Influence of Hospital Waiting Area Conditions on Patient Satisfaction in Nigerian Public Hospitals

 

Abstract

The physical environment in which healthcare services are delivered can influence patients' perceptions of service quality, comfort, dignity, safety, and overall healthcare experience. Hospital waiting areas constitute an important component of the patient environment because patients may spend considerable periods waiting for registration, consultation, diagnosis, treatment, or other healthcare services. Hospital waiting area conditions refer to the physical and environmental characteristics of waiting spaces, including cleanliness, seating adequacy, ventilation, lighting, temperature, privacy, noise levels, sanitation facilities, crowding, accessibility, and general comfort. Patient satisfaction refers to the extent to which patients' expectations and perceptions regarding healthcare services and their overall hospital experience are met. In Nigerian public hospitals, overcrowding, inadequate seating, poor ventilation, insufficient sanitation facilities, excessive noise, inadequate cleanliness, and uncomfortable waiting environments may negatively affect patients' experiences and perceptions of healthcare quality. Conversely, clean, safe, accessible, adequately furnished, and comfortable waiting areas may improve patients' perceptions of hospital services and contribute to greater satisfaction. Against this background, this study investigates the influence of hospital waiting area conditions on patient satisfaction in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, the Servicescape Theory, and Expectancy-Disconfirmation Theory. Donabedian's Model provides a framework for understanding the physical environment and infrastructure of hospitals as structural components that can influence healthcare processes and patient outcomes. Servicescape Theory emphasizes the influence of the physical service environment on users' perceptions, emotions, behaviour, and evaluation of service quality. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing patients' expectations regarding the healthcare environment and services with their actual experiences. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital waiting area conditions may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from patients attending selected Nigerian public hospitals using a structured patient questionnaire and a standardized waiting-area environmental assessment checklist. A multistage sampling technique will be employed to select states, public hospitals, departments or service units, and eligible patients. Hospital waiting area conditions will be assessed using indicators such as cleanliness, seating capacity and adequacy, ventilation, lighting, temperature, noise level, crowding, availability of sanitation facilities, accessibility for persons with disabilities, privacy, safety, information displays, and general physical comfort. Where feasible, direct observation and facility assessment will be used to objectively evaluate waiting-area conditions rather than relying solely on patient perceptions. Patient satisfaction will be assessed using indicators such as satisfaction with physical comfort, cleanliness, privacy, waiting environment, staff interaction, perceived service quality, responsiveness, and overall satisfaction with the hospital experience. Additional information will be collected on patients' age, sex, educational level, type of healthcare service received, frequency of hospital visits, waiting time, and relevant healthcare characteristics. Descriptive statistics will be used to summarize waiting-area conditions, patient characteristics, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the influence of waiting area conditions on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that favourable hospital waiting area conditions have a significant positive influence on patient satisfaction in Nigerian public hospitals. Patients attending hospitals with clean, comfortable, adequately ventilated, well-lit, safe, and appropriately furnished waiting areas are expected to report higher levels of satisfaction than patients exposed to poor waiting environments. Adequate seating, effective ventilation, appropriate temperature, reduced noise, good sanitation, and reduced overcrowding are expected to improve patients' comfort and perceptions of hospital service quality. A well-maintained waiting environment may also communicate organizational efficiency, respect for patients, and commitment to quality care. Conversely, overcrowding, poor sanitation, inadequate seating, excessive noise, poor ventilation, and uncomfortable temperatures are expected to contribute to dissatisfaction, particularly when patients experience prolonged waiting times. However, waiting area conditions alone may not fully determine patient satisfaction because satisfaction is also influenced by healthcare provider behaviour, waiting time, medicine availability, clinical outcomes, healthcare costs, and accessibility of services. Therefore, improvements in waiting environments should form part of a broader patient-centred healthcare quality strategy. The study is expected to contribute to the literature on hospital waiting environments, patient satisfaction, healthcare quality, hospital infrastructure, patient experience, healthcare management, public hospital services, and public health in Nigeria by providing empirical evidence on the relationship between the physical waiting environment and patients' perceptions of healthcare services. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, facility managers, healthcare quality-improvement teams, healthcare professionals, development partners, and policymakers regarding strategies for improving patient-centred healthcare environments. The study will also provide evidence-based recommendations for improving waiting-area cleanliness, seating capacity, ventilation, lighting, sanitation, privacy, accessibility, noise management, crowd control, and general comfort, while promoting regular facility maintenance and patient-experience monitoring in Nigerian public hospitals.
