Impact of Mental Health First-Aid Education on Mental Health First-Aid Knowledge among Secondary School Teachers in Nigeria

Abstract

Mental health problems among adolescents and young people constitute an important public health concern, while secondary school teachers are often among the adults who interact with students regularly and may be in a position to recognize early signs of emotional distress, psychological difficulties, self-harm risk, substance-related problems, and other mental health concerns. However, many teachers may have limited knowledge and confidence regarding appropriate first-aid responses to students experiencing mental health difficulties. Mental health first-aid education provides teachers with knowledge and practical information on recognizing common mental health problems, identifying warning signs, responding appropriately to students in distress, providing immediate supportive assistance, maintaining appropriate boundaries, reducing stigma, promoting safety, and facilitating referral to qualified mental health professionals. Mental health first-aid knowledge refers to teachers' understanding of common mental health conditions, warning signs, appropriate immediate responses, crisis recognition, supportive communication, referral pathways, and appropriate sources of professional assistance. In Nigeria, inadequate mental health first-aid knowledge among secondary school teachers may limit early recognition and appropriate response to students experiencing mental health difficulties. Effective mental health first-aid education may improve teachers' ability to recognize mental health concerns and respond appropriately within the school environment. Against this background, this study investigates the impact of mental health first-aid education on mental health first-aid knowledge among secondary school teachers in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding teachers' recognition and response to mental health concerns through perceived susceptibility, perceived severity, perceived benefits of early intervention, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, and the school environment in shaping teachers' ability to respond to students experiencing mental health difficulties. The Knowledge-Attitude-Practice framework provides a basis for examining how mental health first-aid education may improve teachers' knowledge and attitudes and strengthen their capacity to provide appropriate initial assistance. Collectively, these theoretical perspectives provide a suitable framework for explaining how mental health first-aid education may influence mental health first-aid knowledge among secondary school teachers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from teachers in selected public and private secondary schools in Nigeria using a structured self-administered questionnaire and a standardized mental health first-aid knowledge assessment instrument. A multistage sampling technique will be employed to select states, local government areas, schools, and eligible secondary school teachers. Where feasible, selected schools or teacher groups will be assigned to intervention and comparison groups. Teachers in the intervention group will receive structured mental health first-aid education covering common mental health conditions among adolescents, signs and symptoms of mental health difficulties, warning signs of psychological crisis, appropriate communication with students experiencing distress, supportive first-aid responses, stigma reduction, confidentiality and professional boundaries, suicide and self-harm warning signs, referral procedures, and appropriate sources of professional mental health support. Mental health first-aid knowledge will be assessed using indicators covering recognition of common mental health problems, identification of warning signs, appropriate immediate responses, knowledge of referral pathways, crisis recognition, supportive communication, and misconceptions regarding mental health. Descriptive statistics will be used to summarize teachers' sociodemographic and professional characteristics, previous exposure to mental health education, and mental health first-aid knowledge scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the impact of mental health first-aid education on teachers' knowledge. Where appropriate, knowledge scores before and after the educational intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that mental health first-aid education has a significant positive impact on mental health first-aid knowledge among secondary school teachers in Nigeria. Teachers exposed to structured mental health first-aid education are expected to demonstrate improved ability to identify common mental health problems and recognize warning signs requiring immediate attention. Education is also expected to improve teachers' understanding of appropriate supportive responses, referral procedures, professional boundaries, crisis recognition, and the importance of seeking qualified mental health assistance. Practical case scenarios, role-play exercises, and school-based examples are expected to strengthen teachers' ability to apply mental health first-aid knowledge to situations involving students experiencing psychological distress. However, education alone may not ensure comprehensive management of students' mental health concerns because teachers may continue to face challenges such as limited access to school counsellors, shortages of mental health professionals, stigma, inadequate referral systems, confidentiality concerns, and insufficient institutional support. Consequently, mental health first-aid education may be most effective when supported by functional school counselling services, clear referral pathways, teacher support systems, mental health policies, and collaboration between schools and qualified mental health professionals. The study is expected to contribute to the literature on mental health first aid, mental health education, school health, teacher training, adolescent mental health, health promotion, and public health in Nigeria by providing empirical evidence on the impact of mental health first-aid education on teachers' knowledge. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, secondary schools, school counselling units, teacher-training institutions, public health practitioners, psychologists, counsellors, mental health professionals, development partners, and policymakers regarding strategies for strengthening mental health support within Nigerian schools. The study will also provide evidence-based recommendations for integrating mental health first-aid education into teacher training and professional development programmes, strengthening school counselling services, establishing effective referral systems, reducing mental health stigma, improving early recognition of students experiencing psychological difficulties, and promoting supportive school environments for adolescent mental health in Nigeria.
