Impact of HIV Prevention Education on HIV Testing Practices among Out-of-School Youth in Nigeria

Abstract

Human immunodeficiency virus (HIV) remains an important public health concern in Nigeria, particularly among populations that may experience limited access to health information and preventive services. Out-of-school youth may face challenges including limited access to formal health education, inadequate knowledge of HIV transmission and prevention, stigma, misconceptions, concerns about confidentiality, and barriers to accessing HIV testing services. These factors may reduce uptake of HIV testing and delay awareness of HIV status. HIV prevention education provides an opportunity to improve young people's understanding of HIV transmission, prevention methods, testing procedures, confidentiality, treatment, and the benefits of early diagnosis. Against this background, this study investigates the impact of HIV prevention education on HIV testing practices among out-of-school youth in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Belief and Behaviour Change principles. The Health Belief Model explains how young people's perceptions of their susceptibility to HIV, perceived severity of HIV infection, perceived benefits of HIV testing, perceived barriers, and cues to action may influence their testing practices. Social Cognitive Theory emphasizes observational learning, self-efficacy, social influence, and environmental factors in the adoption of HIV prevention and testing behaviours. Behaviour Change principles emphasize the role of accurate information, risk perception, motivation, supportive environments, and access to appropriate health services in encouraging preventive health behaviours. Collectively, these theoretical perspectives provide a suitable framework for explaining how HIV prevention education may influence HIV testing practices among out-of-school youth in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise out-of-school youth aged 15–24 years residing in selected urban, semi-urban, and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, youth groups, and eligible participants. HIV prevention education will be assessed using indicators such as exposure to structured HIV education, frequency and duration of educational sessions, information on HIV transmission, prevention methods, HIV testing procedures, benefits of knowing HIV status, confidentiality, treatment and prevention, stigma reduction, available testing services, and referral information. HIV testing practices will be assessed using indicators such as previous HIV testing, recent HIV testing, intention to test, repeated testing where appropriate, use of available testing services, knowledge of testing locations, willingness to seek testing after potential exposure, and linkage to appropriate healthcare services following testing. Data will be collected using structured questionnaires, validated HIV knowledge and testing-behaviour instruments, HIV education attendance records, youth programme records, and relevant health facility testing records where ethically and appropriately accessible. Descriptive statistics will be used to summarize participants' characteristics, exposure to HIV prevention education, and HIV testing practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of HIV prevention education on HIV testing practices. Where a quasi-experimental design is adopted, HIV testing practice indicators before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that HIV prevention education has a significant positive impact on HIV testing practices among out-of-school youth in Nigeria. Youth who receive structured and accurate HIV prevention education are expected to demonstrate improved knowledge of HIV transmission and prevention and greater engagement with HIV testing services. Education may improve awareness of where testing services are available, increase understanding of confidentiality and the benefits of knowing one's HIV status, reduce misconceptions, and encourage timely testing. Peer education, community outreach, youth-friendly communication, and interactive educational approaches may further improve acceptance of HIV testing. However, stigma, fear of a positive result, concerns about confidentiality, misinformation, limited availability of youth-friendly services, transportation costs, financial barriers, and fear of discrimination may reduce testing uptake. The study therefore expects accessible, confidential, youth-friendly, culturally appropriate, and regularly reinforced HIV prevention education to contribute significantly to improved HIV testing practices among out-of-school youth in Nigeria. The study is expected to contribute to the literature on HIV prevention education, HIV testing practices, out-of-school youth, HIV prevention, youth health, health education, stigma reduction, community-based health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, National Agency for the Control of AIDS, state ministries of health, primary healthcare facilities, community-based organizations, youth organizations, HIV prevention programmes, development partners, and policymakers regarding strategies for improving HIV testing among vulnerable young populations. The study will also provide evidence-based recommendations for expanding community-based HIV prevention education, strengthening youth-friendly HIV testing services, improving confidentiality, reducing HIV-related stigma, using peer educators and community outreach approaches, improving referral systems, and increasing access to HIV testing among out-of-school youth across Nigeria.
