Impact of Telemedicine Awareness on Telemedicine Service Utilization among Adults in Rural Nigeria

 

Abstract

Telemedicine has emerged as an important approach to improving access to healthcare by enabling patients to receive health information, consultation, assessment, follow-up, and selected healthcare services through telecommunications and digital technologies. It may be particularly valuable in rural communities where geographical distance, transportation difficulties, shortage of healthcare professionals, and limited availability of healthcare facilities may restrict access to conventional healthcare services. Despite its potential benefits, telemedicine utilization among adults in rural Nigeria may remain limited because of inadequate awareness, low digital literacy, concerns about privacy and confidentiality, limited knowledge of available platforms, poor internet and mobile network connectivity, cost of digital services, and uncertainty about the quality and reliability of remote healthcare. Telemedicine awareness may improve adults' understanding of available services, benefits, limitations, access procedures, and appropriate situations for seeking remote healthcare. Against this background, this study investigates the impact of telemedicine awareness on telemedicine service utilization among adults in rural Nigeria. The study will be anchored on the Technology Acceptance Model, Health Belief Model, and Andersen's Behavioral Model of Health Services Use. The Technology Acceptance Model explains how perceived usefulness and perceived ease of use may influence adults' willingness to adopt and utilize telemedicine services. The Health Belief Model explains how individuals' perceptions of health needs, perceived benefits of telemedicine, perceived barriers, and cues to action may influence their decision to use remote healthcare services. Andersen's Behavioral Model explains how predisposing characteristics, enabling resources, and perceived healthcare needs influence healthcare utilization. Collectively, these theoretical perspectives provide a suitable framework for explaining how telemedicine awareness may influence telemedicine service utilization among adults in rural Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise adults aged 18 years and above residing in selected rural communities in Nigeria. A multistage sampling technique will be used to select states, local government areas, rural communities, households, and eligible adults. Telemedicine awareness will be assessed using indicators such as awareness of telemedicine, knowledge of available telemedicine platforms, understanding of teleconsultation, awareness of remote diagnosis and follow-up services where applicable, knowledge of how to access telemedicine, awareness of the benefits and limitations of telemedicine, knowledge of privacy and confidentiality measures, awareness of cost, and knowledge of situations appropriate for telemedicine. Telemedicine service utilization will be assessed using indicators such as previous use of teleconsultation, frequency of telemedicine use, registration on telemedicine platforms, use of remote medical consultations, use of digital follow-up services, utilization of remote health information, referrals through telemedicine platforms, and willingness to use telemedicine services when healthcare needs arise. Data will be collected using structured questionnaires, telemedicine utilization records where available, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, levels of awareness, and patterns of telemedicine utilization. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of telemedicine awareness on telemedicine service utilization. Where a quasi-experimental design is adopted, telemedicine utilization before and after an awareness intervention may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that telemedicine awareness has a significant positive impact on telemedicine service utilization among adults in rural Nigeria. Adults who have adequate knowledge of telemedicine services are expected to have a higher likelihood of utilizing teleconsultation, remote follow-up, and other appropriate digital healthcare services than adults with limited awareness. Increased awareness may help individuals understand how telemedicine works, identify healthcare situations suitable for remote consultation, and overcome misconceptions about digital healthcare. Awareness programmes may also improve confidence in using telemedicine platforms and encourage individuals to seek professional healthcare advice without necessarily travelling long distances for every consultation. However, poor internet connectivity, limited mobile network coverage, lack of smartphones or suitable devices, digital illiteracy, cost of data services, concerns about privacy, limited availability of healthcare providers, and distrust of remote healthcare may continue to constrain utilization. The study therefore expects accessible, culturally appropriate, and practical telemedicine awareness programmes, combined with adequate digital infrastructure and reliable service delivery, to contribute significantly to improved telemedicine utilization among adults in rural Nigeria. The study is expected to contribute to the literature on telemedicine awareness, telemedicine service utilization, digital health, rural healthcare access, telehealth adoption, healthcare utilization, digital literacy, primary healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, telemedicine providers, telecommunications stakeholders, digital health organizations, development partners, and policymakers regarding strategies for expanding digital healthcare services in underserved communities. The study will also provide evidence-based recommendations for strengthening telemedicine awareness campaigns, improving digital health literacy, expanding reliable internet and mobile connectivity, increasing access to appropriate digital devices, addressing privacy and confidentiality concerns, training healthcare providers, and integrating telemedicine into rural primary healthcare services across Nigeria.
