Influence of Mobile Health Clinics on Primary Healthcare Utilization among Hard-to-Reach Communities in Nigeria

Abstract

Access to primary healthcare remains a major public health challenge in many hard-to-reach communities in Nigeria, particularly in areas affected by geographical isolation, poor road infrastructure, limited transportation, inadequate healthcare facilities, and shortages of healthcare workers. Mobile health clinics provide an alternative approach to healthcare delivery by taking essential health services directly to underserved populations that may experience difficulties accessing conventional fixed healthcare facilities. Mobile health clinics may provide services such as health screening, treatment of common illnesses, maternal and child health services, immunization, health education, family planning, medication distribution, basic laboratory services, and referral to higher-level healthcare facilities. Primary healthcare utilization refers to the extent to which individuals access and use available primary healthcare services for preventive, promotive, curative, and rehabilitative needs. In Nigeria, inadequate geographical access to healthcare facilities may contribute to delayed healthcare-seeking, unmet healthcare needs, and low utilization of essential primary healthcare services among hard-to-reach populations. Mobile health clinics may reduce geographical and transportation barriers by bringing healthcare services closer to communities. Against this background, this study investigates the influence of mobile health clinics on primary healthcare utilization among hard-to-reach communities in Nigeria. The study will be anchored on Andersen's Behavioral Model of Health Services Use, the Health Systems Framework, and the Social Ecological Model. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how improved geographical access to healthcare may influence service utilization. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical technologies, financing, and access to healthcare as important components of an effective health system. The Social Ecological Model recognizes that healthcare utilization is influenced by interacting individual, interpersonal, community, environmental, and institutional factors. Collectively, these theoretical perspectives provide a suitable framework for explaining how mobile health clinics may influence primary healthcare utilization in hard-to-reach communities in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from residents of selected hard-to-reach communities that receive or have access to mobile health clinic services across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, hard-to-reach communities, mobile clinic locations, and eligible participants. Mobile health clinic services will be assessed using indicators such as frequency of mobile clinic visits, range of healthcare services provided, availability of healthcare workers, availability of essential medicines, availability of diagnostic services, duration of service delivery, geographical coverage, community awareness of clinic schedules, accessibility, affordability, referral services, and continuity of service. Primary healthcare utilization will be assessed using indicators such as attendance at primary healthcare facilities, use of preventive services, outpatient consultations, maternal and child health services, immunization, family planning, health screening, treatment of common illnesses, and follow-up visits within a defined reference period. Structured questionnaires, mobile clinic records, community health records, and facility service records will be used for data collection where available. Descriptive statistics will be used to summarize participants' characteristics, mobile clinic service availability, and primary healthcare utilization patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the influence of mobile health clinics on primary healthcare utilization. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that mobile health clinics have a significant positive influence on primary healthcare utilization among hard-to-reach communities in Nigeria. Residents who have regular access to mobile health clinics are expected to demonstrate greater utilization of primary healthcare services than residents who depend exclusively on distant fixed healthcare facilities. Mobile clinics may reduce geographical and transportation barriers by bringing essential healthcare services closer to underserved populations. Regular mobile clinic visits may also improve access to preventive services, early disease detection, maternal and child healthcare, immunization, family planning, and treatment of common illnesses. Availability of essential medicines and basic diagnostic services through mobile clinics may further encourage community members to seek care when needed. Conversely, irregular mobile clinic schedules, limited service coverage, inadequate staffing, medicine shortages, poor road infrastructure, and weak referral systems may limit their effectiveness. However, mobile health clinics alone may not fully determine primary healthcare utilization because household income, health knowledge, cultural beliefs, perceived need for healthcare, community support, availability of fixed healthcare facilities, and transportation conditions may also influence healthcare-seeking behaviour. The study is expected to contribute to the literature on mobile health clinics, primary healthcare utilization, healthcare accessibility, rural health, hard-to-reach populations, community health, health equity, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, mobile healthcare providers, community health workers, development partners, and policymakers regarding strategies for improving healthcare access in underserved communities. The study will also provide evidence-based recommendations for expanding mobile clinic coverage, improving the frequency and reliability of mobile clinic visits, ensuring adequate staffing and essential medicine availability, strengthening community awareness and mobilization, improving referral systems, integrating mobile clinics with existing primary healthcare facilities, and developing sustainable mobile healthcare programmes for hard-to-reach communities across Nigeria.
