Influence of Community-Based Thyroid Screening on Thyroid Disorder Detection among Adults in Nigeria

 

Abstract

Thyroid disorders remain an important public health concern among adults in Nigeria and include conditions such as hypothyroidism, hyperthyroidism, thyroid nodules, goitre, and other abnormalities of thyroid function and structure. These conditions may affect metabolism, cardiovascular health, reproductive health, mental well-being, physical functioning, and overall quality of life. Thyroid disorders may remain undetected because some symptoms are mild, nonspecific, or mistaken for other health conditions, resulting in delayed diagnosis and treatment. Limited awareness, inadequate access to diagnostic services, financial constraints, geographical barriers, and shortages of trained healthcare personnel may further contribute to underdiagnosis of thyroid disorders. Community-based thyroid screening provides an opportunity to bring thyroid health assessment closer to adults through community outreach programmes, primary healthcare centres, mobile screening services, health campaigns, and other community platforms. Screening may include assessment of thyroid-related symptoms and risk factors, physical examination, thyroid-stimulating hormone testing, other thyroid function tests, and appropriate referral for further evaluation where indicated. Early detection may facilitate timely treatment, monitoring, and management of thyroid disorders and reduce the risk of complications. However, inadequate screening coverage, limited laboratory capacity, insufficient screening equipment, shortage of trained healthcare workers, low public awareness, financial barriers, and weak referral and follow-up systems may limit the effectiveness of community-based thyroid screening programmes in Nigeria. Against this background, this study investigates the influence of community-based thyroid screening on thyroid disorder detection among adults in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how adults' perceptions of susceptibility to thyroid disorders, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community-based thyroid screening. Community Health Theory emphasizes the delivery of accessible preventive health services through community structures and active community participation in health promotion, disease prevention, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective thyroid screening and healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based thyroid screening may influence thyroid disorder detection among adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adults aged 18 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible adults. Community-based thyroid screening will be measured using indicators such as availability of screening services, screening coverage, frequency of community screening activities, accessibility of screening locations, assessment of thyroid-related symptoms and risk factors, thyroid function testing, physical thyroid examination, availability of trained healthcare personnel, laboratory facilities, health education, and referral arrangements. Thyroid disorder detection will be assessed using indicators such as abnormal thyroid-stimulating hormone results, abnormal thyroid hormone levels, newly identified hypothyroidism, hyperthyroidism, thyroid nodules, goitre, other suspected thyroid abnormalities, confirmed thyroid disorders following appropriate clinical assessment, referrals for further diagnostic evaluation, and linkage to treatment and follow-up care. Data will be collected using structured questionnaires, community screening registers, laboratory reports, thyroid assessment forms, healthcare facility records, referral registers, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, thyroid assessment findings, and patterns of thyroid disorder detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of community-based thyroid screening on thyroid disorder detection. Where appropriate, thyroid disorder detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based thyroid screening has a significant positive influence on thyroid disorder detection among adults in Nigeria. Adults who participate in community-based thyroid screening are expected to have a higher likelihood of having previously undetected thyroid abnormalities identified than adults without access to such services. Community-based screening may facilitate early identification of abnormal thyroid function and suspected structural abnormalities and provide opportunities for confirmatory testing, clinical evaluation, treatment, monitoring, and referral to specialist services where necessary. Bringing thyroid screening closer to adults may reduce geographical, financial, and time-related barriers and encourage individuals who may not routinely seek healthcare to participate in preventive health assessments. Early detection may also reduce the risk of complications associated with untreated thyroid disorders and improve opportunities for timely management. Community-based thyroid health education may further increase awareness of symptoms, risk factors, and the importance of seeking appropriate medical evaluation. Conversely, inadequate screening coverage, limited laboratory capacity, shortage of trained healthcare personnel, low awareness, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of community-based thyroid screening. The study therefore expects accessible and well-organized community-based thyroid screening to contribute significantly to improved detection and early management of thyroid disorders among adults in Nigeria. The study is expected to contribute to the literature on community-based thyroid screening, thyroid disorder detection, adult health, thyroid health, endocrine disorders, preventive healthcare, community health services, health promotion, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, endocrinologists, laboratory professionals, community health workers, development partners, and policymakers regarding strategies for improving early detection and management of thyroid disorders. The study will also provide evidence-based recommendations for expanding community-based thyroid screening programmes, improving access to appropriate thyroid function testing, strengthening community education on thyroid disorders, increasing availability of trained healthcare and laboratory personnel, improving referral and follow-up systems, reducing financial and geographical barriers to screening, strengthening linkage to treatment, and promoting early detection and management of thyroid disorders among adults across Nigeria.
