Influence of Oral Health Education on Dental Hygiene Practices among Secondary School Students in Nigeria

Abstract

Oral health is an important component of overall health and well-being, while poor oral hygiene among adolescents may contribute to dental caries, periodontal diseases, halitosis, tooth loss, pain, and difficulties with eating and learning. Secondary school students are an important population for oral health promotion because adolescence is a critical period for establishing behaviours that may persist into adulthood. Dental hygiene practices include regular tooth brushing, appropriate brushing technique, use of fluoride toothpaste, appropriate frequency of brushing, cleaning between teeth where appropriate, limiting excessive consumption of sugary foods and drinks, and seeking professional dental care when necessary. Oral health education is an important health promotion strategy for improving students' knowledge of oral diseases, causes, prevention, appropriate tooth-brushing techniques, dietary practices, and the importance of regular dental care. In Nigeria, inadequate oral health knowledge, limited access to oral health services, poor availability of preventive dental programmes, high consumption of sugary foods and beverages, and socioeconomic and cultural factors may negatively influence adolescents' oral hygiene practices. Effective school-based oral health education may improve students' understanding of oral health and encourage healthier dental hygiene behaviours. Against this background, this study investigates the influence of oral health education on dental hygiene practices among secondary school students in Nigeria. The study is anchored on the Health Belief Model (HBM), Social Cognitive Theory (SCT), and Knowledge-Attitude-Practice (KAP) framework. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding students' decisions to adopt appropriate oral hygiene practices. Social Cognitive Theory emphasizes the interaction between knowledge, behavioural capability, personal beliefs, observational learning, self-efficacy, and environmental influences in shaping health behaviours. The Knowledge-Attitude-Practice framework provides a basis for examining how improved oral health knowledge and attitudes may translate into appropriate dental hygiene practices. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how oral health education may influence dental hygiene practices among secondary school students in Nigeria. The study adopts a quantitative quasi-experimental research design. Primary data will be collected from students in selected public and private secondary schools in Nigeria using a structured, self-administered questionnaire and, where feasible, an oral hygiene observation checklist or appropriate clinical assessment conducted by qualified dental personnel. A multistage sampling technique will be employed to select states, local government areas, schools, classes, and eligible students. Where feasible, selected schools will be assigned to intervention and comparison groups. Students in the intervention group will receive structured oral health education covering causes and prevention of dental caries and gum disease, correct tooth-brushing techniques, appropriate brushing frequency, fluoride toothpaste use, dietary practices, reduction of excessive sugar consumption, oral hygiene maintenance, and the importance of seeking professional dental care. Dental hygiene practices will be assessed using indicators such as frequency and duration of tooth brushing, brushing technique, use of fluoride toothpaste, cleaning between teeth, dietary practices related to oral health, and utilization of dental services when necessary. Descriptive statistics will be used to summarize students' sociodemographic characteristics, oral health knowledge, and dental hygiene practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be employed to determine the influence of oral health education on dental hygiene practices. Where clinical assessment is included, appropriate oral health indices may be used by trained dental personnel. Diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, and robustness of the findings. The study anticipates that oral health education will have a significant positive influence on dental hygiene practices among secondary school students in Nigeria. Students exposed to structured oral health education are expected to demonstrate improved knowledge of oral diseases and greater adherence to recommended dental hygiene practices. Education may encourage regular tooth brushing, appropriate use of fluoride toothpaste, improved brushing technique, healthier dietary choices, reduced consumption of sugary foods and beverages, and greater willingness to seek professional dental care. However, oral health education alone may not guarantee sustained behavioural change because students may face barriers such as limited access to dental services, lack of oral hygiene materials, household socioeconomic constraints, inadequate parental supervision, school environmental factors, and established behavioural habits. Consequently, school-based oral health education is expected to be more effective when supported by parental involvement, availability of appropriate oral hygiene materials, accessible preventive dental services, and supportive school health policies. This study is expected to contribute to the literature on adolescent health, oral health promotion, school health education, dental hygiene, and public health in Nigeria by providing empirical evidence on the relationship between oral health education and dental hygiene practices among secondary school students. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, dental health professionals, school administrators, teachers, parents, community health workers, dental associations, and policymakers regarding strategies for improving adolescent oral health. The study will also provide evidence-based recommendations for strengthening school-based oral health education, integrating oral health promotion into school health programmes, improving access to preventive dental services, encouraging parental participation in adolescent oral hygiene, promoting appropriate use of fluoride toothpaste, and reducing preventable oral health problems among secondary school students across Nigeria.
