Influence of Community-Based Cognitive Screening on Cognitive Impairment Detection among Older Adults in Nigeria

 

Abstract

Cognitive impairment remains an important public health concern among older adults in Nigeria and may affect memory, attention, thinking, communication, decision-making, independence, and overall quality of life. Cognitive decline may develop gradually, and early changes are sometimes overlooked or considered a normal part of ageing, resulting in delayed recognition and healthcare assessment. Undetected cognitive impairment may affect older adults' ability to manage daily activities, medications, finances, healthcare decisions, and social relationships and may increase the burden placed on families and caregivers. Older adults may also face barriers to cognitive assessment, including limited awareness, stigma, financial constraints, geographical inaccessibility, shortage of trained healthcare professionals, and limited availability of geriatric and neurological services. Community-based cognitive screening provides an opportunity to identify older adults with possible cognitive difficulties through accessible screening activities conducted within communities, primary healthcare centres, elderly health programmes, mobile health services, and other community platforms. Early detection may facilitate comprehensive clinical assessment, identification of potentially reversible causes, appropriate management, caregiver education, and referral for specialist evaluation where necessary. Against this background, this study investigates the influence of community-based cognitive screening on cognitive impairment detection among older adults in Nigeria. The study will be anchored on the Health Belief Model, the International Classification of Functioning, Disability and Health (ICF) Framework, and the Health Systems Framework. The Health Belief Model explains how older adults' perceptions of susceptibility to cognitive impairment, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community-based cognitive screening. The ICF Framework emphasizes the effects of cognitive impairment on body functions, activities, participation, environmental factors, and personal factors, thereby providing a framework for understanding the functional consequences of cognitive decline among older adults. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective community-based cognitive screening and referral programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based cognitive screening may influence cognitive impairment detection among older adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adults aged 60 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible older adults. Community-based cognitive screening will be measured using indicators such as availability of screening services, screening coverage, frequency of screening activities, accessibility of screening locations, use of validated cognitive screening instruments, assessment of memory and other cognitive functions, assessment of functional difficulties, availability of trained healthcare personnel, cognitive health education, caregiver involvement, and referral arrangements. Cognitive impairment detection will be assessed using indicators such as abnormal cognitive screening results, difficulties in memory, attention, orientation, language, or executive functioning, newly identified cognitive impairment, severity of cognitive impairment, functional limitations associated with cognitive decline, referrals for comprehensive clinical assessment, specialist evaluation where available, and linkage to appropriate healthcare and support services. Data will be collected using structured questionnaires, community screening registers, cognitive assessment forms, healthcare facility records, referral registers, caregiver reports, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, cognitive assessment findings, and patterns of cognitive impairment detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of community-based cognitive screening on cognitive impairment detection. Where appropriate, cognitive impairment detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based cognitive screening has a significant positive influence on cognitive impairment detection among older adults in Nigeria. Older adults who participate in community-based cognitive screening are expected to have a higher likelihood of having previously unrecognized cognitive impairment identified than older adults without access to such services. Community-based screening may facilitate early recognition of memory, attention, orientation, and other cognitive difficulties and provide opportunities for comprehensive clinical assessment and investigation of potentially reversible causes. Early detection may support appropriate management, cognitive health education, safety planning, caregiver support, and referral to specialist services where necessary. Bringing cognitive screening services closer to older adults may reduce geographical, financial, and time-related barriers and encourage individuals who might otherwise not seek assessment to participate in preventive health services. Community-based cognitive health education may also improve awareness among older adults, families, and caregivers regarding early warning signs, appropriate healthcare-seeking behaviour, and the importance of timely assessment. Conversely, inadequate screening coverage, shortage of trained healthcare personnel, limited specialist services, low awareness, stigma, financial constraints, cultural misconceptions, and weak referral and follow-up systems may reduce the effectiveness of community-based cognitive screening. The study therefore expects accessible and well-organized community-based cognitive screening to contribute significantly to improved detection and early management of cognitive impairment among older adults in Nigeria. The study is expected to contribute to the literature on community-based cognitive screening, cognitive impairment detection, older adult health, cognitive health, healthy ageing, geriatric healthcare, neurological health, community health services, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, geriatric health programmes, neurologists, psychiatrists, psychologists, community health workers, caregivers, development partners, and policymakers regarding strategies for improving early recognition of cognitive impairment among older adults. The study will also provide evidence-based recommendations for expanding community-based cognitive screening programmes, improving access to validated cognitive assessment tools, strengthening training of healthcare workers in cognitive screening, increasing public and caregiver awareness of cognitive impairment, reducing stigma, strengthening referral and follow-up systems, improving access to specialist assessment and supportive services, and integrating cognitive health assessment into appropriate community-based healthy ageing programmes across Nigeria.
