Impact of Maternal Emergency Referral Training on Referral Decision-Making among Community Health Workers in Nigeria

 

Abstract

Timely recognition and referral of maternal emergencies are essential for reducing preventable maternal and newborn complications and improving outcomes during pregnancy, labour, delivery, and the postpartum period. Community health workers (CHWs) play an important role in linking women and communities with primary healthcare facilities and referral hospitals, particularly in rural and underserved areas of Nigeria. However, inadequate knowledge of maternal danger signs, uncertainty about referral indications, limited emergency preparedness, poor communication with receiving facilities, and unclear referral protocols may contribute to delays in referral decision-making. Maternal emergency referral training provides an opportunity to strengthen CHWs' knowledge, skills, confidence, and ability to make appropriate and timely referral decisions when maternal emergencies occur. Against this background, this study investigates the impact of maternal emergency referral training on referral decision-making among community health workers in Nigeria. The study will be anchored on the Three Delays Model, Social Cognitive Theory, and Self-Efficacy Theory. The Three Delays Model explains how delays in recognizing an emergency and deciding to seek care, reaching an appropriate healthcare facility, and receiving adequate care may contribute to poor maternal outcomes. Social Cognitive Theory explains how CHWs acquire and apply referral skills through training, observation, guided practice, feedback, reinforcement, and social support. Self-Efficacy Theory emphasizes the importance of CHWs' confidence in identifying maternal danger signs, interpreting referral protocols, communicating with healthcare facilities, and initiating appropriate referrals. Collectively, these theoretical perspectives provide a suitable framework for explaining how maternal emergency referral training may influence referral decision-making among CHWs in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise community health workers involved in maternal and reproductive healthcare services in selected communities, primary healthcare centres, community health programmes, and referral networks across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, primary healthcare facilities, community health programmes, and eligible CHWs. Maternal emergency referral training will be assessed using indicators such as exposure to structured training, training frequency and duration, content coverage, recognition of obstetric danger signs, use of referral protocols, emergency preparedness, communication procedures, referral documentation, transportation coordination, and practical or simulation-based training. Referral decision-making will be assessed using indicators such as ability to identify conditions requiring urgent referral, accuracy of referral decisions, timeliness of referral initiation, selection of appropriate referral facilities, communication with receiving facilities, completion of referral documentation, coordination of transportation, and appropriate follow-up after referral. Data will be collected using structured questionnaires, knowledge and skills assessment tools, clinical or referral case scenarios, observation checklists, referral registers, CHW records, and relevant programme documents. Descriptive statistics will be used to summarize CHWs' characteristics, training exposure, knowledge, and referral decision-making patterns. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of maternal emergency referral training on referral decision-making. Where a quasi-experimental design is adopted, referral decision-making scores before and after training will be compared to determine changes associated with the intervention. Scenario-based assessments may also be used to evaluate CHWs' ability to make appropriate decisions under different maternal emergency conditions. Diagnostic tests will be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that maternal emergency referral training has a significant positive impact on referral decision-making among community health workers in Nigeria. CHWs who receive structured, practical, and competency-based referral training are expected to demonstrate improved ability to recognize maternal danger signs, determine the urgency of referral, select appropriate referral facilities, communicate effectively with receiving healthcare providers, and initiate referrals promptly. Simulation exercises, case-based learning, and practical training may improve CHWs' confidence and ability to apply referral protocols in real-life situations. Effective training may also strengthen coordination among community health workers, primary healthcare centres, emergency transport providers, and referral hospitals. However, inadequate referral resources, transportation difficulties, poor communication networks, limited availability of receiving facilities, staff shortages, high workload, and weak referral feedback mechanisms may constrain the effectiveness of training. The study therefore expects regular, practical, competency-based, and contextually appropriate maternal emergency referral training to contribute significantly to improved referral decision-making among community health workers in Nigeria. The study is expected to contribute to the literature on maternal emergency referral training, community health workers, referral decision-making, maternal emergency preparedness, obstetric danger signs, the Three Delays Model, primary healthcare, maternal healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare centres, referral hospitals, community health worker programmes, maternal health organizations, development partners, and policymakers regarding strategies for strengthening community-level maternal emergency referral systems. The study will also provide evidence-based recommendations for strengthening CHW training programmes, incorporating practical simulations and case-based learning, standardizing maternal emergency referral protocols, improving communication between CHWs and healthcare facilities, strengthening transportation and referral networks, establishing regular refresher training, and improving referral feedback and supervision systems across Nigeria.
