Impact of School-Based Mental Health Screening on Mental Health Problem Detection among Secondary School Students in Nigeria

Abstract

Mental health problems among secondary school students remain an important public health and educational concern in Nigeria, as adolescents may experience depression, anxiety, emotional distress, behavioural difficulties, suicidal thoughts, substance-related problems, and other mental health conditions during a critical stage of physical, emotional, and social development. Many mental health problems may remain undetected because students may be reluctant to disclose their experiences due to stigma, fear of discrimination, lack of awareness, limited access to mental health professionals, and inadequate mental health services within schools and communities. Undetected mental health problems may negatively affect academic performance, attendance, social relationships, behaviour, and overall well-being. School-based mental health screening provides an opportunity to identify students who may be experiencing mental health difficulties through systematic assessment within the school environment. Early detection may facilitate counselling, psychosocial support, referral to appropriate mental health professionals, and timely intervention. However, inadequate trained personnel, limited screening resources, concerns about confidentiality, parental consent challenges, stigma, and weak referral systems may limit the effectiveness of school-based mental health screening programmes in Nigeria. Against this background, this study investigates the impact of school-based mental health screening on mental health problem detection among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Andersen's Behavioral Model of Health Services Use. The Health Belief Model explains how students' perceptions of susceptibility to mental health problems, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in school-based mental health screening. The Social Ecological Model emphasizes the influence of individual, interpersonal, school, community, and broader societal factors on adolescent mental health and help-seeking behaviour. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding students' access to and utilization of mental health screening and support services. Collectively, these theoretical perspectives provide a suitable framework for explaining how school-based mental health screening may influence the detection of mental health problems among secondary school students in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise students enrolled in selected public and private secondary schools across Nigeria. A multistage sampling technique will be used to select states, local government areas, schools, classes, and eligible students. School-based mental health screening will be measured using indicators such as availability of screening programmes, screening coverage, frequency of screening, availability of trained school counsellors or mental health personnel, screening tools used, confidentiality procedures, student participation, parental consent processes, and referral arrangements. Mental health problem detection will be assessed using indicators such as identification of students with symptoms of depression, anxiety, emotional distress, behavioural problems, suicidal ideation, substance-related problems, and other relevant mental health concerns, as well as confirmed cases following professional assessment, referrals, and follow-up. Data will be collected using structured questionnaires, validated mental health screening instruments, school health records, counselling records, referral registers, and relevant programme documents. Descriptive statistics will be used to summarize students' characteristics, screening coverage, and patterns of mental health problem detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of school-based mental health screening on mental health problem detection. Where appropriate, detection rates before and after implementation of school-based screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school-based mental health screening has a significant positive impact on mental health problem detection among secondary school students in Nigeria. Students who participate in school-based mental health screening are expected to have a higher likelihood of having previously unrecognized mental health problems identified than students without access to routine screening. Regular screening may improve the identification of symptoms associated with depression, anxiety, emotional distress, behavioural difficulties, and other mental health concerns before they become more severe or significantly interfere with students' academic and social functioning. Early detection may also facilitate timely counselling, psychosocial support, professional assessment, referral, and follow-up care. Conducting screening within schools may reduce barriers related to transportation, cost, and limited awareness while providing opportunities for mental health concerns to be identified in a familiar environment. Conversely, stigma, inadequate confidentiality, shortage of trained personnel, limited screening resources, parental consent barriers, and weak referral systems may reduce participation and limit the effectiveness of school-based mental health screening. The study therefore expects accessible, confidential, and appropriately implemented school-based mental health screening to contribute significantly to improved detection of mental health problems among secondary school students in Nigeria. The study is expected to contribute to the literature on school-based mental health screening, adolescent mental health, mental health problem detection, student well-being, school health services, preventive mental healthcare, counselling, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, secondary schools, school counsellors, psychologists, healthcare providers, adolescent health programmes, development partners, parents, and policymakers regarding strategies for strengthening mental health services within schools. The study will also provide evidence-based recommendations for expanding school-based mental health screening programmes, training school counsellors and teachers in appropriate identification and referral procedures, improving confidentiality and safeguarding systems, increasing student and parental awareness of adolescent mental health, strengthening referral pathways to mental health professionals, integrating mental health screening into school health programmes, and establishing effective follow-up mechanisms to support early identification and management of mental health problems among secondary school students across Nigeria.
