Impact of Health Education on Early Healthcare-Seeking Practices among Adults with Symptoms of Infectious Diseases in Nigeria

Abstract

Infectious diseases remain an important public health concern in Nigeria and can result in substantial morbidity, mortality, and economic burden when diagnosis and treatment are delayed. Early healthcare seeking is important because prompt evaluation of symptoms can facilitate timely diagnosis, appropriate treatment, infection prevention, and referral where necessary. Early healthcare-seeking practices involve seeking appropriate medical attention promptly after recognizing symptoms of illness rather than delaying care, self-medicating extensively, relying solely on traditional remedies, or waiting until symptoms become severe. Health education is an important public health intervention that can improve individuals' knowledge of infectious diseases, recognition of warning signs, understanding of disease transmission, appropriate prevention measures, and awareness of when and where to seek professional healthcare. In Nigeria, delayed healthcare seeking may be associated with inadequate health knowledge, financial constraints, transportation difficulties, misconceptions about illness, reliance on self-medication or traditional remedies, fear of healthcare costs, perceived severity of symptoms, and limited access to healthcare facilities. Effective health education may improve symptom recognition, perceived risk, knowledge of appropriate healthcare services, and willingness to seek timely professional care. Against this background, this study investigates the impact of health education on early healthcare-seeking practices among adults with symptoms of infectious diseases in Nigeria. The study is anchored on the Health Belief Model (HBM), Andersen's Behavioral Model of Health Services Use, and the Knowledge-Attitude-Practice (KAP) framework. The Health Belief Model explains health-seeking behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding individuals' decisions to seek healthcare promptly. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors, providing a framework for examining how individual characteristics, healthcare accessibility, and perceived need influence healthcare-seeking behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved health knowledge and attitudes resulting from health education may influence healthcare-seeking practices. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how health education may influence early healthcare seeking among adults experiencing symptoms of infectious diseases in Nigeria. The study adopts a quantitative quasi-experimental research design. Primary data will be collected from adults presenting with symptoms suggestive of selected infectious diseases at participating primary healthcare centres, outpatient clinics, and community health settings in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, healthcare facilities, communities, and eligible participants. Where feasible, selected communities or healthcare facilities will be assigned to intervention and comparison groups. Participants in the intervention group will receive structured health education covering recognition of common infectious disease symptoms, danger signs requiring urgent medical attention, modes of transmission, appropriate prevention measures, risks associated with delayed treatment, dangers of inappropriate self-medication, available healthcare services, and appropriate referral pathways. Early healthcare-seeking practices will be assessed using indicators such as time between symptom recognition and first contact with an appropriate healthcare provider, use of formal healthcare services, prompt response to danger signs, and avoidance of prolonged delays in seeking professional care. Descriptive statistics will be used to summarize participants' sociodemographic characteristics, health education exposure, symptoms, and healthcare-seeking practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple logistic regression analysis, will be employed to determine the impact of health education on early healthcare-seeking practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study anticipates that health education will have a significant positive impact on early healthcare-seeking practices among adults with symptoms of infectious diseases in Nigeria. Adults exposed to structured health education are expected to demonstrate improved recognition of infectious disease symptoms and warning signs and a greater likelihood of seeking appropriate professional healthcare promptly. Education may also reduce misconceptions, discourage prolonged self-medication, improve knowledge of available healthcare services, and encourage individuals to seek care before symptoms become severe. However, health education alone may not eliminate delays because adults may continue to face barriers such as healthcare costs, transportation difficulties, long distances to healthcare facilities, limited availability of services, fear of diagnosis, cultural beliefs, reliance on traditional remedies, and previous negative experiences with healthcare providers. Consequently, health education is expected to be most effective when combined with accessible, affordable, timely, and high-quality healthcare services. This study is expected to contribute to the literature on infectious disease prevention, health education, healthcare-seeking behaviour, community health, health promotion, and public health in Nigeria by providing empirical evidence on the impact of health education on early healthcare-seeking practices among adults experiencing symptoms of infectious diseases. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention (NCDC), National Primary Health Care Development Agency (NPHCDA), state ministries of health, primary healthcare facilities, hospitals, community health workers, public health practitioners, development partners, and policymakers regarding strategies for improving timely healthcare utilization. The study will also provide evidence-based recommendations for strengthening community-based health education, improving recognition of infectious disease symptoms and danger signs, promoting appropriate healthcare-seeking behaviour, addressing misconceptions and harmful self-treatment practices, strengthening referral systems, and improving access to primary healthcare services across Nigeria.
