability representing an important structural component of healthcare quality. Collectively, these theoretical perspectives provide a suitable framework for explaining how essential medicine stock availability may influence treatment continuity in Nigerian primary healthcare centres. The study will adopt a quantitative analytical cross-sectional or retrospective facility-based research design. The study population will comprise selected primary healthcare centres and patients receiving treatment for selected health conditions in these facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, primary healthcare centres, healthcare workers, and eligible patients. Essential medicine stock availability will be measured using indicators such as availability of tracer essential medicines, frequency of stock-outs, duration of stock-outs, percentage of days medicines are available, adequacy of medicine quantities, availability of medicines at the point of dispensing, stock-record accuracy, procurement regularity, replenishment time, and availability of appropriate storage facilities. Treatment continuity will be assessed using indicators such as continuation of prescribed treatment, completion of treatment regimens, availability of prescribed medicines during follow-up visits, medication refill continuity, treatment interruption, treatment switching due to medicine unavailability, missed treatment periods, referrals resulting from medicine stock-outs, and patients' ability to obtain prescribed medicines from the same healthcare facility. Data will be collected using structured questionnaires, medicine stock cards, inventory records, facility registers, patient treatment records, pharmacy records, and relevant procurement and supply-chain documents. Descriptive statistics will be used to summarize facility characteristics, medicine availability, stock-out patterns, and treatment continuity. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of essential medicine stock availability on treatment continuity. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that essential medicine stock availability has a significant positive impact on treatment continuity in Nigerian primary healthcare centres. Primary healthcare centres with consistent availability of essential medicines are expected to demonstrate higher levels of treatment continuity and fewer treatment interruptions among patients. Reliable medicine availability may enable patients to obtain prescribed medicines during initial treatment and subsequent visits, reduce the need to purchase medicines from alternative sources, minimize missed doses, and support completion of recommended treatment regimens. Conversely, frequent and prolonged medicine stock-outs may increase treatment interruptions, medication switching, delayed treatment, referrals, additional financial costs, and reduced confidence in primary healthcare services. The study therefore expects improved procurement, inventory management, forecasting, distribution, stock monitoring, and timely replenishment of essential medicines to contribute significantly to improved treatment continuity in Nigerian primary healthcare centres. The study is expected to contribute to the literature on essential medicine availability, medicine stock-outs, treatment continuity, primary healthcare, pharmaceutical supply chains, healthcare access, medication management, health-system performance, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare centre managers, pharmaceutical supply agencies, healthcare workers, development partners, and policymakers regarding strategies for strengthening medicine availability at the primary healthcare level. The study will also provide evidence-based recommendations for improving medicine forecasting and procurement, strengthening inventory and stock-monitoring systems, reducing medicine stock-outs, improving distribution and replenishment mechanisms, strengthening facility-level pharmaceutical management, and developing sustainable essential medicine supply systems to support uninterrupted treatment in primary healthcare centres across Nigeria.
