Impact of Pregnancy Nutrition Counselling on Maternal Dietary Diversity among Pregnant Women in Nigeria

 

Abstract

Adequate maternal nutrition during pregnancy is essential for maintaining maternal health, supporting fetal growth and development, and reducing the risk of nutrition-related pregnancy complications. Maternal dietary diversity is particularly important because consuming a variety of foods from different food groups can improve the likelihood of obtaining essential nutrients required during pregnancy. However, pregnant women in Nigeria may experience poor dietary diversity because of limited nutrition knowledge, food insecurity, household economic constraints, cultural food restrictions, food preferences, seasonal food availability, and inadequate access to appropriate nutrition counselling. Pregnancy nutrition counselling provides an opportunity for healthcare providers to educate women about the importance of dietary diversity, appropriate food choices, meal planning, nutrient-rich foods, safe food preparation, and healthy eating practices during pregnancy. Against this background, this study investigates the impact of pregnancy nutrition counselling on maternal dietary diversity among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Promotion Model. The Health Belief Model explains how pregnant women's perceptions of the health consequences of poor nutrition, perceived benefits of dietary diversity, perceived barriers to healthy eating, and cues to action may influence their dietary practices. Social Cognitive Theory emphasizes knowledge, self-efficacy, behavioural capability, observational learning, social support, and environmental influences in the adoption of healthy dietary behaviours. The Health Promotion Model emphasizes individual experiences, perceived benefits and barriers, interpersonal influences, and behavioural factors that influence health-promoting nutritional practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how pregnancy nutrition counselling may influence maternal dietary diversity among pregnant women in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise pregnant women attending selected antenatal clinics in hospitals and primary healthcare centres across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, antenatal clinics, and eligible pregnant women. Pregnancy nutrition counselling will be assessed using indicators such as exposure to nutrition counselling, frequency and duration of counselling sessions, individualized dietary assessment, information on food groups, meal planning guidance, nutrient requirements during pregnancy, locally available nutritious foods, food safety education, dietary demonstrations, educational materials, healthcare-provider involvement, and follow-up support. Maternal dietary diversity will be assessed using indicators such as consumption of foods from major food groups, dietary diversity score, frequency of consumption of diverse foods, intake of fruits and vegetables, legumes and nuts, animal-source foods, whole grains or other staple foods, and appropriate consumption of nutrient-rich foods during pregnancy. Where appropriate, validated dietary diversity assessment tools will be used. Data will be collected using structured questionnaires, 24-hour dietary recall or other appropriate dietary assessment methods, antenatal nutrition records, and relevant maternal health programme documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to nutrition counselling, dietary patterns, and dietary diversity scores. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of pregnancy nutrition counselling on maternal dietary diversity. Where a quasi-experimental design is adopted, dietary diversity scores before and after structured nutrition counselling may be compared, while relevant socioeconomic, household, food-security, educational, and obstetric factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that pregnancy nutrition counselling has a significant positive impact on maternal dietary diversity among pregnant women in Nigeria. Women who receive structured and individualized nutrition counselling are expected to demonstrate higher dietary diversity scores and greater consumption of foods from multiple food groups than women with limited or no exposure to nutrition counselling. Counselling may improve women's understanding of nutrient requirements, encourage the inclusion of locally available nutrient-rich foods, and help women develop practical meal plans within their household resources. Repeated counselling during antenatal visits may reinforce appropriate dietary practices and provide opportunities to address misconceptions and cultural food restrictions. However, household food insecurity, poverty, high food prices, seasonal availability of foods, cultural practices, limited household decision-making power, and inadequate access to diverse foods may limit the effect of nutrition counselling alone. The study therefore expects accessible, practical, culturally appropriate, and context-specific pregnancy nutrition counselling to contribute significantly to improved maternal dietary diversity among pregnant women in Nigeria. The study is expected to contribute to the literature on pregnancy nutrition counselling, maternal dietary diversity, maternal nutrition, antenatal care, nutritional health, food security, maternal and child health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, nutritionists, dietitians, midwives, nurses, maternal health programmes, nutrition programmes, development partners, and policymakers regarding strategies for improving maternal nutrition during pregnancy. The study will also provide evidence-based recommendations for integrating comprehensive nutrition counselling into routine antenatal care, training healthcare providers in practical nutrition counselling, developing culturally appropriate dietary education materials, promoting locally available nutrient-rich foods, strengthening nutrition education and follow-up, and improving linkages between pregnant women and relevant food and nutrition support programmes across Nigeria.
