Influence of Community-Based Mental Health Screening on Depression Detection among Adults in Nigeria

 

Abstract

Depression is a common mental health condition that can substantially affect emotional wellbeing, social functioning, productivity, and overall quality of life. Despite its public health importance, depression remains underdiagnosed in many communities because of limited mental health services, inadequate awareness, stigma, shortage of trained mental health professionals, and poor access to screening and diagnostic services. These challenges may be particularly significant in underserved Nigerian communities where mental healthcare is often concentrated in specialized or urban healthcare facilities. Community-based mental health screening provides an opportunity to identify adults who may have symptoms of depression within their communities and facilitate timely referral for further assessment and appropriate care. Such screening may be conducted through primary healthcare facilities, community outreach programmes, trained community health workers, health campaigns, or other community-based platforms using appropriate validated screening instruments. Depression detection refers to the identification of adults with possible depressive symptoms through systematic screening and subsequent clinical assessment where required. Against this background, this study investigates the influence of community-based mental health screening on depression detection among adults in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Health Systems Framework. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action and provides a framework for understanding participation in mental health screening. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a basis for understanding how accessible screening services may improve detection of depression. The Health Systems Framework emphasizes service delivery, health workforce, health information, health technologies, financing, and access to healthcare as important components of an effective mental health system. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based mental health screening may influence depression detection among adults in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from adults aged 18 years and above residing in selected communities across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, primary healthcare facilities, community screening points, and eligible participants. Community-based mental health screening will be assessed using indicators such as availability of screening services, frequency of screening activities, accessibility of screening locations, availability of trained screening personnel, use of validated screening instruments, privacy and confidentiality, screening coverage, community awareness, referral services, follow-up procedures, and continuity of screening activities. Depression detection will be assessed using indicators such as identification of adults with possible depressive symptoms, completion of standardized screening, referral for clinical assessment, confirmed diagnosis where appropriate, and time between screening and further assessment. Structured questionnaires, screening records, referral registers, primary healthcare records, and relevant programme documents will be used for data collection where available. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, and depression detection patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the influence of community-based mental health screening on depression detection. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that community-based mental health screening has a significant positive influence on depression detection among adults in Nigeria. Adults who have access to regular and accessible community-based screening are expected to have a greater likelihood of having depressive symptoms identified than adults who rely exclusively on conventional mental healthcare services. Community screening may reduce geographical and financial barriers and provide opportunities for early identification among individuals who may not otherwise seek mental healthcare. Screening conducted through trusted community and primary healthcare platforms may also increase awareness of depression and reduce misconceptions surrounding mental health. Early identification may facilitate timely referral for comprehensive assessment and appropriate management. Conversely, inadequate screening coverage, shortages of trained personnel, limited referral services, concerns about confidentiality, stigma, and insufficient mental health resources may reduce the effectiveness of community-based screening. However, community-based screening alone may not fully determine depression detection because individuals' awareness of mental health, willingness to disclose symptoms, socioeconomic circumstances, stigma, availability of clinical assessment services, and healthcare-seeking behaviour may also influence detection. The study is expected to contribute to the literature on community-based mental health screening, depression detection, mental health services, community health, primary healthcare, epidemiology, health promotion, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, National Primary Health Care Development Agency, mental health programme managers, primary healthcare facilities, community health workers, mental health professionals, development partners, and policymakers regarding strategies for improving early identification of depression. The study will also provide evidence-based recommendations for expanding community-based mental health screening, training primary healthcare and community health workers, increasing public awareness of depression, strengthening referral and follow-up systems, improving confidentiality and privacy during screening, integrating mental health screening into routine primary healthcare, and increasing access to appropriate mental health assessment and treatment services across Nigeria.
