Impact of Menopause Education on Healthcare-Seeking Practices among Middle-Aged Women in Nigeria

Abstract

Menopause is a natural stage of reproductive ageing that can be associated with physical, psychological, and social changes that may affect women's health and quality of life. Symptoms such as hot flashes, night sweats, sleep disturbances, mood changes, vaginal dryness, urinary symptoms, and changes in menstrual patterns may occur during the menopausal transition, although experiences vary among women. Menopause education provides women with accurate information about the menopausal transition, common symptoms, possible health implications, lifestyle measures, available treatment and management options, warning signs requiring medical attention, and appropriate healthcare services. Healthcare-seeking practices refer to the actions taken by women to obtain appropriate professional care when experiencing menopausal symptoms or health concerns, including timely consultation with qualified healthcare providers, utilization of appropriate health facilities, and adherence to professional recommendations. In Nigeria, limited knowledge, cultural beliefs, misconceptions about menopause, stigma, normalization of severe symptoms, financial constraints, and limited access to appropriate women's health services may discourage women from seeking professional care. Effective menopause education may improve knowledge, reduce misconceptions, increase recognition of symptoms requiring attention, and encourage appropriate healthcare-seeking practices. Against this background, this study investigates the impact of menopause education on healthcare-seeking practices among middle-aged women in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains health-seeking behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding women's decisions to seek healthcare during the menopausal transition. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from menopause education may influence healthcare-seeking practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how menopause education may influence healthcare-seeking practices among middle-aged women in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from middle-aged women, preferably those within the defined menopausal-transition age range, in selected communities and healthcare facilities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, communities, healthcare facilities, and eligible women. Where feasible, selected participant groups or communities will be assigned to intervention and comparison groups. Women in the intervention group will receive structured menopause education covering the menopausal transition, common symptoms, possible health effects, lifestyle and self-care measures, appropriate management options, misconceptions and cultural beliefs, warning signs requiring professional attention, and available women's health services. Healthcare-seeking practices will be assessed using indicators such as recognition of symptoms requiring professional attention, timely consultation with qualified healthcare providers, utilization of appropriate health facilities, disclosure of menopausal concerns to healthcare professionals, and adherence to recommended follow-up or management. Descriptive statistics will be used to summarize participants' sociodemographic and menopausal characteristics, menopause knowledge, exposure to education, and healthcare-seeking practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the impact of menopause education on healthcare-seeking practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that menopause education has a significant positive impact on healthcare-seeking practices among middle-aged women in Nigeria. Women exposed to structured menopause education are expected to demonstrate improved knowledge of the menopausal transition and greater awareness of symptoms and health concerns that may require professional attention. Education may reduce misconceptions, discourage inappropriate self-management of persistent or severe symptoms, encourage timely consultation with qualified healthcare providers, and improve utilization of appropriate women's health services. However, education alone may not guarantee appropriate healthcare seeking because women may continue to face barriers such as cultural beliefs, stigma, financial constraints, limited availability of specialized services, geographical barriers, and perceptions that menopausal symptoms are a normal condition that does not require medical attention. Consequently, menopause education is expected to be most effective when supported by accessible, affordable, respectful, and age-appropriate women's healthcare services. The study is expected to contribute to the literature on menopause, women's health, health education, healthcare-seeking behaviour, reproductive ageing, and public health in Nigeria by providing empirical evidence on the impact of menopause education on healthcare-seeking practices among middle-aged women. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, primary healthcare facilities, hospitals, women's health programmes, healthcare providers, community health workers, development partners, and policymakers regarding strategies for improving health outcomes during the menopausal transition. The study will also provide evidence-based recommendations for strengthening menopause education, integrating menopause counselling into primary healthcare and women's health services, improving healthcare-provider capacity, reducing misconceptions and stigma, strengthening community-based health education, and promoting timely healthcare seeking among middle-aged women across Nigeria.
