Influence of Medical Waste Management Education on Healthcare Waste Disposal Practices among Healthcare Workers in Nigeria
Abstract
Healthcare waste management is an important component of occupational safety, environmental health, infection prevention, and public health because inappropriate handling and disposal of healthcare waste can expose healthcare workers, patients, waste handlers, visitors, and surrounding communities to infectious materials, sharps injuries, hazardous chemicals, and environmental contamination. Healthcare workers generate and handle different categories of healthcare waste, including infectious waste, sharps, pathological waste, pharmaceutical waste, chemical waste, and general non-hazardous waste. Medical waste management education provides healthcare workers with knowledge and practical skills relating to healthcare waste classification, segregation, handling, storage, transportation, treatment, disposal, sharps safety, personal protective equipment, spill management, and prevention of occupational and environmental exposure. Healthcare waste disposal practices refer to the procedures followed by healthcare workers in segregating, handling, storing, transporting, and disposing of healthcare waste according to established safety and environmental requirements. In Nigeria, inadequate waste management knowledge, insufficient waste segregation facilities, shortages of appropriate waste containers, inadequate personal protective equipment, weak supervision, poor waste-management infrastructure, and inconsistent implementation of healthcare waste policies may affect safe healthcare waste disposal practices. Effective medical waste management education may improve healthcare workers' knowledge, practical skills, risk perception, and adherence to appropriate waste disposal procedures. Against this background, this study investigates the influence of medical waste management education on healthcare waste disposal practices among healthcare workers in Nigeria. The study is anchored on Social Cognitive Theory, the Health Belief Model, and the Knowledge-Attitude-Practice framework. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, behavioural capabilities, social influences, and environmental conditions in shaping healthcare waste management behaviour. The Health Belief Model explains preventive occupational and environmental health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding healthcare workers' adoption of safe waste disposal practices. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from medical waste management education may influence healthcare waste disposal practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how medical waste management education may influence healthcare waste disposal practices among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from healthcare workers in selected public and private healthcare facilities in Nigeria using a structured self-administered questionnaire, medical waste management knowledge assessment, and an observational healthcare waste disposal checklist. A multistage sampling technique will be employed to select states, local government areas, healthcare facilities, departments or units, and eligible healthcare workers. Where feasible, selected healthcare facilities or healthcare worker groups will be assigned to intervention and comparison groups. Healthcare workers in the intervention group will receive structured medical waste management education covering healthcare waste classification, waste segregation at the point of generation, appropriate use of colour-coded waste containers, sharps management, safe handling and storage, internal transportation, treatment and disposal procedures, personal protective equipment, spill management, occupational exposure prevention, and environmental protection. Healthcare waste disposal practices will be assessed using indicators such as correct segregation of waste at the point of generation, appropriate use of designated waste containers, safe handling of sharps, proper use of personal protective equipment, appropriate temporary storage, safe transportation of waste, correct disposal procedures, and compliance with healthcare waste management guidelines. Where feasible, direct observational assessment will complement self-reported information to improve measurement accuracy. Descriptive statistics will be used to summarize healthcare workers' sociodemographic and professional characteristics, medical waste management knowledge, exposure to education, and healthcare waste disposal practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of medical waste management education on healthcare waste disposal practices. Where appropriate, pre-intervention and post-intervention disposal practice scores will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that medical waste management education has a significant positive influence on healthcare waste disposal practices among healthcare workers in Nigeria. Healthcare workers exposed to structured waste management education are expected to demonstrate improved knowledge of healthcare waste hazards and greater compliance with recommended disposal procedures. Education may improve waste segregation, correct use of designated waste containers, safe handling of sharps, appropriate use of personal protective equipment, temporary storage, internal transportation, and final disposal practices. It may also improve healthcare workers' ability to recognize the occupational and environmental consequences of inappropriate waste disposal. However, education alone may not guarantee sustained safe waste disposal practices because healthcare facilities may continue to experience barriers such as inadequate waste containers, unreliable waste collection systems, insufficient personal protective equipment, poor waste-management infrastructure, overcrowding, inadequate supervision, and limited access to appropriate waste treatment and disposal facilities. Consequently, medical waste management education is expected to be most effective when supported by adequate waste-management infrastructure, appropriate waste containers, reliable collection and disposal systems, regular monitoring, supportive supervision, and strong institutional healthcare waste management policies. The study is expected to contribute to the literature on healthcare waste management, environmental health, occupational safety, infection prevention and control, healthcare worker behaviour, health education, patient safety, and public health in Nigeria by providing empirical evidence on the influence of medical waste management education on healthcare waste disposal practices among healthcare workers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Environment, Nigeria Centre for Disease Control and Prevention, National Primary Health Care Development Agency, state ministries of health and environment, healthcare facility managers, infection prevention and control committees, environmental health officers, waste-management authorities, occupational health professionals, public health practitioners, development partners, and policymakers regarding strategies for improving healthcare waste management. The study will also provide evidence-based recommendations for strengthening medical waste management education, improving waste segregation infrastructure, ensuring continuous availability of appropriate waste containers and personal protective equipment, strengthening healthcare waste collection and disposal systems, conducting regular compliance monitoring, and promoting environmentally safe and occupationally safe healthcare waste management practices across Nigeria.
