Impact of Infertility Support Services on Infertility Healthcare Utilization among Women in Nigeria

Abstract

Infertility is an important reproductive health concern that can affect women's physical health, emotional wellbeing, social relationships, and quality of life. Women experiencing infertility may require timely access to appropriate reproductive health assessment, diagnosis, counselling, treatment, and follow-up services. However, infertility healthcare utilization among women in Nigeria may be affected by financial constraints, limited availability of specialized fertility services, geographical barriers, stigma, cultural expectations, misinformation, fear of treatment, long waiting periods, and inadequate referral systems. Infertility support services can provide women with reproductive health information, counselling, emotional support, referral assistance, treatment guidance, and follow-up care, potentially improving their ability and willingness to access appropriate infertility healthcare. Against this background, this study investigates the impact of infertility support services on infertility healthcare utilization among women in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Social Ecological Model. The Health Belief Model explains how women's perceptions of infertility-related health risks, perceived benefits of seeking professional care, perceived barriers, and cues to action may influence their utilization of infertility healthcare services. Andersen's Behavioral Model explains how predisposing characteristics, enabling resources, and healthcare needs influence the use of reproductive healthcare services. The Social Ecological Model emphasizes the influence of individual, interpersonal, organizational, community, and broader societal factors on women's access to and utilization of infertility support and healthcare services. Collectively, these theoretical perspectives provide a suitable framework for explaining how infertility support services may influence infertility healthcare utilization among women in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise women of reproductive age who have experienced infertility and are attending selected hospitals, fertility clinics, primary healthcare centres, reproductive health facilities, and community-based reproductive health programmes across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, fertility clinics, and eligible women. Infertility support services will be assessed using indicators such as availability of infertility counselling, reproductive health education, emotional support, peer-support opportunities, referral assistance, treatment information, fertility-care navigation, appointment support, follow-up services, availability of trained healthcare personnel, and accessibility of support programmes. Infertility healthcare utilization will be assessed using indicators such as attendance at infertility clinics, consultation with qualified healthcare providers, infertility investigations, diagnostic testing, treatment initiation, follow-up attendance, adherence to recommended referrals, use of appropriate reproductive health services, and continuity of infertility care. Data will be collected using structured questionnaires, infertility clinic attendance registers, medical records, referral registers, treatment records, and relevant reproductive health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to infertility support services, and patterns of infertility healthcare utilization. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of infertility support services on infertility healthcare utilization. Where a quasi-experimental design is adopted, healthcare utilization before and after participation in structured infertility support services may be compared, while relevant demographic, socioeconomic, duration of infertility, previous treatment history, healthcare-access, and other relevant factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that infertility support services have a significant positive impact on infertility healthcare utilization among women in Nigeria. Women who receive accessible and structured infertility support are expected to be more likely to seek professional infertility assessment, undergo recommended investigations, initiate appropriate treatment, attend follow-up appointments, and maintain continuity of care than women with limited access to support services. Reproductive health education and counselling may improve women's understanding of infertility causes, available treatment options, and the importance of seeking care from qualified healthcare professionals. Emotional and peer support may also reduce feelings of isolation, stigma, fear, and uncertainty that can discourage healthcare utilization. However, high treatment costs, limited availability of fertility specialists, long distances to specialized facilities, stigma, cultural expectations, misinformation, and inadequate referral systems may reduce the effectiveness of infertility support services. The study therefore expects accessible, confidential, culturally sensitive, and comprehensive infertility support services to contribute significantly to improved utilization of appropriate infertility healthcare among women in Nigeria. The study is expected to contribute to the literature on infertility support services, infertility healthcare utilization, reproductive health, fertility care, women's health, reproductive health counselling, healthcare access, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, fertility clinics, reproductive health centres, healthcare providers, counsellors, community health workers, development partners, and policymakers regarding strategies for improving infertility care. The study will also provide evidence-based recommendations for strengthening infertility counselling and support services, improving referral pathways, increasing access to qualified fertility healthcare providers, integrating infertility support into reproductive healthcare services, reducing barriers to infertility care, strengthening follow-up systems, and promoting confidential and patient-centred infertility healthcare services across Nigeria.
