Impact of Occupational Health Screening on Early Detection of Work-Related Health Problems among Healthcare Workers in Nigeria
Abstract
Healthcare workers are exposed to a wide range of occupational hazards that may increase their risk of work-related health problems, including infectious diseases, musculoskeletal disorders, respiratory conditions, skin disorders, hearing problems, psychological stress, and injuries. Despite their important role in healthcare delivery, occupational health screening may not be routinely accessible or consistently utilized among healthcare workers in Nigeria. Delayed identification of occupational health problems may result in prolonged illness, reduced work performance, absenteeism, increased healthcare costs, and potential risks to patients and other workers. Occupational health screening provides an opportunity to identify work-related health problems at an early stage through systematic assessment of workers' health status, occupational exposures, and relevant risk factors. Against this background, this study investigates the impact of occupational health screening on early detection of work-related health problems among healthcare workers in Nigeria. The study will be anchored on the Health Belief Model, Job Demands-Resources Model, and Occupational Health and Safety Framework. The Health Belief Model explains how healthcare workers' perceptions of susceptibility, severity, benefits of screening, barriers, and cues to action may influence participation in occupational health screening. The Job Demands-Resources Model emphasizes the relationship between workplace demands, available resources, employee wellbeing, stress, and work-related health outcomes. The Occupational Health and Safety Framework emphasizes hazard identification, risk assessment, health surveillance, prevention, early detection, and workplace interventions as essential components of worker protection. Collectively, these theoretical perspectives provide a suitable framework for explaining how occupational health screening may influence early detection of work-related health problems among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise healthcare workers aged 18 years and above employed in selected public and private healthcare facilities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, healthcare facilities, departments, professional categories, and eligible healthcare workers. Occupational health screening will be assessed using indicators such as frequency of screening, participation rates, pre-employment and periodic health assessments, occupational exposure assessment, blood pressure measurement, body mass index assessment, respiratory assessment, hearing assessment where appropriate, mental health assessment, immunization status review, infectious disease screening where indicated, ergonomic assessment, occupational injury assessment, and access to occupational health professionals. Early detection of work-related health problems will be assessed using indicators such as newly identified occupational health conditions, abnormal screening findings, early-stage work-related symptoms, occupational injuries, infectious disease exposures, musculoskeletal problems, respiratory abnormalities, hearing problems, psychological health concerns, and referrals for further medical evaluation. Data will be collected using structured questionnaires, occupational health screening records, medical examination reports, workplace health surveillance records, laboratory results where appropriate, occupational injury registers, and relevant health facility documents. Descriptive statistics will be used to summarize participants' characteristics, occupational exposures, screening participation, and detected health problems. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of occupational health screening on early detection of work-related health problems. Where a quasi-experimental design is adopted, detection rates before and after implementation of occupational health screening may be compared with those of a comparison group or facility to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that occupational health screening has a significant positive impact on early detection of work-related health problems among healthcare workers in Nigeria. Healthcare workers who participate in regular occupational health screening are expected to have a greater likelihood of previously undetected work-related health problems being identified than workers without comparable access to screening. Systematic screening may facilitate early recognition of occupational hazards, abnormal health findings, infectious disease exposures, musculoskeletal disorders, respiratory problems, hearing difficulties, psychological health concerns, and other work-related conditions. Early identification may allow timely medical evaluation, treatment, workplace modification, rehabilitation, vaccination, counselling, referral, and preventive interventions. Occupational health screening may also strengthen institutional awareness of workplace health risks and support the development of targeted occupational health and safety programmes. However, inadequate screening facilities, shortage of occupational health professionals, limited funding, poor record-keeping, low worker participation, concerns about confidentiality, inadequate follow-up, and weak occupational health policies may reduce effectiveness. The study therefore expects regular, confidential, accessible, and comprehensive occupational health screening to contribute significantly to improved early detection and management of work-related health problems among healthcare workers in Nigeria. The study is expected to contribute to the literature on occupational health screening, healthcare worker safety, occupational disease prevention, workplace health surveillance, health promotion, worker wellbeing, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, state ministries of health, healthcare institutions, occupational health professionals, hospital managements, professional healthcare associations, development partners, and policymakers regarding strategies for strengthening occupational health surveillance. The study will also provide evidence-based recommendations for establishing regular occupational health screening programmes, strengthening workplace health surveillance systems, improving access to occupational health professionals and screening equipment, protecting healthcare workers' confidentiality, strengthening referral and follow-up mechanisms, and integrating occupational health screening into routine healthcare facility management across Nigeria.
