Impact of Skilled Birth Attendance on Maternal Health Outcomes in Nigeria

Abstract

Maternal health remains an important public health concern in Nigeria, where preventable complications during pregnancy, childbirth, and the postnatal period continue to contribute substantially to maternal morbidity and mortality. Skilled birth attendance is an important strategy for improving maternal health because trained healthcare professionals can identify complications early, provide appropriate obstetric care, manage emergencies, and facilitate timely referral when advanced care is required. Skilled birth attendance refers to childbirth assisted by a trained and competent healthcare professional, such as a doctor, midwife, or appropriately trained nurse, who has the skills and resources required to provide safe delivery care. Maternal health outcomes refer to the health conditions and events experienced by women during childbirth and the immediate postnatal period, including maternal complications, postpartum haemorrhage, obstetric infections, hypertensive complications, severe maternal morbidity, referral for emergency obstetric care, and maternal survival. In Nigeria, barriers such as limited access to skilled healthcare providers, inadequate health facilities, geographical distance, financial constraints, poor transportation systems, cultural practices, and shortages of essential maternal health resources may affect women's access to skilled birth attendance. Strengthening skilled attendance at birth may therefore contribute to improved maternal health outcomes by increasing access to timely and appropriate obstetric care. Against this background, this study investigates the impact of skilled birth attendance on maternal health outcomes in Nigeria. The study will be anchored on the Health Systems Framework, Donabedian's Structure-Process-Outcome Model, and Andersen's Behavioral Model of Health Services Use. The Health Systems Framework emphasizes service delivery, health workforce, medical technologies, health information, and access to essential healthcare services as important components of effective health systems. Donabedian's Model explains how healthcare structures and care processes influence health outcomes and provides a framework for examining the relationship between skilled delivery care and maternal outcomes. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors. Collectively, these theoretical perspectives provide a suitable framework for explaining how skilled birth attendance may influence maternal health outcomes in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from women who have recently given birth and from selected healthcare facilities providing maternity services across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, communities, and eligible women. Skilled birth attendance will be assessed using indicators such as attendance by a qualified doctor, midwife, or nurse, availability of skilled personnel during labour and delivery, use of appropriate delivery procedures, monitoring of labour, availability of essential obstetric equipment, ability to identify complications, availability of emergency obstetric services, and timely referral where required. Maternal health outcomes will be assessed using indicators such as postpartum haemorrhage, hypertensive complications, obstetric infections, prolonged or obstructed labour, severe maternal morbidity, emergency referral, admission following delivery complications, and maternal survival. Structured questionnaires, facility assessment checklists, and maternal health records will be used for data collection. Descriptive statistics will be used to summarize participants' characteristics, skilled birth attendance patterns, and maternal health outcomes. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of skilled birth attendance on maternal health outcomes. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that skilled birth attendance has a significant positive impact on maternal health outcomes in Nigeria. Women who receive skilled assistance during childbirth are expected to experience fewer preventable maternal complications and improved access to timely obstetric interventions than women who give birth without skilled attendance. Skilled healthcare providers can monitor labour, recognize danger signs, manage complications such as postpartum haemorrhage and hypertensive emergencies, provide appropriate treatment, and initiate timely referral when advanced care is required. The availability of skilled birth attendants may also increase adherence to safe delivery practices and improve infection prevention during childbirth. Conversely, delivery without skilled assistance may increase the likelihood of delayed recognition and management of obstetric complications. However, skilled birth attendance alone may not fully determine maternal health outcomes because the availability of essential medicines and equipment, emergency transportation, blood transfusion services, referral systems, facility readiness, distance to healthcare facilities, and the quality of obstetric care may also influence outcomes. The study is expected to contribute to the literature on skilled birth attendance, maternal health outcomes, maternal morbidity and mortality, safe childbirth, reproductive health, maternal healthcare utilization, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, National Primary Health Care Development Agency, hospitals, primary healthcare facilities, midwifery services, maternal health programme managers, healthcare workers, development partners, and policymakers regarding strategies for improving maternal health. The study will also provide evidence-based recommendations for increasing access to skilled birth attendants, strengthening maternity services, improving healthcare-worker capacity, ensuring availability of essential obstetric equipment and medicines, strengthening emergency referral systems, improving transportation for obstetric emergencies, and expanding quality maternal healthcare services across Nigeria.
