Impact of Health Communication Campaigns on Disease Prevention Practices among Urban Residents in Nigeria
Abstract
Health communication is an important component of public health because effective communication can provide individuals and communities with timely information that supports disease prevention, health promotion, and appropriate health-seeking behaviour. Health communication campaigns use coordinated communication strategies, including mass media, social media, community outreach, interpersonal communication, posters, radio, television, and other information channels, to provide health information and encourage the adoption of appropriate health behaviours. Disease prevention practices refer to the actions individuals take to reduce their risk of acquiring or transmitting diseases, including hand hygiene, vaccination, use of preventive medications where appropriate, safe food and water practices, environmental sanitation, use of protective measures, early healthcare-seeking, and adherence to recommended public health guidance. In Nigeria, inadequate adoption of disease prevention practices among urban residents may be associated with misinformation, limited access to reliable health information, misconceptions about diseases, low risk perception, communication gaps, and inconsistent exposure to credible health messages. Effective health communication campaigns may improve residents' knowledge and understanding of disease risks and encourage the adoption of appropriate preventive practices. Against this background, this study investigates the impact of health communication campaigns on disease prevention practices among urban residents in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding residents' adoption of disease prevention practices through perceived susceptibility to disease, perceived severity of health consequences, perceived benefits of preventive actions, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between health information, self-efficacy, observational learning, social influences, communication environments, and preventive health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how health communication campaigns may improve knowledge and attitudes and translate these improvements into appropriate disease prevention practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how health communication campaigns may influence disease prevention practices among urban residents in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from residents of selected urban communities in Nigeria using a structured interviewer-administered questionnaire, health-information exposure assessment, and standardized disease prevention practice checklist. A multistage sampling technique will be employed to select states, local government areas, urban communities, households, and eligible adult residents. Where feasible, selected communities will be assigned to intervention and comparison groups. Residents in the intervention communities will be exposed to a structured health communication campaign delivered through multiple communication channels, including community meetings, radio messages, social media content, posters, health talks, interpersonal communication, and other appropriate channels. The campaign will provide information on common disease risks, modes of transmission, preventive measures, vaccination, hygiene, environmental sanitation, safe food and water practices, appropriate healthcare-seeking, and reliable sources of health information. Disease prevention practices will be assessed using indicators such as hand hygiene, vaccination uptake, environmental sanitation, safe food and water handling, appropriate use of protective measures, adherence to disease-prevention recommendations, and timely healthcare-seeking when symptoms occur. Descriptive statistics will be used to summarize residents' sociodemographic characteristics, sources of health information, exposure to communication campaigns, health knowledge, and disease prevention practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the impact of health communication campaigns on disease prevention practices. Where appropriate, disease prevention practices before and after the campaign will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that health communication campaigns have a significant positive impact on disease prevention practices among urban residents in Nigeria. Residents exposed to well-designed and sustained health communication campaigns are expected to demonstrate improved knowledge of disease risks, increased awareness of preventive measures, and greater adoption of recommended disease prevention practices. Multichannel communication campaigns are expected to be particularly effective because they can reach diverse urban populations through different information sources and reinforce health messages across multiple settings. Campaigns using clear, culturally appropriate, credible, and actionable messages are expected to improve residents' understanding of preventive behaviours and encourage their adoption. However, communication campaigns alone may not guarantee sustained behaviour change because residents may continue to face barriers such as misinformation, financial constraints, limited access to preventive services, inadequate sanitation infrastructure, distrust of health institutions, and information overload. Consequently, health communication campaigns may be most effective when supported by accessible preventive healthcare services, reliable public health information, supportive community environments, and sustained engagement. The study is expected to contribute to the literature on health communication campaigns, disease prevention practices, health promotion, health education, health communication, urban health, behaviour change, and public health in Nigeria by providing empirical evidence on the impact of health communication interventions on disease prevention behaviour among urban residents. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, state ministries of health, local government health authorities, public health agencies, media organizations, community-based organizations, healthcare providers, health educators, communication specialists, development partners, and policymakers regarding strategies for improving disease prevention. The study will also provide evidence-based recommendations for strengthening health communication campaigns, expanding multichannel public health messaging, improving the credibility and accessibility of health information, engaging communities in health communication planning, addressing misinformation, and promoting sustained disease prevention practices among urban residents in Nigeria.
